Currently, she leads the client services and sales teams for Phoenix Home Care & Hospice that serves four states. Most recently, Tara s strategic initiatives have centered on ACOs, Bundled Payment, Value-Based Programs and developing a Chronic Care Management program. Tara continues to work closely with Acute Care and Post-Acute Care providers in creating successful partnerships in the Accountable Care Environment. Throughout her career, Tara has had extensive experience developing relationships and promoting integration with acute, post-acute, and long-term care providers and third-party payers across the healthcare continuum. Stephanie Jackman-Havey CPA, CFO & COO, VNA Care Stephanie has over 20 years of experience in Senior Leadership roles in management. She oversees the Information Systems and administrative/facilities services at VNA Care. Stephanie assists in financial analysis to make informed decisions of streamlining, efficiencies and cost containment. She has developed internal/external financial and operational policies and audit controls along with streamlining the managed care and insurance verification departments. Teresa Jennewien COO, Central US ACO As COO of Central US ACO since 2014, Teresa works with 525 providers in a 7 state region administrating and developing the infrastructure for a track 1 ACOs as well as Next Generation ACOs. Prior to joining Central US ACO, Teresa worked at Mercy Health Systems in governmental audit and reimbursement with 7 hospitals, 33 independent rural health clinics, home health agencies, skilled nursing facilities and hospice for a three state region. Mary Ann O Connor BSN, MBA, President & CEO, VNA Care Mary Ann has over thirty-five years of uninterrupted management experience with twenty-four years of direct experience leading a home health organization as President & CEO. In 2005, she led the merger that combined United Visiting Nurse Association with Visiting Nurse Services of CT. In January 2012, she became President & CEO of VNA Care Network Foundation & Subsidiaries. In 2012, Mary Ann led the successful merger of UMass Memorial Home 6

Benita Pope PhD, MHA, RN Director of Business Development, Duke HomeCare and Hospice Benita has over fourteen years of leadership experience in Home Health and Hospice. She has spent the last five years with Duke Health as the Director of Business Development for the HomeCare and Hospice Division. In this role, she oversees the alignment of agency strategies with that of Health Systems, Physician Practices, ACOs, Palliative Care Teams, SNFs, and other community care providers. She also oversees the Technology Integration component of this division ensuring that the value of Information Systems is realized and optimized in current and new programming. Strategically, her focus is on building business through the engagement of innovative collaborations that create value added service for both patients and providers. Benita is currently participating in multiple Physician led Population Health Management teams addressing Care Redesign needs for today and tomorrow s anticipated health care delivery models. Throughout her career, she has worked to promote Home Health and Hospice as a valuable and vital connection to meeting patient experience and industry expectations.
