This session will provide a practicing physician s perspective on the current environment of decreasing reimbursement among increased scrutiny, and share views on what practicing physicians and physician advisors expect from their case managers. 1. We will explore the 12 step process and how care management roles and performance of nurses and social workers can have a positive effect on the downstream viability of an organization. 1. The specific challenges that palliative care can present such as patient s goals for care and treatment, symptom management, capacity for decision making and patient/family and health care team communication will be addressed. Through audience participation, we will discuss each of the cases presented. The speaker will also discuss palliative medicine/care resources available for patients undergoing the experience of a serious disease. 1.
