5 July 1, 2016 Intrductin Welcme Letter Dear Resident: On behalf f The University f Tled Medical Center (UTMC), I wuld like t welcme yu t the Medical Center. We are cmmitted t the prvisin f utstanding pst-graduate residency training prgrams. We currently ffer an array f experiences designed t prepare individuals fr rles in pharmacy with a fcus n managed care. The primary emphasis f yur residency prgram will be the develpment f practice skills in an ambulatry care and utpatient pharmacy setting. Yu will be delegated medicatin therapy management respnsibilities under the preceptrship f ur utpatient and ambulatry care teams and help t manage the University f Tled emplyee prescriptin benefit under my directin and the rest f the managed care team. Yur preceptrs will assist in the develpment f yur ne-n-ne patient skills while develping yur clinical knwledge in specific patient care areas. Yu will als be given teaching respnsibilities t further develp yur cmmunicatin skills and abilities as a teacher. Yu will participate in nging service activities t further develp yur prblem slving skills and yur ability t wrk with thers. Preceptrs will establish minimum expectatins fr yur perfrmance during rtatins. Yu will be encuraged t surpass these minimum expectatins and prvide cntinual feedback t me during yur experiences. Yu will be delegated respnsibilities and will be allwed t achieve as much independence as desirable and achievable. At the end f this residency yu will have a greater understanding f what it means t be a managed care pharmacist. We will prepare yu t create highly managed patients that are mtivated t achieve maximum value fr their healthcare dllars. As a managed care pharmacist yu will learn hw t be at the frefrnt as an innvatr wh is willing t explre and implement a variety f clinical and plan design strategies t achieve ptimal health utcmes and reduce csts. 6 RESIDENCY INFORMATION MISSION STATEMENT The missin f the University f Tled Medical Center and the Cllege f Pharmacy and Pharmaceutical Sciences (CPPS) Residency Prgrams is t prvide utstanding residency pprtunities that will allw pharmacists t advance their careers. Guiding principles are persnal integrity, respect fr humanity, and prfessinalism. The missin f the University f Tled PGY-1 residency is t develp a well-runded pharmacist wh exhibits qualities as a strng leader and teacher while prviding ptimal drug therapy t patients in an ambulatry care and utpatient setting with a managed care emphasis. The first year residency prgram enhances general cmpetencies in managing medicatin use systems and supprts ptimal medicatin therapy utcmes fr patient with a brad range f disease states. Guiding principles include persnal integrity, respect fr humanity, and prfessinalism. PURPOSE STATEMENT PGY1 pharmacy residency prgrams build n Dctr f Pharmacy (Pharm.D.) educatin and utcmes t cntribute t the develpment f clinical pharmacists respnsible fr medicatin-related care f patients with a wide range f cnditins, eligible fr bard certificatin, and eligible fr pstgraduate year tw (PGY2) pharmacy residency training. The prgram will prvide the resident with the knwledge and skills t pursue a career as a pharmacist in the fllwing areas: Utilizatin management Ambulatry care Medicatin therapy management Transitins f care Pharmacy benefit manager Anticagulatin pharmacist Frmulary management Specialty pharmacy/ prir authrizatin 340B specialist Clinical prgram manager Adjunct faculty Envirnments that will be well suited fr graduates f this prgram will be include managed care, healthcare netwrks, emplyer based benefit design, ambulatry clinics, utpatient pharmacies, drug distributin, patient safety, clinical prgram develpment, business and cst management and accuntable care rganizatins. In additin residents cmpleting this prgram will be prepared t cmplete a variety f PGY-2 prgrams including but nt limited t Ambulatry Care, Pharmactherapy, and administrative residencies. EXPLANATION OF PURPOSE STATEMENT / PROGRAM DESIGN The purpse f the University f Tled PGY-1 Residency is t train the pharmacy resident t becme a wellrunded pharmacist. The pharmacy resident will train at The University f Tled Medical Center utpatient clinics, utpatient pharmacies, tw ambulatry care settings as well as cllabrating with the current emplyee prescriptin benefit manager while meeting the cmpetency areas, gals, bjectives within the ASHP standards. The resident will be expected t becme cmpetent within the learning experiences in the fllwing seven required cmpetency areas assciated with learning experience dcuments and bjectives: (1) Patient Care (2) Advancing Practice and Imprving Patient Care (3) Leadership and Management (4) Teaching, Educatin, and Disseminatin f Knwledge (5) Managed Care (6) Teaching and Learning (7) Specialty Pharmacy Preceptrs assure required cmpetency in each f the abve areas by teaching r evaluating required educatinal gals and bjectives. Cmpetency within these six areas will be achieved by the teaching and evaluatin f these gals thru learning experiences in the fllwing required areas f fcus: 5

