Awardees are recognized for their exceptional leadership in their specific area that increases the visibility of important issues, creates solutions to significant problems, and positively impacts the overall quality of care in long-term care CALTCM Leadership Award Recipient KJ Page, RN-BC, LNHA KJ Page is a licensed nursing home administrator and a registered nurse board certified in gerontology. She has served as an Administrator since December KJ has led Chaparral House to become one of the few Skilled Nursing Facilities to be Joint Commission Accredited, Post-Acute and Memory care certified. Chaparral House focuses on innovative personcentered solutions to complex problems and communication concerns along the long-term health care spectrum. 3 ! 3 of 10! 2017 Pre-Conference CALTCM Best Practices Summit Pre-Conference Overview This conference serves as a platform for sharing and disseminating best practices in long-term care. In collaboration with Health Services Advisory Group (HSAG), the Medicare Quality Improvement Organization for California, five best practices on overcoming various long-term care challenges will be showcased to help spread innovation throughout the community. Attendees will have the opportunity to develop action plans supplemented with structured coaching to foster sustainable adoption of these practices. Target Audience Physicians, advanced practice nurses, gero-psychiatrists, pharmacists, licensed nurses, administrators, and other members of the interdisciplinary team who care for patients and residents in Post-Acute and Long-Term Care. Learning Objectives By participation in the pre-conference, participants will have the ability to: 1. Describe best practices on managing chronically ill patients; 2. Utilize Quality Assurance & Performance Improvement (QAPI) techniques for creating, modifying and sustaining implementation plans; 3. Identify barriers and develop contingency plans to overcome barriers for implementation; 4. Disseminate resources and tools for facility-wide performance improvement project (PIP) implementation plans. Accreditation This event is accredited for up to 4.75 continuing education credits. 5 ! 5 of! 10 43rd Annual Meeting Quality Through Best Practices Friday & Saturday, April 28-29, 2017 Program Overview The CALTCM 43rd Annual Meeting entitled Quality Through Best Practices is focused on an interdisciplinary approach to address common conditions seen in long-term care. From utilizing a QAPI approach to developing best practices, to interactive case studies, participants will review the latest in evidence-based care of frail, older adults. The 43rd Annual Meeting features three half-day sessions: Friday Afternoon: Management of Diabetes, Metabolic Syndrome and Obesity Saturday Morning: Managing Pain and Depression Saturday Afternoon: Just Culture and Patient Safety Target Audience Physicians, advanced practice nurses, gero-psychiatrists, pharmacists, licensed nurses, administrators, and other members of the interdisciplinary team who care for patients and residents in Post-Acute and Long-Term Care. Learning Objectives By participation in the annual meeting, participants will have the ability to: 1. Examine common conditions occurring in post-acute/long-term care residents from an interdisciplinary perspective; 2. Describe common nutrition-related issues in post-acute/long-term care residents; 3. Evaluate the risks and benefits of anticoagulation in the post-acute/long-term care setting; 4. Identify fall risk in nursing home residents from a person-centered perspective; 5. Explain the latest changes in the Nursing Home Requirements of Participation.
