2 INTRODUCTION: Partnering for the future will be essential in the years to come as all of us find new ways to combine our knowledge and skills to provide healthcare on both sides of the hospital door. We are on the cutting edge of healthcare reform. Know what the changes mean to you, your hospital and community. GLOBAL OBJECTIVES: At the end of the conference, participants will be able to do the following from the sessions they have chosen: 1. Describe how case management plays an integral role in managing patient care delivery. (Session 1 - Ulmer) 2. Discuss the cycle of homelessness, and ways healthcare providers can partner with community providers to break the revolving door cycle. (Session 2 Breakout A - Carmody) 3. Evaluate best-practice organizational planning for case management leaders. (Session 2, Breakout B - Pritchard) 4. Explain trends and strategies resulting from Recovery Audit Contractor legislation. (Session 3, Breakout C - Walker, Seward) 5. Analyze how the Affordable Care Act and healthcare reform law may be implemented and impact resources available to case managers and their clients. (Session 3 Breakout D - Silberman) 6. Recognize the ongoing politics of healthcare reform and the role of our ACMA legislative committee. (Session 4 - Wilson) 7. Identify techniques for dealing with stress in our ever-changing healthcare environment. (Session 5 - Benfield) 8. Describe the many challenges of developing discharge plans for difficult patient populations and how solutions are created with limited resources. (Session 6 - Bonds, Holmes)
3 >> KEYNOTE ADDRESS: Healthcare Reform 2013: Operational Strategies for Case Management to Enable Successful System Navigation Nick Ulmer, MD, CPC Vice President, Clinical Services and Medical Director of Case Management Spartanburg Regional Physician Group, Spartanburg Regional Healthcare System, Spartanburg, SC As a Certified Professional Coder, Dr. Ulmer teaches documentation guidelines and Medicare compliance to hospitals. Session Abstract: Healthcare reform has dominated discussions at board rooms all across America. This keynote will help outline some of the Reform concepts and will also define several strategies where case management can play integral roles to help systems manage the new world of patient care delivery. >> SESSION 2, BREAKOUT A: Stopping the Cycle: Connecting Homeless Patients to Benefits with SOAR Emily Carmody, LCSW Project Specialist, NC SOAR State Lead, North Carolina Coalition to End Homelessness Session Abstract: SOAR (SSI/SSDI Outreach, Access and Recovery) provides training and support to caseworkers in the field to build their capacity to increase access to benefits for homeless adults. When homeless individuals have access to income and health benefits, they are better able to access housing and needed medical care to stabilize chronic medical and mental health conditions, reduce readmissions and shorten lengths of stay in the hospital. >> SESSION 2, BREAKOUT B: Utilization Management Plan A Working Session Barbara H. Pritchard, BSN, MBA Director, Consulting Management and Development Joint Commission Resources, Oak Brook, IL Session Abstract: This presentation will focus on the CMS requirement for UM plans, with the speaker leading a brief discussion of the related issue of CMS surveys for discharge planning, involving case management. Attendees may bring their UM plans for discussion with peers and the speaker to determine whether they are compliant.

The session will focus on topics of interest to case managers, including efforts to improve population health, expand patient centered medical homes, and other strategies to improve individual health outcomes. >> SESSION 4: ACMA Public Policy Update J. Suzanne Wilson, RN, MBA Director, Medical Resource Management AnMed Health, Anderson, SC Session Abstract: This session will provide information on ACMA s recent national public policy efforts and priorities in case management advocacy in an ever changing healthcare environment.

Benfield, RN, Med Director, Service Excellence CMC-Northeast, Concord, NC Session Abstract: Rolling with the Waves of Change helps participants learn new techniques for dealing with stress in an ever-changing healthcare environment. Discussion will include examining various stressors, from dealing with change in the job demands to dealing with inappropriate behavior of colleagues. Various methods for finding work/life balance will be explored. >> SESSION 6: Consider This Case: Reflections on Discharge Planning and Other Care Coordinator Challenges Gay S. Bonds, MSW, LCSW Complex Care Coordinator, Patient Resource Management Duke University Hospital, Durham, NC Cynthia Holmes, LBSW, MHA, ACM Manager, Medical Social Work, Department of Medical Management Palmetto Health Richland, Columbia, SC Session Abstract: This is a practical and interactive session regarding discharge planning challenges and ethical considerations within parameters of available resources. The lessons learned from previous sessions will be applied to actual case studies pulled from ACMA s Learning Link. This will serve as a platform for further discussion and provide opportunity to clarify issues associated with complex discharge planning. In order to address the multifaceted delay in placement for patients with complex care issues, South Carolina case managers have created an infrastructure to better bridge the gap between acute and long term care. This has strengthened the partnership with facilities participating in the care of the more complex care patients for optimal outcome. The initiative has allowed for reduction in length of stay, reduction in avoidable re-admission and better outcome for the patient. >> CONFERENCE WRAP-UP: Vicki Vanderburg and Cynthia Holmes Presidents of NC and SC Chapters of ACMA respectively