The resident will achieve tw thirds f their resident experiences prviding direct patient care t specialty pharmacy patients, emplyees enrlled in the UT prescriptin benefits plan, inpatients transitining ut f the hspital, patients seen within the UT anticagulatin clinic, patients seen under the medicatin management clinic as well as patients cared fr during the staffing cmpnent f the prgram. 8 RESIDENCY PROGRAM DESCRIPTIONS & DEFINITIONS In May f 2014 the CEO f the University f Tled Medical Center (UTMC) was apprached in supprt f adding a residency prgram dedicated specifically t the managed care area. UTMC was in the prcess f implementing a plan ptimizatin prgram fr its emplyee prescriptin benefit with prjected additinal savings f 1.1 millin dllars in additinal saving t the plan. As part f the return n this prjected plan ptimizatin savings, additinal funding was requested fr a new PGY1 residency that wuld be fully dedicated t the OP pharmacies, ambulatry spaces and managed care. In July 2014 the first PGY1 residency was started at UTMC that fcused n managed care, utpatient and ambulatry services. The rganizatinal structure f the prgram is set up t reprt t the UTMC hspital. In February 2015 the applicatin prcess was started fr ASHP accreditatin f the prgram as a PGY1 residency prgram. Site evaluatin is schedule fr spring- summer f Residents cmpleting the current prgram will retractively be awarded a new certificated as an ASHP accredited prgram. Directr f Pharmacy The pharmacy shall be managed by a prfessinally cmpetent, legally qualified pharmacist. The Directr f Pharmacy is knwledgeable abut and is expected t have experience in hspital pharmacy practice and management. An advanced management degree (e.g., M.B.A., M.H.A., r M.S.) r an administrative specialty residency is desirable. The Pharmacy Directr is respnsible fr the fllwing: Designating the individual respnsible fr the administratin and crdinatin fr the PGY1 Residency Prgram Establishing the missin, visin, gals, and scpe f services f the pharmacy based n the needs f the patients served, the needs f the hspital (and any health system f which the hspital may be a cmpnent), and develpments and trends in health care and hspital pharmacy practice Develping, implementing, evaluating, and updating plans and activities t fulfill the missin, visin, gals, and scpe f services f the pharmacy Actively wrking with r as a part f hspital r health-system leadership t develp and implement plicies and prcedures that prvide safe and effective medicatin use fr the patients served by the institutin Mbilizing and managing the resurces, bth human and financial, necessary fr the ptimal prvisin f pharmacy services Ensuring that patient care services prvided by pharmacists and ther pharmacy persnnel are delivered in adherence t applicable state and federal laws and regulatins, hspital privileging requirements, and natinal practice standards Respnsible fr handling grievances that are still disputed beynd the Residency Prgram Directr (RPD) and Resident Advisry Cmmittee (RAC level Residency Prgram Directr (RPD) the individual respnsible fr directing the activities f a particular residency prgram and is respnsible fr cmpletin f the Residency Learning Systems (RLS) quarterly and final evaluatins. The RPD must als maintain the prgram s cmpliance with the prvisins f the current versin f the ASHP Regulatins n Accreditatin f Pharmacy Residencies thrughut the accreditatin cycle such as activities related t the recruitment, selectin, instructin, supervisin, cunseling, evaluatin, advancement f residents, evaluatin and develpment f preceptrs, and the maintenance f recrds related t prgram accreditatin. (See Appendix 5) Residency Crdinatr the individual(s) wh the PGY-1 Residency Prgram Directr assigns as a persnal and/r prfessinal advisr t ne r mre PGY-1 Residents. 