1 Cuncil Camp Staff and the Annual Health & Medical Recrd CampDc FAQs What is CampDc? CampDc.cm is an electrnic health recrd system designed by physicians, nurses, and camp directrs fr use in camps. Web-based strage f infrmatin and frms allws camp health staff t have instant access t vital medical infrmatin, such as medical cnditins, medicatins, allergies, and emergency cntacts; this allws them t mre safely, accurately, and timely treat yur health cncerns while yu are wrking at camp. Why use CampDc? What s wrng with paper? CampDc prvides a fast, cnvenient, and custmized way f cllecting and string the By Scuts f America Annual Health and Medical Recrd (AHMR), much like WrkBright stres all yur ther emplyment dcuments. In additin t cnvenience, electrnic health recrds have ther demnstrated benefits, such as imprvement in quality, standardizatin, and crdinatin f health care. Smetimes staff members may becme ill r injured while at wrk and require the services f the camp health staff. We want thse services t be delivered in the safest pssible fashin; using this technlgy will help us d s. By Scuts f America Natinal Cuncil has authrized the use f CampDc in the Cradle f Liberty Cuncil. What kind f infrmatin ges int CampDc? All the same infrmatin that yu wuld prvide n yur AHMR! CampDc has custmized its sftware t cllect the exact same infrmatin required in: Part A, including Infrmed Cnsent, Release Agreement, and Authrizatin Part B, including General Infrmatin/Health Histry

2 CAMPDOC FAQs AND DETAILED INSTRUCTIONS Part C is still printed s that it may be cmpleted and signed by yur healthcare prvider. Hwever, it is easily upladed int yur accunt. In additin t cmpleting Parts A, B, and C, yu ll uplad ther dcuments, such as yur immunizatin recrd, a cpy f yur health insurance card, and the Grand View Hspital Cnsent Frm (Musser Scut Reservatin nly). Lastly, parents/guardians f minr staff can directly give cnsent fr the use f ver-the-cunter medicatin stcked by ur health staff. In CampDc, all this infrmatin is referred t as yur Health Prfile, and it becmes instantly available at the pint f care allwing health staff t make the best treatment decisins pssible. Wh is respnsible fr cmpleting my CampDc Health Prfile? If yu are 18 years f age r lder, yu will cmplete yur wn Health Prfile and uplad required dcuments. If yu are under 18 years f age, a parent r guardian must cmplete yur Health Prfile and uplad required dcuments, just as they wuld cmplete and give cnsent n yur paper AHMR. This plicy cincides with general age f medical cnsent in Pennsylvania. If yu are under 18 years f age, WrkBright will cllect an address f a parent r guardian wh is legally able t give cnsent n yur behalf. I already cmpleted my Health Prfile in CampDc last year. D I have t d it all ver again? N! Much f yur infrmatin is retained; all yu will have t d is t d sme updating. If yu are under 18 years f age, yur parent/guardian will becme an authrized user. Is CampDc used in any ther setting in the Cuncil? N. Currently CampDc is nly used fr camp staff purpses. Therefre, if yu attend resident camp, NYLT, r vlunteer at a Cuncil event, yu will need a paper cpy f yur AHMR. But, there is an easy fix fr that; all yu need t d is print ut yur Health Prfile directly frm CampDc and bring the paper cpy with yu t the event. I am cncerned abut the privacy and security f my health infrmatin. Hw des CampDc prtect it? 3 CAMPDOC FAQs AND DETAILED INSTRUCTIONS Detailed Instructins fr Cmpleting the Health Prfile Invitatin Yu (staff member 18 years f age r parent/guardian f minr staff) will receive an directly frm CampDc inviting yu t cmplete yur Health Prfile. The utilized will be the ne yu entered in WrkBright; make sure it is crrect. Remember that minr staff must have a parent/guardian cmplete their Health Prfile. Prepare Dcuments fr Uplading Befre cmpleting yur Health Prfile, it is helpful t have digitized cpies f the fllwing dcuments available. Acceptable frmats include PDF, JPEG, GIF, and PNG. Cell phne pictures are acceptable if they are gd quality and easy t read. There are many free smartphne apps available that will turn phts int PDFs. Files are easily added via drag and drp r uplad. Health Insurance Card Immunizatin Recrd (We understand that sme adults may nt have access t this, s this is nt a mandatry field. Hwever, By Scuts f America des recmmend immunizatin f its members accrding t standardly accepted clinical guidelines.) Part C f the Annual Health and Medical Recrd (This is cmpleted, signed, and dated by an authrized healthcare prvider and must be dne annually.) Grand View Hspital Cnsent Frm This frm is nly needed fr minr staff at Musser Scut Reservatin. This frm is available within CampDc; dwnlad, print, cmplete, sign, digitize. Gather Persnal and Medical Infrmatin Current height and weight Names, phne numbers, and addresses f emergency cntacts Health histry infrmatin Current medicatin list, including names and dsages Date (r apprximate) f last tetanus vaccinatin

4 CAMPDOC FAQs AND DETAILED INSTRUCTIONS General Tips fr Cmpleting the Health Prfile The Health Prfile is rganized by sectins; sectins d nt have t be cmpleted in sequence. Yu can navigate between sectins by clicking the Previus Step r Next Step buttns at the bttm f the page. Yu may als jump between sectins by clicking the sectin name n the right f the page. HEALTH PROFILE SECTIONS Yur answers will save as yu type them. Yu d nt have t cmplete the entire Health Prfile at nce. * Starred fields bxed in red are required fields. Drp-dwn bxes are available fr yur speed and cnvenience fr many items, such as state name, numbers fr weight, etc. Fr existing Health Prfiles, sme infrmatin may be reviewed, updated as needed, then cnfirmed. Yu will see clred bxes at the tp and bttm f sectins where updates are allwable:

