Case management is increasingly being assigned to manage the coordination and work of these changes. This session will identify and discuss the top health care agenda issues and how they may affect the daily work of case managers. In addition, the speaker will discuss projects currently in the pilot-state related to transitional planning and re-admissions. Lastly, the session will discuss how case management can form partnerships to deal with proposed health care reforms. 1. This session will share an early view of two pilots being developed at Southern California hospitals. Bedside rounding is being utilized in patient-centered care to empower the patient to be a valuable participant. The role of the case manager, use of criteria sets and team dynamics will be shared from this initial pilot program. This session will also illustrate the collaboration of ED nursing staff and social work to implement criteria based case management. This test of change model in the ED was developed after an unsuccessful recruitment campaign for RN case managers to cover the ED. 1.