9 Demnstrates cmmitment t advancing the residency prgram and pharmacy services As part f their annual perfrmance review will be assessed n the fllwing: end- f- year preceptr self-assessments review quality f written feedback (sample frm each preceptr) review resident evaluatins f preceptrs and learning experiences fr trends participatin and input fr the Resident Advisry Cmmittee meetings maintain individual learning experience dcuments and develp apprpriate activities t meet gals and bjectives f the residency prgram ability t cmplete preceptr develpment in a timely manner review prgress t date fr preceptrs in training (See Appendix 5b Residency Preceptr Qualificatins and Respnsibility Checklist, See Appendix 5c fr List f Current Residency Preceptrs) A preceptr in training develpment plan will be develped with any Pharmacist new t precepting wh des nt yet meet the qualificatins fr residency preceptrs as stated abve therefre will be Assigned an advisr r cach wh is a qualified preceptr and A Preceptr Develpment plan established between the RPD and the Preceptr in training that will be reviewed quarterly t assure timely prgress. Preceptr-in-training develpment plans are used fr any new preceptr that is nt able t meet the ASHP requirements fr a PGY1 preceptr. The develpment plan may span a ttal f 2 years. (Appendix 5c Residency Preceptr- In -Training Qualificatins and Respnsibility Checklist) (Appendix 5d Preceptr In training Develpment Plan) Residency Advisr- Serves as a prfessinal mentr fr the resident and meets peridically t discuss prgress twards gals and any issues pertaining t the prgram. The residency advisr may als be a preceptr fr ne f the resident rtatins.. Administrative Crdinatr designates the individual that manages and crdinate the day-t-day administrative functins f the residency prgram. Respnsibilities are utlined in the administrative crdinatr jb descriptin Residency Advisry Cmmittee (RAC) Oversight cmmittee t discuss and mnitr prgress f the resident and the PGY1 prgram als respnsible fr establishing preceptr training and develpment. RAC meets nce per mnth and is cmpsed f cre preceptrs and advisrs fr the PGY1 residency prgram. RPD will chair the RAC cmmittee and will assign the respnsibility fr agenda preparatin and RAC meeting minutes t a RAC Cmmittee member. The PGY1 resident will participate in RAC meetings, unless a clsed sessin is needed. Any feedback frm RAC will be given t the resident thrugh their advisr, preceptr, r RPD as apprpriate. An nging preceptr develpment plan will be established thru the residency advisry cmmittee and becme an expectatin f the annual perfrmance review prcess. 10 EXPECTATIONS AND RESPONSIBILITIES OF RESIDENTS- Listed within plicy sectin f manual Prfessinal Cnduct The PGY1 residency is cmmitted t prviding excellence in patient care, teaching and research in an envirnment that is respectful f thers, adaptive t change and accuntable fr utcmes. The resident must understand that he/she is representative f the University f Tled and the Outpatient Pharmacies and Ambulatry Clinics and is expected t cnduct him/herself in a prfessinal manner at all times. It is the respnsibility f all residents t adhere t the University f Tled Medical Center (UTMC) Standards f Cnduct plicy which states: The University f Tled s gals f excellence in educatin, research, and in quality patient care are supprted by a set f standards f cnduct. A detailed descriptin f the standards may be fund at: Prfessinal Dress All residents are expected t dress in an apprpriate prfessinal manner whenever they are in the institutin r attending any functin as a representative f UTMC. Clean, pressed white lab cats f full length will be wrn at all times in patient care areas. Any specific prblems with dress will be addressed by the resident's prgram directr and/r preceptr. A detailed plicy may be fund at: Identificatin Badges UTMC Security requires all persnnel t wear his/her badge at all times when they are n campus. If the badge is misplaced, a temprary badge is available at Security in the basement f the hspital. If the emplyee badge is lst, the resident must reprt the lss immediately t Security, and render a fee fr replacement. This identificatin badge will als be utilized t btain access t any area requiring a badge swipe fr admittance. A detailed plicy regarding emplyee badges may be fund at Patient Cnfidentiality Patient cnfidentiality will