All frms must be cmpletely prcessed befre the January 25, 2019 deadline by the Admissin Cunselr fr Prgrams with Special Criteria (P.W.S.C.) Phase 1) Schedule & Take TEAS (Test f Essential Academic Skills) TEAS: is a cmputerized test f reading cmprehensin, sentence skills, science, and math prblem slving. The TEAS (6) test is the ONLY cmpnent used in the ranking and placement prcess fr the 2019 LPN t ADN cmpetitin. The TEAS (6) will be prctred n the Davidsn and Davie campus f DCCC. TEAS appintment can be scheduled nline thrugh ATI ( beginning August 13, 2018 thru January 25, 2019 (deadline). The TEAS must be cmpleted and a scre f 62 r higher must be btained by all applicants in rder t mve frward in the 2019 LPN t ADN cmpetitin. In the event f a TEAS scre tie, TEAS reading scre, TEAS math scre and date verificatin f practice was received will be used t determine admissin; the earlier submissin will be ranked higher. Multiple dates and times will be available fr the TEAS starting August 13, 2018 thrugh January 25, 2019 (deadline) hwever, the TEAS can nly be taken ONCE (n multiple attempts) at DCCC. A TEAS scre frm ther institutins are allwed ne submissins as well; with a ttal f tw (2) TEAS scre submissins per persn per 2019 LPN t ADN selectin cycle at DCCC. TEAS scres must be after February 1, 2018 t be cnsidered current and valid. LPN t ADN applicants DO NOT need t travel t DCCC t take their TEAS; bth (2) TEAS submissins can be frm ther institutins utside DCCC. The TEAS test cst is seventy dllars ($70), subject t change; an additinal testing fee f twenty five dllars ($25) subject t change, will be incurred n the applicant s date f testing at DCCC if applicant des nt list DCCC as the receiving institutin fr their TEAS scre when scheduling their TEAS date/time with ATI and if the applicant s name desn t appear n the ATI TEAS attendance rster at DCCC testing center. This grup f frmer DCCC PNE graduates must re-take the TEAS test. Applicants frm prgrams ther than the DCCC PNE Prgram will then be placed n the rank list frm highest t lwest TEAS scre. Tp thirty (30) applicants with the highest TEAS scres after ranking prcesses have been applied will be ffered cnditinal prgram acceptance. Remaining students that met the TEAS benchmark scre (62 r higher) will be placed n an alternate list in ranked rder. Alternates will be called in that rder shuld space becme available. Applicants wh remain n the alternate list will nt be carried ver t a waiting list fr the fllwing year and must renew the intent prcess fr cnsideratin the fllwing year. July 2019 LPN TO ADN OPTION FACTS: Students must pssess effective cmputer skills, have an active address and access t a cmputer meeting the minimum standards fr nline educatin and be willing and able t purchase and install apprpriate sftware. The ptin begins in the fall and is cmpleted at the end f the fllwing spring semester. Althugh primary instructin will be delivered nline, students are required t return t campus fr specified activities such as rientatins and tests. (Generally campus days are limited t 3-4 days per semester.) Each f the tw LPN t ADN curses requires 144 hurs f clinical practice, UNPAID. Clinical may be with a preceptr r in an assigned clinical grup. If with a RN preceptr, this may be arranged in the student s area f residence and pssibly within the current place f wrk, but may 3

4 nt be within the same unit f emplyment. Acute care inpatient settings are the nly acceptable sites. All clinical experiences must be within Nrth Carlina and the nursing faculty will make final apprval f facilities and preceptr(s). Students are expected t initiate lcating an apprved preceptr and clinical site. Students will then wrk with the clinical crdinatr t finalize clinical arrangements. Select them frm the drp dwn list and enter yur answers (yu may select the eye icn t view yur answer). Once cmplete select Save Answers, then Cntinue n the next page. Security Questins Answers Step 4) Enter yur new passwrd fllwing the rules n the screen and select Change Passwrd nce the check mark appears (the Strength meter lets yu knw hw secure yur passwrd is and will nt fail if it says weak ). Wait fr the status bar t reach 100% then click Cntinue. The testing dr will clse five minutes befre the test starts t allw time fr testing instructins. There will be n exceptins t this plicy and test sessins will nt be delayed fr any tester. Individuals must present a valid (current, nt expired and gvernment issued) pht ID. Inclement Weather In the case f inclement weather when the Cllege is clsed, the scheduled TEAS test will be cancelled and rescheduled t anther date. If this happens, DCCC will prvide ATI with the new date as sn as pssible and testers shuld cntact ATI t reschedule their test fr a different date. DCCC Nursing Applicatin Deadline: The deadline fr all TEAS testing is Friday, January 25, 2019 at 5:00 pm. Health Evaluatin fr cnditinally accepted LPN t ADN ptin students Applicants shuld be aware that admissin t an Assciate Degree Nursing Prgram (LPN t ADN ptin) is dependent upn physical and emtinal health, cmpatible with the ability t prvide safe patient care; cnditinally accepted Assciate Degree Nursing students are prvided a cpy f the Student Health Frms t dcument evidence f satisfactry health. A physical examinatin is required within ne year f entry int the prgram. Prf f childhd immunizatins including Tetanus/Diphtheria/Pertussis (Tdap), Measles, Mumps, Rubella (MMR), Chickenpx (Varicella) and pli are required. Prf f Tetanus bster, annual seasnal influenza vaccine, Hepatitis B (three-part) vaccine, and a tw-step Tuberculin Skin Test are als required. The cst t btain the physical examinatin and required vaccinatins is the respnsibility f the student and shuld be cmpleted by the July deadline. An applicant with prblems in physical r emtinal health must prvide evidence that apprpriate treatment and/r cunseling has taken place and that the prblem has been alleviated. Persns with physical r emtinal prblems, which have nt respnded t treatment within a reasnable time, may be denied admissin t the prgram. All health frms must be cmpleted and returned t the Assciate Degree Nursing Clinical Crdinatr (Briggs Technlgy) n later than July 1, If these frms d nt appear in the student s nline immunizatin tracker accunt r are incmplete by that date, admissin int the Assciate Degree Nursing Prgram will be revked and the space will be ffered t anther applicant. Detailed infrmatin abut the due dates fr health recrds will be prvided t cnditinally accepted applicants. CRIMINAL BACKGROUND CHECK & DRUG SCREENING fr Cnditinally Accepted ADN Students Clinical sites require a criminal backgrund check and a 12-panel urine drug screen prir t participatin in clinical site rtatins. Students will be prvided instructins fr cmpleting these requirements. If any facility refuses t allw the student t participate in the clinical experience at that agency as a result f thse findings, the student will nt be able t prgress in the prgram. Inability t prgress will result in failure f the curse and remval frm the prgram. SCHEDULING OF CLASSES Enrllment in the LPN t ADN Optin will require nline attendance fr classes and clinical experience at varying times f the day r evening and varying days f the week. Travel t and frm clinical agencies will be necessary. Students are respnsible fr their wn transprtatin. All applicable DCCC academic prgrams prepare students t sit fr licensure in Nrth Carlina. In rder t cmply with U.S. Department f Educatin regulatins regarding distance educatin and prfessinal licensure, DCCC is required t make the fllwing disclsure with respect t prfessinal licensure utside the state f 6
