If yu have never attended the University f Nrthern Clrad, yu must apply t UNC Frmal applicatins t the University are fund at Apply as an undergraduate transfer student seeking nursing. If yu are presently enrlled at the University f Nrthern Clrad r were recently enrlled and withdrew in gd academic standing, yu need nly apply t the Schl f Nursing. If yu graduated with yur bachelr s degree frm UNC r it has been mre than ne year since yu have attended, yu will need t cmplete the Student Infrmatin Frm thrugh the UNC Office f the Registrar t reactivate yur student accunt. TRANSCRIPTS: Yu must submit fficial transcripts frm all schls previusly attended after high schl graduatin t UNC if yu have nt previusly dne s. These transcripts must: Include prf f previus degree (if applicable) Include Interim semester grades (if applicable) Include Fall 2018 Semester grades (if applicable) They must be sent t: Office f Admissins, Carter Hall 3006, Campus Bx 10, Greeley, CO r ed t UNC.transcripts@unc.edu. STEP 2 - Applicatin t the Schl f Nursing Applicatins must be submitted thrugh NursingCAS. N hard cpy applicatins will be accepted. DUE DATE fr submissin f the cmpleted applicatin and ALL MATERIALS is Sunday, JANUARY 27, 2019 by midnight Muntain Time (MT). Yu may submit yur applicatin any time befre the due date nce all f yur materials are upladed and the applicatin is cmplete and all Fall 18/Interim curse grades are in. Yu will have the pprtunity t review yur nline applicatin prir t final submissin. Yu will receive a cnfirmatin that yur applicatin has been received. Please d nt call the Schl f Nursing t verify that yur applicatin was received. N requests fr an applicatin extensin will be granted. Repeat applicants must submit all required dcuments.

2 A COMPLETE SCHOOL OF NURSING APPLICATION INCLUDES THE FOLLOWING: 1) Applicatin a) All applicatins must be submitted thrugh NursingCAS. All questins must be answered and all apprpriate dcuments must be upladed t submit yur applicatin. Set up an accunt with NuringCAS. Please g t t create an accunt and start the applicatin fr UNC Secnd-degree r Traditinal BSN prgram. Yu may NOT apply t bth prgrams, yu must chse ne r the ther. The cst t apply t the prgram thrugh NursingCAS is $50.00 and will be paid t NursingCAS, nt the Schl f Nursing thrugh their nline prtal. (NursingCAS discunts the price f applicatin if yu apply t mre than ne prgram the cst fr any additinal prgram is $35.00). b) Resume Uplad a cpy f yur resume, using the fllwing frmat. N mre than 1 page in length. N smaller than 11 pint fnt. The resume is used nly in the event f a tie in admissins scres. c) Persnal Essay Uplad yur essay answering the fllwing statement: 1. Beynd yur academic abilities, describe ONE attribute yu bring t the nursing prfessin. NOTE: The essay is limited t 1 page, single-spaced, 12-pt fnt with 1 inch margins, n lnger than 400 wrds. Essays ver this limit will disqualify yur applicatin. NOTE: Yu may uplad a previusly submitted essay t yur current applicatin if yu applied t the BSN prgram in previus applicatin cycles. Please ensure that yu update the essay respnse t reflect yu at this current time in yur career jurney. d) Verificatin f Enrllment fr Uncmpleted Prerequisite Curses Yu may nly be enrlled in a maximum f 4 prerequisite curses at the time f applicatin. All prerequisite curses must be cmplete by May 10 th, 2019 with a grade f C r higher. Uplad an fficial r unfficial transcript verifying enrllment in any uncmpleted prerequisite curses during the Spring 2019 semester. Curses taken ver a trimester r quarter system that are nt cmpleted by May 10 th are nt acceptable fr admissin t either summer r fall semester starts. ALL prerequisites fr ALL students admitted fr summer r fall 2019 must be cmpleted by May 10 th, 2019.