be strictly maintained by all residents. Any cnsultatin cncerning a patient will be held in privacy with the utmst cncern fr the patient s and family s emtinal and physical well-being. A detailed plicy may be fund at Electrnics and Respnsible Use The use f any electrnics (bth persnal and University-prvided) and the use f cmputing resurces are subject t the requirements f legal and ethical behavir within the university cmmunity. The resident may be given university-prvided electrnics fr use while emplyed at the University f Tled Medical Center. All electrnics must be returned in gd-wrking cnditin at the end f residency. Any lst, stlen, r damaged electrnics (by negligence, etc.) will be replaced at the resident s expense. A detailed plicy regarding electrnics and respnsible use can be fund at A detailed plicy regarding electrnic mail services can be fund at Attendance Residents are expected t attend all functins as required by the Residency Prgram Directr (RPD) and rtatin preceptrs. The residents are slely respnsible fr their assigned peratinal pharmacy practice and are respnsible fr assuring that these service cmmitments are met in the event f an absence. All leave requests shuld be discussed at least 30 days in advance with the invlved preceptr(s) t assure that service respnsibilities can be fulfilled. An excused absence is defined as vacatin, sick, r prfessinal leave discussed with and signed ff by the respective rtatin preceptr and prgram directr. This schedule may have slight variatins depending n factrs such as clinics, staffing, teaching, prfessinal bligatins and unexpected variability in preceptr schedules. (See Appendix 3 Resident Schedule) The PGY1 Resident is required t staff ne Saturday a mnth in additin t weekday requirements, during pharmacy business hurs (nt t exceed 7am-7pm). In the case f inclement weather r ther emergencies, all essential services n the Health Science Campus will cntinue. Essential persnnel are required t reprt t wrk during all emergencies when scheduled. The PGY1 Resident will reprt t wrk when the utpatient pharmacies and clinics remain pen. 11 Leave f Absence Residents are expected t perfrm their duties fr 12 calendar mnths. Therefre, absence due t a vacatin, illness, persnal business, etc. shuld nt exceed alltted time. Any unexpected hardship must be cmmunicated with the residency prgram directr and absence will be planned. Any make up time must be added t the prjected date f cmpletin f the required 12 mnths f training. All leave requests shuld be discussed in advance with the invlved preceptr t assure that service respnsibilities can be fulfilled. Arrangements with individual residents and the RPD will be made fr any unfreseen lng term leave f absence and may include a furlugh f pay and cmpletins t be determined when apprpriate. (See Appendix 6- Leave f Absence Frm) Letter f Acceptance and Cntract The residency prgram directr (RPD) must prvide the PGY1 resident with a letter utlining their acceptance. (See Appendix 7 Letter f Acceptance & Appendix 8 Cntract Template) Duty Hurs- Duty hurs are defined as all clinical and academic activities related t the residency prgram, i.e., patient care (bth inpatient and utpatient), administrative duties related t patient care, the prvisin fr transfer f patient care, and scheduled academic activities such as cnferences. Duty hurs d nt include reading and preparatin time spent away frm the duty site. ASHP duty- hur standards Duty Hurs must be limited t 80 hurs per week, averaged ver a fur week perid, inclusive f all in-huse call activities. Residents must be prvided with 1 day in 7 free frm all educatinal and clinical respnsibilities, averaged ver a 4 week perid. It is highly unlikely that the residents within the PGY1 residency will be expected t be n call. One day is defined as ne cntinuus 24-hur perid free frm all clinical, educatinal, and administrative activities Mnlighting- (internal r external) must nt interfere with the ability f the resident t achieve the educatinal gals and bjectives f the residency prgram. The ability t mnlight must be discussed with the RDP and if allwed the fllwing must be cnsidered and dcumented in the resident emplyee recrd: All mnlighting hurs must be cunted twards the 80 hur maximum weekly hur limit. Prgrams that allw mnlighting must have a dcumented structured prcess t mnitr mnlighting that includes at a minimum: The type and number f mnlighting hurs allwed by the prgram A