2 INTRODUCTION: Case Managers facilitate the flow of patients through healthcare systems by not only excelling in their own area of expertise, but by being keenly aware of the skills and services offered by others. We manage change and know what our patients require in order to thrive. Our keynote speaker, Kelly Swanson, author and motivational speaker, acknowledges the stressors of our work, and then teaches her approach to being happier and more productive despite it all. We have a special guest from the Hastings Center in New York who will explore the ethical challenges of managing the mandates of providing advocacy and care to people in the face of inadequate resources. There are featured sessions on Money Follows the Person. We have an expert on how the Social Security disability (SSI/SSDI) system works. There are panel discussions on care transitions initiatives between hospitals and home health; hospitals and skilled facilities; and healthcare systems and mental health providers. Hear panelists present a model of transitioning chronically ill young adults to adulthood so they gain skills to manage their own health needs. Get the update on current trends in hospital utilization, changes in observation status, and what the 2-midnight rule is really about. We will close with sage advice on how to stay positive in the midst of constant change in healthcare. Hanging on to humor when life doesn t go the way you planned Kelly Swanson, Motivational Speaker, Author Session Abstract: A motivating program designed to make attendees laugh, feel appreciated for what they do and the difference they make. Author will have a booksigning after her presentation. >> SESSION 2: Undocumented People and Access to Care - Ethics Nancy Berlinger, PhD Co-Director, Undocumented Patients Project The Hastings Center, Garrison, NY Session Abstract: NC is a "gateway state" for immigrants. A seriously ill or injured undocumented person can be a "complex" patient due to lack of funds and local caregivers. Case managers face competing mandates to ensure patient safety post-discharge while pressured to "clear the bed;" to develop productive relationships with community organizations while dealing with institutional concerns about becoming a "magnet" for the uninsured. This session provides an overview of common challenges and the case managers role in advocacy. >> SESSION 3, BREAKOUT A: Transition: The Life Highway, a Process - Not an Event Panel discussion: Nirmish Shah, MD, Division of Hematology-Oncology; Vivian Lewis, Child Life Specialist; Amanda Rozycki, MSW, LCSWA, Shelia Rittgers, MSW, LCSW, Dept. of Pediatrics, Duke University Hospital, Durham, NC Session Abstract: This panel will explore barriers to transitioning young adults from pediatric to adult care, and what is being done to promote patient involvement and prepare them for the transfer of care. Emphasis will be on strategies to empower and educate teens and young adults and using the electronic medical record as a tool to track progress.
4 >> SESSION 3, BREAKOUT B: Money Follows the Person Demonstration Project Trish Farnham Project Director NC Dept. of Health and Human Services, Division of Medical Assistance, Raleigh, NC Session Abstract: NC Money Follows the Person provides an opportunity to individuals with long-term needs to return to their homes and communities after prolonged institutionalization in skilled nursing facilities or hospitals. Learn how MFP partners with other publicly funded long-term support services, such as home and community-based waivers, PACE and other non-medicaid services. >> SESSION 4, BREAKOUT A: Partnering with Skilled Nursing Facilities for a Successful Population Health Program Panel discussion: Lynn Watkins, PT, MSPT, MHA; Angela Alley, RN, BSN, MHA; Stacy Wright, MBA, LCSW, Population Health and Case Management, Novant Health, Winston-Salem, NC Session Abstract: Panelists will describe how Novant Health is fostering the evolution of a SNF and health system partnership to provide the best patient-centered system of care. The partnership is a key component to improving patient health, health care services and decreasing costs of care. >> SESSION 4, BREAKOUT B: Disability 101: The Basics of Supplemental Security Income (SSI) / Social Security Disability Income (SSDI) and Documenting Disability Judith Romanowski, RN, JD Attorney PLLC, Durham, NC Session Abstract: This session will help discharge planners assist patients who are disabled by health impairments and deserving of disability benefits. It will provide an overview of how the Social Security disability (SSI/SSDI) system works, and dispel some of the myths that many health care providers and patients hold about the system. The session will focus on how discharge planners can avoid common pitfalls; how they can document disability issues; and how they can advocate for their disabled patients.

Discharging Mechanically Ventilated Patients to Home Panel discussion: Kathy Short, RRT, RN; Diane Labiak, MSW, LCSW; UNC Hospitals, Chapel Hill, NC; Stacey Rice, RN, BSN, Bayada Home Health Care, Greensboro, NC Session Abstract: A respiratory therapist, hospital discharge planner, a former patient, and a home care transitional care manager will discuss the process of successfully discharging mechanically ventilated patients with complex needs and limitations to home. >> SESSION 5, BREAKOUT B: Yours, Mine, Ours: New Ways of Collaborating for a Shared Population in a Changing Environment Ann Oshel, MS Chief Community Relations Officer Alliance Behavioral Healthcare, Durham, NC Session Abstract: No longer are we able to improve patient care and cost savings by working in siloes. Using a System of Care approach, this session will present strategies to not only improve case management practices but to develop the community collaboration required to achieve optimal outcomes. We will discuss how to eliminate common barriers (i.e., information sharing) that will help collaboratives be more successful. >> SESSION 6: Behavioral Health Integration (MH), Carolinas HealthCare System Panel discussion: Melissa Candela, LCSW; Linda Thomas RN, Nurse Manager of Patient Placement; Christine Zazzaro, LPC, Director, Behavioral Health Patient Placement, Carolinas HealthCare System, Charlotte, NC Session Abstract: Through innovation and technology, Carolinas HealthCare System has created a virtual model of Behavioral Health Integration to increase patient access to needed behavioral health services, while also increasing treatment education to Primary Care Providers within the system.