Incmplete sectins will be marked with a red dt. Sectins requiring updating will be marked with an range clck. I am unsure hw t answer a questin. If yu are unsure hw t cmplete a field, chances are there is a questin mark icn t the right f the field title; hver yur cursr ver the icn t receive additinal instructins n cmpleting the field. ADDITIONAL INSTRUCTIONS Printing yur Health Prfile. If after cmpleting yur Health Prfile yu chse t print a paper cpy, there may be a brief delay befre upladed dcuments are available fr printing. Allw mre time befre printing. I still need mre help. If yu require even mre help while cmpleting yur Health Prfile, click n the questin mark icn in at the tp right in the green bar. This will bring yu t articles that answer many cmmn questins. Click Patient then Health Prfile.

Emergency Cntacts Fr yuth staff: At least ne parent/guardian and ne alternative emergency cntact (name, phne number, and ) must be listed. Think carefully wh yu wuld want cntacted if a parent/guardian was nt reachable during an emergency. Fr adult staff: At least ne emergency cntact (name, phne number, and ) must be listed. Insurance Infrmatin Name f carrier, plicy number. Uplad a cpy f the insurance card here. Health Histry These questins match thse listed in Part B f the AHMR. Diet Restrictins List fd allergies and medical r religius fd restrictins. This infrmatin is imprtant fr the dining hall staff. Medicatins Supplied by Camp Fr minr staff (adult staff can give verbal cnsent at the pint f care). Parent/guardian gives cnsent r nt fr ver-the-cunter medicatins supplied by camp. (The medicatins listed are the nly nes supplied by camp health staff. If yur minr staff member requires additinal ver-the-cunter medicatins, please send a labeled supply in the riginal cntainer t camp and recrd them in the Current Medicatins sectin.) Current Medicatins Cmplete infrmatin regarding all current medicatins. Yu may add as many medicatins as needed. When this sectin is cmplete, please select the print buttn, print a cpy, and place a cpy with the medicatins brught t camp.

7 CAMPDOC FAQs AND DETAILED INSTRUCTIONS Ntes abut all medicatins brught t camp: Adult staff may stre their medicatin in a lcked cntainer in their place f ldging. Minr staff must bring all medicatins, except fr rescue inhalers, epinephrine autinjectrs, r insulin, t the Health Ldge fr strage and administratin. All medicatins shuld be stred in a zipper-lck bag labeled with staff member s name, be in the riginal cntainer (als labeled with name), and be accmpanied by a printed cpy f this Health Prfile sectin. Allergies Include fd, drug, and envirnmental allergies and their accmpanying reactins. There are many drp-dwn menus available. Yu may select as many reactins as required t prvide a cmplete and accurate descriptin. If there are medicatins r reactins that are nt listed in the drp-dwn menu, they may be typed in. Multiple allergies may be added. Immunizatins By Scuts f America natinal plicy recmmends that members btain immunizatins accrding t the cmmnly accepted age-apprpriate vaccinatin schedules. Natinal plicy requires a current tetanus immunizatin. It is understd that lder staff members may nt have access t their immunizatin recrds; yunger staff members shuld uplad a cpy f their recrds. All staff must add a date (r apprximate) fr their last tetanus immunizatin. The tetanus vaccinatin may be represented in yur recrds as Td, Tdap, r DTaP; in additin, the vaccinatin may be represented by varius trade names. If yu are unsure which entry represents yur mst recent tetanus immunizatin, cntact yur healthcare prvider fr clarificatin. Cnsent and Release Agreement This sectin represents Part A f the AHMR. Read and electrnically sign the Infrmed Cnsent, Release Agreement, and Authrizatin. Indicate any participatin restrictins. Authrized Adults t Transprt This represents infrmatin fund in Part A f the AHMR. This nly appears fr minr staff. It is required that ne persn be listed; hwever, yu may list up t 3 adults. Yu may als list any adults wh are NOT authrized t transprt minr staff. Grand View Cnsent Frm The frm will nly appear fr minr staff wrking at Musser Scut Reservatin. The frm gives cnsent fr Grand View Hspital and Urgent Care t prvide nnemergency treatment fr minrs. Dwnlad the frm, cmplete it, digitize, and uplad. Grand View Hspital will nt accept an electrnic signature at this time.

8 CAMPDOC FAQs AND DETAILED INSTRUCTIONS Healthcare Prvider Frm This represents Part C f the AHMR, which includes a physical exam frm an authrized healthcare prvider. Part C is cnsidered valid thrugh the end f the 12th mnth frm the date it was administered by yur healthcare prvider (date listed with signature). Fr example, a physical administered March 3, 2014, wuld be valid until March 31, Yu may dwnlad a cpy f the frm directly frm CampDc r visit the fllwing link t btain the frm (print page 4): Take this frm, as well as a printut f yur Health Prfile, t yur healthcare prvider fr yur physical exam. Have yur healthcare prvider cmplete, sign, and date the frm. The print buttn is lcated at the bttm f the Health Prfile sectins listed at the right: Once the frm has been cmpleted, digitize it and uplad it int this sectin f yur Health Prfile. Enter the date that the healthcare prvider signed the frm.

9 CAMPDOC FAQs AND DETAILED INSTRUCTIONS Enter any restrictins nted n the frm by yur healthcare prvider, lcated here: Yur Health Prfile is Nw Cmplete! Yu knw yur Health Prfile is 100% cmplete when all Health Prfile sectins n the right are accmpanied by a green check. Yu will als receive this screen:
