Due t the genersity f fans and spnsrs, and underwriting by the team, Phenix Suns Charities has experienced steady grwth and nw distributes mre than a millin dllars annually t hundreds f lcal charities. Since its inceptin, Phenix Suns Charities has dnated nearly 14 millin dllars t wrth Arizna rganizatins. Phenix Suns Charities was the visin f riginal Phenix Suns Chairman and CEO Jerry Clangel. Upn his purchase f the team in 1987, he began t assemble a bard f cmmunity leaders t versee the frmatin and peratin f a charitable effrt that wuld reflect the Suns missin t be a cnstant leader in imprving the quality f life in ur cmmunity. Current Suns managing partner, Rbert Sarver, has extended the reach f the charity by invlving additinal cmmunity stakehlders wh wrk in partnership with Phenix Suns Charities t further leverage and steward the dnatins that cme in t the fundatin. Funds are raised thrugh the NBA seasn thrugh special events such as The Jump Ball, the Suns Glf Classic; Suns Charities 88, the SLAM DUNK street party, thrugh screbard messages presented at Suns hme games; game night auctins and sales; and thrugh special prjects such as Suns license plates. Finally, many generus fans and cmmunity rganizatins designate Phenix Suns Charities as the recipient f their giving lead by a grup f dnrs knws as Playmakers wh pledge $100,000 ver fur years t the charity. Phenix Suns Charities encurages prpsals that: Leverage matching funds Engage strategic, missin-driven cllabratins/partnerships Build the capacity f the rganizatin Page 1

2 Supprt Arizna s children and their families Supprt ne-year prpsals Fr cnsideratin fr a Playmaker Grant ($100,000) The requesting nn-prfit needs t have received a grant frm Suns Charities in the past. In additin, we are lking fr an IMPACT grant that can be brught n-line within the year f receiving the grant. The Suns r Suns Charity brand shuld be able t accmpany the delivery f the prgram/imprvement. We typically dn t want t be part f a generic capital campaign, but will lk at a specific building prject within the campaign. Als, a large single-day event wuld typically NOT be spnsred thrugh the Playmaker Grant. The Suns Charities Bard alng with the dnr grup f Playmakers make the grant decisin fr these large awards. Eligibility Requirements Organizatin must have received funding frm Phenix Suns Charities in the past t be eligible fr the Playmaker Grant. If rganizatin has nt received funding frm PSC in past years, they may be eligible fr the nn-playmaker grant pprtunity (see Phenix Suns Charities when applying nline) Arizna 501(c)(3) rganizatin, schl, municipality, religius rganizatin r tribal entity that perates with fiscal accuntability and respnsibility. Organizatins withut a 501(c)(3) status must use a fiscal agent t apply n their behalf. Fiscal agents must have a visible prfile n this website, and als be a 501(c)(3) rganizatin meeting the ther criteria utlined. Organizatin has been an established 501(c)(3) fr at least 3 years. Prject cannt be a summer camp. Phenix Suns Charities is a cllabrative funder fr the Summer Yuth Prgram Fund which sle funds summer camps. Review and Evaluatin Prcess All prpsals will be reviewed thrugh a cmpetitive evaluatin prcess. The reviewers will be reading the applicants' nline prfiles n the ACF website t btain infrmatin n the rganizatin. Please be sure t update yur rganizatin's prfile befre submitting yur prpsal. Prpsals are reviewed and evaluated by grant cmmittee panels, cmprised f vlunteer Phenix Suns staff members wh assess prpsal strengths and weaknesses accrding t evaluatin criteria. Evaluatin criteria cnsists f a pint system awarded fr impact n cmmunity need, measurable benefits, plan fr sustainability, alignment with the funding criteria, partnerships, gals and bjectives clearly stated, measurable utcmes, ppulatin served, reasnable budget, and whether the prject/ rganizatin enhances cllabratin with ther cmmunity rganizatins and/r agencies.