3 e) Standardized Letters f Recmmendatin (SLORs) Yu must have 2 standardized letters f recmmendatin cmpleted fr yur applicatin- ne frm an academic reference and ne frm a supervisry reference. The letters f recmmendatin will be in the frm f a survey that will be sent t yur reference(s) requesting infrmatin abut yur ability t wrk with thers, cmmunicatin, flexibility, prblem-slving skills, and ther prfessinal attributes yu pssess n a 1-5 pint scale. The recmmendatin shuld nt take lnger than 10 minutes fr yur reference t cmplete. Academic Reference: The academic reference must be frm smene wh has instructed a curse yu have taken in the past 3 years. It culd be frm an instructr yu had in high schl, cllege, r a vcatinal schl. CNA curses wuld be acceptable. It can be frm an nline r n-site curse. If yu have nt taken a curse in the past 3 years, then yu can submit 2 supervisry references. Supervisry Reference: The supervisry reference shuld cme frm smene wh has had direct versight f wrk that yu have dne in the past. This culd cme frm an emplyer, wrk related supervisr, a vlunteer crdinatr, cach, military cmmander, etc. The recmmendatins may NOT cme frm anyne wh currently wrks in the UNC Schl f Nursing. This includes the pre-nursing advisr, prgram advisr, faculty (including faculty wh teach prerequisite curses) r Student Nurses Assciatin advisrs. Recmmendatins may als NOT cme frm ther students, friends, r family. Directins n hw t cmplete the Standardized Letters f Recmmendatin (SLORs) will be prvided thrugh NursingCAS. Yu may nt submit mre than 2 recmmendatins. f) HESI Admissin Assessment Cmpletin f a HESI Admissin assessment is required fr applicatin t the nursing prgram. Fur assessment areas/categries will be required: Reading Cmprehensin Vcabulary and General Knwledge Basic Math Skills Anatmy and Physilgy Yu may nly take the exam nce within the windw f Nv 1, 2018 and Friday, January 25 th, 2019, by 5pm. (Nte- If yu have taken the HESI exam after Nv 1, 2017 and befre Nv 1, 2018, see belw fr instructins fr hw t submit yur previus scres). The cst fr the UNC HESI Admissin Assessment is: $42.00

4 Directins fr taking the HESI Admissin Assessment: At UNC If yu are able t take the test at UNC, please dwnlad the instructins fr HESI ecmmerce Student Purchase Instructins (attached t the website applicatin materials). HESI Student Payment and Registratin Link: Payment ID: Once yu have purchased the HESI assessment with the UNC specific infrmatin, yu will need t call UNC s Career Services testing center t set up a time t take the assessment, Plan n 1 hr fr each sectin yu need t take (4 hurs fr the entire UNC assessment). The fee t take the assessment at UNC s testing center is $ When yu arrive t take yur assessment, yu will need t bring yur cnfirmatin f purchase fr the exam and a gvernment issued pht ID with a signature. Yu will als need t bring with yu yur username and passwrd fr yur Evlve student accunt. Yu will nt be allwed t test if there is any discrepancy between what is listed n yur Evlve student accunt and yur pht ID. If yu require testing accmmdatins, yu will need t cntact the Disabilities Supprt Services (DSS) department at UNC ( ; diabilitysupprt@unc.edu) wh will submit a request fr accmmdatins t the testing center. Yu may nt request accmmdatins fr the exam n the day f the exam, but must g thrugh the DSS ffice. If yu need accmmdatins, please make sure t be sensitive t the time it may take t secure accmmdatins befre yu sit fr the HESI exam. Once yu finish yur exam, yur scres will autmatically be sent t UNC. Yu d nt need t d anything further t send us yur scres. At a distance (nt at UNC) If yu are taking the test frm a distance, please dwnlad the instructins fr HESI- UNC-A2-DISTANCE TESTING (attached t the website applicatin materials). Yu will need t purchase the HESI exam fr UNC and take it thrugh a third party vendr. Specifics fr taking the exam are utlined in the attached instructins frm Elsevier. Yu will purchase yur assessment and then be sent infrmatin t set up yur assessment thrugh Prmetric, a third party testing center with varius lcatins thrughut the cuntry. Please nte that yu will nt use the payment ID nted abve fr testing at UNC, yu will be prvided a different department ID that is listed in the instructins. Once yu finish yur exam, yur scres will autmatically be sent t UNC. Yu d nt need t d anything further t send us yur scres. Previus HESI Examinatin

5 If yu have already taken the HESI assessment fr UNC r anther nursing prgram after Nvember 1, 2017 (ne calendar year frm this applicatin), yu may send us yur scres frm the previus exam. Please dwnlad the instructins fr HESI_Transcript_Request_Frm (attached t the website applicatin materials) and submit the frm t have yur scres sent t UNC. Yu may NOT send us the scres directly. The scres must cme directly frm Elsevier t be included in yur applicatin. On the frm, please include the fllwing under Transcript Prcessing: a. If yu tk the HESI assessment fr anther prgram that did nt include all 4 categries required by UNC, yu may send us yur scres frm yur previus assessment and take the remaining categries thrugh the UNC HESI assessment. When yu sit fr the UNC HESI assessment, yu will register fr the exam as abve (either at UNC r at a distance), but will nly cmplete the prtins f the assessment that yu still need t cmplete. g) English Prficiency requirement If yur primary language is nt English, r if yu are an Internatinal student, yu will need t include in yur applicatin demnstratin f English prficiency.
