3 Keynote Address: The Mysteries of Observation and Condition Code 44 Ronald Hirsch, MD, FACP Vice President, Regulations & Educational Group AccretivePAS Chicago, IL Session Abstract: Hospitals must determine registration status correctly in order to be paid properly for the services they provide to both inpatients and outpatients, but the distinction between inpatient and observation can be very difficult to determine. Whether the patient is coming in through the ED for an acute problem, as a transfer from another facility or for a planned procedure, the hospital risks loss of revenue if the status is incorrect. The hospital may use Condition Code 44 to convert an inappropriate inpatient admission to outpatient status, but the process is complex and several steps must be completed to use it correctly. This session will address the criteria used to assign and change level of care in a variety of real-life situations faced by hospital case managers daily. 1. Understand Medicare rules and regulations concerning level of care and the use of observation 2. Use observation services appropriately for patients having surgery and procedures 3. Apply the Condition Code 44 process to compliantly convert Medicare inpatients to outpatient status Session 2: It Takes a Community Betty DeBlasio, RN, Clinical QI Specialist RN Qsource Nashville, TN Session Abstract: This session will educate case managers about how hospital readmissions are a community problem, not a hospital problem. Attendees will gain knowledge about tools that can assist in care coordination and reducing readmissions. The speaker will explain how case managers can work collaborately with other health care teams to identify the high-risk patient. 1. Communicate with other professionals to coordinate and collaborate on readmission reduction 2. Collaborate with members of health care team to ensure a seamless transition of care 3. Identify patients requiring coordination of services to improve health care outcomes and readmissions Session 3: Understanding Health Policy in an Ever-Changing World Wendy Fletcher, DNP, APRN, FNP-C Family Nurse Practitioner/Professor Northern Kentucky University and Western Kentucky University Morehead, KY Session abstract: This session will help attendees recognize how policy is formulated and describe ways to evaluate the effect of policy on their day-to-day activities. The speaker will also explain how to choose the best methods of action to influence policy in the workplace. 1. Evaluate policy for bias, relevance and motive 2. Describe the process of policy development 3. Utilize three different mechanisms of action to influence public health policy

4 Session 4: CHAMP: Changing High Risk Asthma in Memphis through Partnership A CMS Innovation Grant Susan Steppe, LAPSW Program Director, Le Bonheur Community Health and Well-Being Le Bonheur Children s Hospital Memphis, TN Session Abstract: This session presents the development and implementation of a CMS-funded innovation project to combat high-risk pediatric asthma in Memphis. CHAMP is a collaboration of community service professionals and physicians supporting self-management of asthma to improve health and reduce encounters and health care costs. CHAMP replicates the chronic care model that focuses on coordination of care and reducing barriers to self-management. A key aspect of CHAMP is the development of an asthma registry to track progress in a timely way and use data to gauge success and make changes. The goals of the program are consistent with the triple aim concept. 1. Articulate the concept of triple aim and its importance in emerging developments in health care 2. Identify challenges involved in engaging families to achieve higher levels of adherence to prescribed medical plan and medications 3. Discuss new strategies for effective discharge planning and following up with patients at home Session 5: Why Does Drug Addiction Matter to Me? Dr. Stephen Loyd, MD, FACP Associate Chief of Staff Education, Acting Deputy Chief of Staff Mountain Home VA Medical Center Johnson City, TN Session Abstract: This session will provide a basic understanding of the neurological basis of addictive disease and why people just can t stop. Attendees will also learn how to identify a drug abuser and how missing the diagnosis can make the readmission rates look worse than they need to. What can be done to help? The speaker will examine treatment options and provide suggestions on which options work best. 1. Identify the addictive diseases 2.
