As case management professionals, we are challenged every day with problem resolution it is a role that never ceases and is multi-faceted. This session will provide a practicing physician s perspective on the current environment of decreasing reimbursement among increased scrutiny, and share views on what practicing physicians and physician advisors expect from their case managers. 1. It will examine how to create an admission review process that proactively addresses the appropriateness of readmissions. Additionally, as part of a case study from a hospital participating in the CMS Care Transitions Project, this session will outline a step by step approach to implement a consistent process that can not only ensure regulatory compliance but also create revenue integrity. 1.
