Imprtantly, clinicals (and sme campus labratry times) are ffered in bth day and evening hurs fr bth classes (fall and spring). Althugh sme clinical site and time chices are available fr students, we cannt guarantee preferred sites and times will always be available. Due t clinical agency requirements, the Divisin f Health Prfessins requires an extensive natinwide Backgrund Reprt which includes, but is nt limited t, Oklahma State Bureau f Investigatin (OSBI) backgrund searches fr sex ffender, vilent ffender, and criminal histry. The student is respnsible fr the $65.00 cst f the Backgrund Reprt, which must be paid by credit card r mney rder nly. Clinical agency representative(s) review the reprts. Clinicals are generally ne day per week. Students are nt guaranteed any specific clinical day r facility. Graduates f this stateapprved prgram are eligible t apply t write the Natinal Cuncil Licensure Examinatin (NCLEX) fr registered nurses. Applicants fr Oklahma licensure must meet all state and federal requirements t hld an Oklahma license t practice nursing. In additin t cmpleting a state-apprved nursing educatin prgram that meets educatinal requirements and successfully passing the licensure examinatin, requirements include submissin f an applicatin fr licensure, a criminal histry recrds search, and evidence f citizenship r qualified alien status. T be granted a license an applicant must have the legal right t be in the United States (United States Cde Chapter 8, Sectin 1621). In additin, Oklahma law nly allws a license t be issued t U.S. citizens, U.S. natinals, and legal permanent resident aliens. Other qualified aliens may be issued a temprary license that is valid until the expiratin f their visa status, r if there is n expiratin date, fr ne year. Applicants wh are qualified aliens must present t the Bard ffice, in persn, valid dcumentary evidence f: A valid, unexpired immigrant r nnimmigrant visa status fr admissin int the U.S.; A pending r apprved applicatin fr asylum in the U.S.; Admissin int the U.S. in refugee status; A pending r apprved applicatin fr temprary prtected status in the U.S.; Apprved deferred actin status; r A pending applicatin fr adjustment f status t legal permanent residence status r cnditinal resident status. The infrmatin will be verified thrugh the Systematic Alien Verificatin fr Entitlements (SAVE) Prgram, perated by the U.S. Department f Hmeland Security. The Oklahma Bard f Nursing has the right t deny a license t an individual with a histry f criminal backgrund, disciplinary actin n anther health-related license r certificatin, r judicial declaratin f mental incmpetence. These cases are cnsidered n an individual basis at the time applicatin fr licensure is made, with the exceptin f felny cnvictins. An individual with a felny cnvictin cannt apply fr licensure fr at least five years after cmpletin f all sentencing terms, including prbatin and suspended sentences, unless a presidential r gubernatrial pardn is received. Additinal infrmatin may be btained at CLINICAL INFORMATION The fllwing are the immunizatin and health recrds currently required t attend clinical. These recrds are subject t change as clinical agencies update their requirements. All requirements must remain current thrughut the semester. 1. All students must be currently certified in American Heart Assciatin Healthcare Prvider CPR prir t enrllment in any nursing curse. Online CPR training is nly acceptable if it includes hands-n skill check-ff and is American Heart Assciatin Healthcare Prvider CPR. CPR certificatin is gd fr the perid indicated n yur card. Hwever, it must be current fr the entire semester in which the student is enrlling. E.g., if yur CPR certificatin expires in the middle f a semester, yu must renew prir t the first day f class fr that semester. 2. All nursing students must present prf f the fllwing immunizatins and health recrds. Infrmatin is available in the Health Prfessins Divisin ffice n where the tests and required immunizatins can be btained. In additin t these sites, yur persnal

3 health care prvider, area hspitals with emplyee health services r the Cunty Health Department in yur cmmunity may be resurces and can answer yur questins abut these requirements. 3. Tw MMR (measles, mumps, rubella) vaccinatins OR Psitive titers fr measles, mumps and rubella are required. 4. Tw Varicella vaccinatins OR ne psitive Varicella titer 5. Three Hepatitis B vaccinatins OR Hep B waiver OR One psitive Hep B titer Nte: If titers are equivcal, vaccinatins will be required. 6. Tw negative TB skin tests, given within the last 12-mnth perid prir t beginning clinicals. The test must remain current the entire semester (TB tests are gd fr ne calendar year), n exceptins. OR Dcumented psitive TB test with dcumented negative (-) chest x-ray AND negative (-) annual review f symptms which will remain current the entire semester. Students with a nn-negative TB test must submit a signed Clearance fr Public Cntact Statement frm the Cunty Health Department if the nn-negative TB test results are recent. Please cntact the Divisin ffice if yu have questins regarding yur nn-negative test results. OR TB Quantifern Gld (T-Spt) Bld Test with negative results. Imprtant: A PPD can be applied befre r n the same day that MMR vaccine is given. Hwever, if MMR vaccine is given n the previus day r earlier, the PPD shuld be delayed fr at least ne mnth. Live measles vaccine given prir t the applicatin f a PPD can reduce the reactivity f the skin test because f mild suppressin f the immune system (Reference: CDC). 7. Seasnal flu vaccinatin dcumentatin OR signed flu vaccinatin declinatin statement. Student must have either physician statement fr medical reasns r minister s statement fr religius reasn if vaccine is nt received. 8. The cst f the insurance plicy (at the time f this printing) is $31.02 fr ne calendar year and is purchased in the Bursar s ffice during nrmal hurs f peratin. NESA Requirements All students must view the required NESA (Nursing Educatrs and Service Administratrs) vides and pass the NESA exam, with a scre f 85%, annually prir t attending clinical rtatins. 3. Clinical Hub The Clinical Hub Website is prvided t facilitate the cperative scheduling, crdinatin and arrangement f clinical rtatin schedules and pprtunities amng hspitals, schls and students. Infrmatin available n the Hub includes hspital rientatin material and the student s weekly clinical assignment. T serve this end, the student is required t create an accunt with the Clinical Hub, which will be accessible t crdinatrs, staff and ther administratrs (wh have access t the Website) at yur schl and at the varius hspitals where yu are scheduled fr clinical rtatins. The cst t students fr the Clinical Hub is $20.00 per year due at the time f registratin. Students will be ntified f the need t update their accunt yearly, if necessary. Failure t register will result in the student being unable t attend clinical, which may affect prgressin in the nursing prgram. Students may register fr the Clinical HUB at 4. Cmputer Based Testing All students are required t pay fr and utilize a testing prgram t supprt their learning and enhance the nursing curriculum. Class/Clinical Time Requirements All nursing classes, except Overview f Nursing (NUR 1221) and Nursing Rle Transitin (NUR 1415), have clinical and lab requirements.
