Print ut the Fur Page 2018 RT Applicatin Frm available n the RT website (Please print in the LANDSCAPE frmat and print One Sided Prtrait applicatins will nt be accepted). The applicant must fill ut pages 1 & 4 nly; the Rad Tech Selectin Cmmittee will cmplete pages 2 & 3 during the review f the submitted applicatins. 2. Review all f the infrmatin cntained within this 2018 RT Prgram Applicatin Infrmatin Packet. This packet prvides an explanatin f what is required and hw t file an applicatin t the RT prgram. 3. Review the 2018 RT Prgram Applicatin Checklist and Wrksheet sectin (Pages 4 & 5 f this dcument) t learn hw t earn the additinal pints n the applicatin. Use the checklist prvided t be sure that yu have submitted all f the required dcuments fr the applicatin. Missing infrmatin r incmplete applicatins will nt be cnsidered fr review. 4. All required pre-requisite curse wrk must be cmpleted with a grade f C r better and must be finished by the spring semester fr the applicatin year. 5. Make an appintment t meet with ne f the LACC cunselrs t btain clearance that all general educatin curses have been cmpleted. Please btain dcumentatin in frm f a Student Educatin Plan that all general educatin curses have been cmpleted and submit it with yur applicatin. 6. Yu must cmplete all required dcumentatin and submit an applicatin t the prgram prir t the deadline f March 31, :00pm. 7. Please submit sealed fficial transcripts alng with yur paper applicatin directly t the mailrm in a sealed envelpe and write attentin t Israel Fnseca n the envelpe. All f yur fficial transcripts must demnstrate cmpletin f the seven prerequisite curses, general educatin curses and any additinal ptinal curses yu have cmpleted. LACC transcripts are required t be submitted as well. Students wh are taking in-prgress curses must submit all fficial transcripts at the end f the spring 2018 semester (Deadline: June 15, :00pm. N late r pstmarked transcripts will be accepted) 8. Yur applicatin will be reviewed by the Rad Tech Selectin Cmmittee fr evaluatin and ranking. Page 4 f the RT paper applicatin and yur supprting dcumentatin will be reviewed t determine hw many additinal pints yu may be awarded by the Rad Tech Selectin Cmmittee. 9. In additin t the paper applicatin, yu must cmplete the nline RT Applicatin that will be lcated n the Prgram page f the LACC RT Prgram website. Yu will be ntified f yur ranking range via yur Ls Angeles City Cllege address by Mr. Fnseca n r befre August 3, Keep a cpy f yur dcuments, yur applicatin and supprting dcuments will nt be returned.

2 2018 Radilgic Technlgy Applicatin Infrmatin Page 2 f 7 Applicatin Instructins T file an Applicatin t the Radilgic Technlgy Prgram fr fall 2018 yu must cmplete the fllwing requirements: 1. Yu must be registered as a Ls Angeles City Cllege student, btain an LACC student ID # and an LACC address fr the applicatin. 2. Yu may register t LACC nline please fllw the instructins under the RT Prgram Applicatin Wrksheet Checklist sectin (Pages 4 & 5 f this dcument) 3. Fill ut the Wrksheet Checklist t ensure yu have submitted all required dcuments fr the applicatin and keep a cpy f the cmpleted dcument. 4. All required prerequisite curses (including the general educatin requirements) must be cmpleted with a minimum grade f C t file an applicatin. (See RT Applicatin pg. 2 fr details). 5. Read this entire Applicatin Infrmatin packet and all frms carefully, t be sure that yu can receive the maximum pints pssible. 6. Cllect all the necessary paperwrk required t be eligible t receive all pints pssible n the Radilgic Technlgy Applicatin. 7. Be sure t submit all required applicatin dcuments, (keep a cpy f all dcuments submitted) 8. Any dcuments submitted after the March 31, :00pm deadline will NOT be cnsidered. 9. Make cpies f any supprting dcuments yu submit (ex: a cpy f yur XT Certificate nt the riginal certificate) 10. The 2018 applicatin and supprting dcuments will nt be returned. Other Requirements: 1. Scial Security Number: The state f Califrnia, Department f Public Health, Radilgic Health Branch requires a graduate frm an accredited Diagnstic Radilgic Technlgy prgram t have a valid scial security number t be eligible t take the licensing exams fr Radilgy and Flurscpy. In additin, a scial security number is required t cmplete the mandatry backgrund check prir t acceptance int the prgram. Radilgic Technlgy applicants may be required t shw prf f a valid scial security number prir t acceptance int the radilgy prgram. Yu DO NOT have t submit a cpy f yur scial security card. Only include the last fur digits f yur number n the applicatin. 