reprting mechanism fr residents t infrm the residency prgram directrs f their mnlighting hurs. A mechanism fr evaluating residents verall perfrmance r residents judgment while n scheduled duty perids and affect their ability t achieve the educatinal gals and bjectives f their residency prgram and prvide safe patient care. A plan fr what t d if residents participatin in mnlighting affects their judgment while n scheduled duty hur If it is determired that mnlighting is affecting the ability f the resident t cmplete required PGY1 residency requirements, the resident will be required t immediately stp mnlighting. RESIDENCY REQUIREMENTS Cmpletin Requirements Residents in all residency prgrams will be required t cmplete 12 mnths f training and perfrm r participate in a number f pharmacy related/prfessinal activities thrughut the year. These activities are designed t assure cmpetency with the gals and bjectives utlined by the residency accreditatin standards. Once a tpic and preceptr have been selected, the resident will be alltted time during the residency t cmplete a pharmacy practice research prject within the residency year. A manuscript suitable fr publicatin in a peer-reviewed jurnal describing the results f the prject must be written and submitted t RPD and preceptr verseeing the research. Thse invlved with the research must cmplete CITI and IRB training. Investigatinal Review Bard (IRB) and CITI Training Each resident will need t be trained prir t cmpleting a prject cntaining human subjects as the target ppulatin. The directr f pharmacy, prgram directrs, preceptrs, and faculty will fllw a crrective actin prcess based n cunseling and a warning system when a serius deficiency in the resident s perfrmance is nted. The crrective actin prcess will be used when addressing areas f resident perfrmance r behavir requiring imprvement r eliminatin. It is intended t initiate actin that will assist the resident in crrecting prblems and imprving perfrmance and behavir. Failure t imprve perfrmance as addressed by the crrective actin prcess within the specified schedule(s) will result in the resident nt receiving a certificate f successful cmpletin f the training prgram and may result in invluntary dismissal. Hwever, certain behavirs r actins will be cnsidered immediate grunds fr dismissal and the crrective actin prcess will nt apply. Crrective actin prcess prcedures and grunds fr dismissal are utlined belw. Crrective Actin Prcess The crrective actin prcess will be utilized if the resident fails t present him/herself in a prfessinal manner, fllw plicies and prcedures f the University, Cllege, Department, r Medical Center, make satisfactry prgress n any f the residency gals r bjectives (nt t be determined by ne rtatin), and make satisfactry prgress twards the cmpletin f a residency requirement. The RPD r designate, prir t initiating crrective actin, will cnduct a thrugh investigatin. This includes meeting with the resident t investigate the cncern and ffer the resident an pprtunity t prvide infrmatin relevant t the identified prblem. Fllwing an investigatin, the RPD will review the results f the investigatin t determine the need t initiate a crrective actin prcess. The RPD shall infrm the resident f the results f the review regardless f the final decisin. The crrective actin prcess cnsists f the fllwing: Verbal and written cunseling including specific expectatins fr imprved perfrmance r behavir. Issuance f verbal and written warnings f the duratin f the prbatinary perid. Issuance f a schedule fr any additinal verbal and written reviews and perfrmance/behavir expectatins during the prbatinary perid. A verbal and written statement issued at the end f the prbatinary perid assciated with the crrective actin prcess stating the final evaluatin f the resident s perfrmance. The final evaluatin shall fall int ne f three categries: Successful imprvement and achievement f required prgram perfrmance and r prfessinal behavir by the resident. Partial, yet inadequate, imprvement in r unsuccessful achievement f required perfrmance r behaviral expectatins. If this applies t an inability f the resident t successfully cmplete any requirement fr certificatin f cmpletin f residency training, this is t be accmpanied by a request fr resident vluntary terminatin. Cntinued demnstratin f perfrmance r behavir requiring crrective actin withut imprvement. This is t be accmpanied by issuance f an invluntary terminatin letter. 31
