This framework for building collaborative relationships can be applied universally. The speaker will discuss how this collaborative approach produces an individualized life plan for the patient, reduces recidivism, reduces cost and helps to preserve the patient s independence whenever possible. It can often be challenging for health care providers in offering emotional support to patients and their families at this time. Health care professionals providing end-oflife care to patients across diverse practice settings are keenly aware that this phase of life is usually accompanied by intense emotions, the need to make crucial decisions for future care involving family and coping with anticipated grief and loss. This presentation will focus on the influence of culture and major cultural considerations at end-of-life. Practical guidelines for delivering culturally competent care will be shared.
