2 SESSION 1: Utilization Review and Better Practice Ideas to Reduce Vulnerabilities Day Egusquiza President AR Systems, Inc This session will explore how enhanced communication between case managers conducting utilization review and nursing can change the culture of documentation to support the patient status. The speaker will discuss how moving from a place and chase culture to an interactive utilization review role at the time of placement can move the organization to a more engaged system rather than a reactive environment. 1. Learn how enhanced communication can change the culture of documentation 2. Discuss strategies to implement an interactive utilization review role at time of placement 3. Assess the benefits of having a more engaged system rather than a reactive one SESSION 2: Reconnecting the Homeless Program Gladys Bukvic, MSW Homeless Coordinator / Social Work Lawrence & Memorial Hospital Lawrence & Memorial Hospital s Reconnecting the Homeless program has reduced the utilization of emergency department services and dramatically improved the health status of some of our community s most vulnerable individuals. The program is connecting individuals to housing, health care and social services bringing greater stability to their lives. The position and program started as a pilot and demonstrated outcomes that supported a full time social work case manager to interact with and provide oversight to this patient population. There is significant coordination and collaboration with other community agencies to ensure that the patient is served and duplication reduced. 1. Explain the unique elements of this program and how to implement them 2. Demonstrate the importance of stabilizing our clients 3. Verbalize the benefits of knowing and accessing community resources SESSION 3: The Three D s Dementia, Delirium and Depression in the Elderly Rajesh R. Tampi, MD, MS, FAPA of Behavioral Health, Masonicare Associate Clinical Professor Yale University School of Medicine Dementia, delirium and depression are three common disorders seen in the elderly and are often underdiagnosed and/or misdiagnosed. These disorders are associated with significant morbidity and mortality and cause immense economic strain on the health care system. Available evidence indicates efficacy for some commonly

3 used treatment modalities. This session will discuss the epidemiology, neurobiology, assessments and evidence-based treatments for these important disorders in the elderly. 1. Discuss the assessment and evidence-based treatments for dementia, delirium and depression in the elderly SESSION 4: Value Based Purchasing Hospitals Face Risk and Opportunity Ronald Hirsch, MD, FACP Vice President Accretive Physician Advisory Services Accretive Health With the Supreme Court affirming the Affordable Care Act, the government initiatives to improve care and lower costs can move forward. In this session, the speaker will review Value Based Purchasing, Medicare s first program to link payment to quality and the effects on hospitals and hospital case management. 1. Assess the effect on hospitals and case management SESSION 5: CoSTARR Community-Based Transitions of Care Our Beginning Julie Gelgauda, LCSW, Care Management Agency on Aging of South Central Connecticut Grace Jenq, MD Medical of Community Based Transition Program Associate Medical Yale New Haven Medical Inpatient Hospital Service Lynn Triebel, RN, BSN, ACM Care Coordinator, Acute Care for the Elderly Unit Yale New Haven Hospital Yale New Haven Hospital and the Hospital of Saint Raphael joined with the Agency on Aging of Southern Connecticut and have been awarded a CMS transitions of care grant to demonstrate a decrease in hospital readmissions for patients who are 64 or older, Medicare fee for service and live within a prescribed geographic area. The discussion will present the principles of using a BOOST assessment along with coaching and case management for patients who meet the high-risk criteria, have a history of readmissions and require assistance post-discharge. The speakers will share with the attendees successes and lessons learned. 1. Describe patients who are at high risk for readmissions 2. Define crucial elements of engagement in selection of patients 3. Apply three interventions which have had a positive effect on decreasing readmissions
