3 SeSSIoN 3 Understanding Case Management from a Physician s Perspective Ronald Hirsch, Md, FACP Medical Director Case Management Sherman Hospital Elgin, IL SeSSIoN ABSTRACT: A physician advisor from a community hospital discusses his approach to case management and offers insights into key measures to educate and improve relations with physicians, reduce length of stay and lessen the risk of denials. This session will provide a practicing physician s perspective on the current environment of decreasing reimbursement among increased scrutiny, and share views on what practicing physicians and physician advisors expect from their case managers. 1. Understand current constraints on physician practice 2. Interpret medical literature to support evidence-based practice 3. Learn techniques to interface with your physician advisor and other physicians SeSSIoN 4 Health Care Finance: Case Management s Role in the Revenue Cycle Christy Whetsell, BSN, RN, MBA, ACM Director Care Management West Virginia University Hospitals Morgantown, WV SeSSIoN ABSTRACT: This session will discuss how the roles of case management affect the hospital revenue cycle. We will explore the 12 step process and how care management roles and performance of nurses and social workers can have a positive effect on the downstream viability of an organization. 1. Discuss the health care revenue cycle and how it impacts your organization and its patients 2. Understand how process and employees can contribute to the financial success 3. Identify key barriers when steps are missed in the revenue cycle and what must occur to correct the issue Continuing education CE Certificates will be awarded upon completion of the conference. Participants may earn up to 8.7 Nursing Contact Hours (based on 50-min hour) and up to 7.25 Social Work Contact Hours. CE credits can be applied toward ACM Certification renewal.

4 SeSSIoN 5 Keeping all Eyes on the Goal: Palliative Care Challenges in the Acute Care Setting denise R. verosky, MSN Director Palliative Care UPMC Mercy SeSSIoN ABSTRACT: This session will discuss the utility of palliative care for the patient population in acute care through the use of case study presentations. The specific challenges that palliative care can present such as patient s goals for care and treatment, symptom management, capacity for decision making and patient/family and health care team communication will be addressed. Through audience participation, we will discuss each of the cases presented. The speaker will also discuss palliative medicine/care resources available for patients undergoing the experience of a serious disease. 1. Explore the benefits of palliative care for patients with serious diseases 2. Identify resources for palliative care in the community 3.
