1 Psitin Descriptin Psitin Title: Direct Reprts: Lcatin: Nurse Team Leader HNS IPU Team Leader, Cmmunity Nursing Team, Cmmunity Vlunteer Crdinatr and Physitherapist Clinical Administratrs (dtted line) Hspice Nrth Shre Date: May 2017 Reprting T: Hurs: Clinical Services Manager 1.0 FTE 40 hurs per week, Mn - Fri Psitin Objective The purpse f the Nurse Team Leader s rle is t ensure the prvisin f specialist palliative care t patients and families in the Inpatient unit and the cmmunity thrugh leadership and management f resurces, systems and prcesses. On a day-t-day basis the rle will primarily be based in the cmmunity team and will pick up a 50% patient and/r caching lad depending n the acuity levels and verall need. The rle will have verall respnsibility fr bth nursing teams ensuring the prvisin f high quality, seamless care. This rle will als wrk clsely with the Clinical Develpment Specialist and the Referral Crdinatr. Key Accuntabilities and Expected Results Accuntability Expected Results Ensures the prvisin f Ensures clinical service prvisin fr the Hspice is driven by rganisatinal values, visin and strategy quality, cmprehensive palliative nursing Takes primarily respnsibility fr the daily versight f what is ccurring in the cmmunity team and versees the IPU Team Leader t ensure the nurses are well supprted and patient/family needs are being met care Ensures relatinships are established and maintained with external prviders: primary and cmmunity prviders, such as the DHB s, District Nurses, Te Puna Haura, ARCF s Attends rganisatinal meetings, multidisciplinary meetings, case review and family meetings as required Organises and runs mnthly, frmal team meetings t facilitate exchange f infrmatin and discussin f team and rganisatinal issues Prvides supprt fr Hibiscus and Warkwrth Wellsfrd Hspices as directed by the Clinical Services Manager.
2 Empwers the Team Effectively manages resurces within an agreed annual budget Undertakes the delivery f internal and external educatin such as the Fundamentals f Palliative Care Effectively leads the nursing teams by: Rle mdelling required behaviurs t facilitate psitive team culture Using caching techniques with individual staff t ensure expectatins are understd and negative behaviurs are discuraged. Supprting the wrk f the team by facilitating the planning, implementatin and evaluatin f care t patients and families in a manner which prmtes excellence and reflects rganisatinal values. Actively participants in and supprts the ptimal functining within the MDT. Actively participates in educating staff n a daily basis and wrks with the Clinical Develpment Specialist t implement Nurse Cmpetencies and develp in-service educatin t achieve best practice Supprts staff develpment thrugh caching and mentring t ensure that nurses practice safely within their scpe and are adequately skilled and resurced Is invlved in clinical supervisin and supprt prgrammes Takes respnsibility fr cmpleting develpment plans and perfrmance appraisals fr each member f the cmmunity nursing team annually Prvides supprt t the IPU Team Leader t ensure develpment plans and perfrmance appraisals fr each member f the inpatient nursing team are cmpleted annually Wrks with the Clinical Develpment Specialist t implement career pathways Takes verall respnsibility fr the rientatin f new staff int the cmmunity team Encurages a multi prfessinal team riented culture by develping psitive, effective wrking relatinships with all staff by clearly cmmunicating expectatins Is required t be part f the Senir Nurse On Call systems fr afterhurs supprt. Takes full respnsibility fr the CPC rster t ensure safe levels f care and alignment f rles, effective wrklad distributin, staff technical cmpetencies and expertise ensuring wise stewardship f resurces including mnetary. Oversees staffing requirements fr bth nursing teams t maintain safe levels f care and alignment f rles, effective wrklad distributin, staff technical cmpetencies and expertise. Wrks with the senir nurses and Clinical Develpment Specialist t develp and implement a prgramme that enables upskilling f nurses t wrk acrss IPU and CPC when required. Takes respnsibility fr IPU when the IPU Team Leader is absent Respnsible fr identifying, develping and supprting designated staff t undertake higher duties

Manages the IPU and CPC budget reprting any unusual high spending t the Clinical Services Manager Wrks with the Clinical Services Manager t set yearly budget requirements Ensures cmpliance with OSH, Infectin Cntrl and Fire regulatins. Implements measures t cntrl hazards and ensure all staff members are aware f hazards. Ensures staff adherence t Lne Wrker Prcedures Ensures the crrect and safe strage, administratin and dispsal f all drugs and devices. Actively participates in the Infectin Cntrl Cmmittee and prmte activities related t Infectin Cntrl. Ensures all nurses attend cmpulsry annual educatin updates Advises Clinical Services Manager f any recurrent issues r themes impacting n staff, patient/family well-being.