2. Pht identificatin card: Fr purpses f identificatin verificatin, applicants will be required t submit a clear cpy f a current Califrnia Driver s License (CDL) r State issued ID card with the applicatin. Accepted applicants will als be required t prvide prf f a valid Ls Angeles City Cllege ID card during the mandatry student rientatin prir t acceptance int the prgram. 3. Cver letter and Resume: As part f the applicatin prcess, each applicant is required t submit a ne page cver letter explaining their interest and cmmitment t becme a part f the Ls Angeles City Cllege Radilgic Technlgy Prgram. In additin, the applicant is required t include a ne page resume, describing at a minimum, their educatinal backgrund, wrk histry t include any health care r military experience (if any). 4. Letter f Recmmendatin: Applicants are required t submit ONE prfessinal letter f recmmendatin t the prgram with the applicatin. The letter must be written within 2 years frm the date f applicatin. Letters dated later than 2 years will nt be accepted. Letter must be dated. Character references must be n letterhead statinary and/r attached business card f the persn writing the recmmendatin letter. The letter must include a wet signature with the writer s cntact infrmatin. Prfessinal r Business references nt persnal (family r friends). Letter may be frm an emplyer, supervisr, prfessr with a lab class, r cunselr. If there is n date r signature, it will nt be accepted.

All cmpleted applicatins will be reviewed and ranked by the LACC Radilgic Technlgy Applicatin Cmmittee, based n the multi-criteria pints system n the applicatin. 2. Applicants will be ntified by the RT Prgram Directr, Israel Fnseca, r his designee via their Ls Angeles City Cllege n r befre August 3, 2018 if the applicatin has received sufficient pints t make the tp 30 r nt. 3. The tp 22 applicants will be ffered a cnditinal acceptance fr a psitin in the FALL 2018 Radilgic Technlgy Class. 4. The remaining #23-30 applicants will be ranked as pssible alternates fr the Fall 2018 class. If smene frm the tp 22 declines the psitin n r befre August 7, 2018 the next ranked applicant n the alternate list will be ffered a prvisinal psitin. 5. All tp 22 applicants will be expected t reply via t the Prgram Directr, that they either accept r decline the ffer by the deadline psted in the . The tp 22 will be expected t attend the mandatry summer new student rientatin. 6. Upn receipt f the applicant s acceptance the applicant will be prvided infrmatin abut the Mandatry Orientatin that shall be held during the Summer 2018 semester. The rientatin is tentatively scheduled fr the secnd r third week f August Failure t send an reply by the specified date, r failure t attend the mandatry summer rientatin, will result in the remval f the applicant s name frm the applicant pl. 8. Once an applicant attends the mandatry rientatin, and has successfully cmpleted all required paperwrk (backgrund check and physical health exam, etc.) the ffer cnverts frm a cnditinal placement t a cnfirmed RT Prgram placement beginning the fall 2018 semester. Previus applicatins Applicants wh have previusly applied and were deemed eligible fr entry int the LACC RT Prgram shuld clearly indicate n page 1 f the RT Applicatin all previus applicatin years. Fr students taking in-prgress curses during the spring 2018 semester the deadline t submit fficial transcripts t the LACC mailrm is June 15, 2018 at 3:00pm. It is highly recmmended that Anatmy, Physilgy & RT 202 shuld be cmpleted within 5 years f the applicatin filing date. Yu are eligible t file an applicatin if the curse is ver the 5 years, but it is strngly recmmended that these curses r their equivalent shuld be repeated t imprve the student s success in the RT Prgram. M. Review the RT Applicatin fr the areas where additinal pints can be awarded. Cllect all supprting dcumentatin necessary t prvde evidence fr eligibilty fr the additinal pints n the applicatin See the Applicatin Evaluatin Criteria Frm n Pages 6-7 fr explanatin f the pints system fr each area n the applicatin. Cmmunity Service Dcumentatin (letter r certificate) f participatin in a cmmunity service t benefit a health care related rganizatin r Vlunteer / Service Learning fr an area in the ECC Health Sciences & Athletics Divisin (ex: Avn Breast Cancer Walk, American Heart Assc., Hspice, Health Fairs, Nursing Hme, HS& A Divisin Student Vlunteer, Fd Banks, Feeding the Hmeless vlunteer, etc ) The letter must include riginal signature and applicant s name, start date and number f hurs vlunteered and examples f vlunteer duties.
