2 Statements & Search Criteria Statements Mission Statement The organization's mission is to nurture the healing ministry of the Church by bringing it new life, energy and viability in the 21st century Fidelity to the Gospel urges us to emphasize human dignity and social justice as we move toward the creation of healthier communities. The Saint Joseph Hospital Foundation s mission is to assist the health care professionals of Saint Joseph Hospital, Saint Joseph East and Saint Joseph Jessamine by raising and allocating funding for innovative programs and projects that enrich the health of patients, family and community in the tradition of the Sisters of Charity of Nazareth. The greatest benefit of supporting Saint Joseph Hospital is the knowledge that you are making a difference in the lives of others. Gifts like yours enable us to continue our mission that the Sisters of Charity of Nazareth started over 133 years ago; to provide compassionate care to all who seek it. Background Statement Established in 1989, to serve our patients and families, the Saint Joseph Hospital Foundation staff and board members are entrusted to receive all charitable gifts to the Hospital, responsibly steward those funds, and ensure they are used in accordance with the donor s wishes. We are a tax-exempt, non-profit corporation 501 (3). All contributions are fully tax deductible as allowed by law. Annually, over $1 million is raised in Lexington from generous supporters in support of the core values and mission ofsaint Joseph,Saint Joseph East, Saint Joseph Women s Hospital, and the Saint Joseph Jessamine RJ Corman Ambulatory Care Center that bridge the gap between medicine and compassion. Impact Statement The pioneering spirit and unwavering focus on compassionate care that characterized the founding of Saint Joseph by the Sisters of Charity of Nazareth in 1877 continues to manifest today as the hospital responds to changes in medical science, technology, the community and the economy. In fiscal year 2011, through the support of our donors, grateful patients, physicians, clinical and non-clinical associates, hospital and foundation boards, and other community members, collectively the (5) Saint Joseph Hospital Foundation's in Kentucky raised over $2.1 million for the mission-related programs, services, and technological needs. The top accomplishments for 2011 include funding major capital & technological improvements at each of our facilities, awarding thousands of dollars in nursing & allied health scholarships and providing mission and outreach services to Kentucky's most vulnerable. This was achieved by surpassing our fundraising goal despite a down economy and local and national disasters. In 2012, KentuckyOne Health was formed when two major Kentucky health care organizations came together in KentuckyOne Health combines the Jewish and Catholic heritages of the two former systems Jewish Hospital & St. Mary s HealthCare and Saint Joseph Health System.The nonprofit system is committed to improving the health of Kentuckians by integrating medical research, education, technology and health care services wherever patients receive care. KentuckyOne Health has nearly 200 locations including hospitals, physician groups, clinics, primary care centers, specialty institutes and home health agencies, with more than 13,600 employees across the state of Kentucky and southern Indiana. KentuckyOne Health is the largest health system in Kentucky and has more than 2,325 licensed beds. 4 Programs Programs Appalachian Outreach Program (AOP) Description Category Population Served Program Short Term Success Program Long term Success Appalachian Outreach Program: Our award-wining AOP Program was established in 1986 by Sister Betty Shelton and the Sisters of Charity of Nazareth to provide follow-up and support services for patients and families in 27 central and eastern Kentucky counties after they leave Saint Joseph Hospital. Today the Sisters and others offer pastoral, social and nutritional services and other short-term services such as food and clothing, bringing our ministry to their homes. Pastoral Services: Compassionate Listening, Grief Counseling, Spiritual Counseling, Family Support and Prayer Social Services: Referral Assistance, County Agencies, Physicians Offices Nutritional Services: Dietary Assistance for: Diabetes, Heart Disease, High Blood Pressure, Weight Management and other Nutritional Problems Meal Planning for Wellness: Registered Dietitian Services. Health Care, General/Other Home Health Care At-Risk Populations, Poor,Economically Disadvantaged,Indigent, Unemployed, Underemployed, Dislocated Now serving 27 Kentucky counties, Dr. Dan Varga adds, "One of the things we are trying to emphasize is getting primary care services close to where people are. If we don't, there is a high likelihood they'll end up in a much more expensive venue of care, much sicker and less likely for a good outcome." Celebrating 25 years of serving Appalachia. When she started the outreach, Sr. Shelton traveled the back roads in nine counties to help patients who had been discharged from Saint Joseph Hospital. In 1991, Sr. Joan Wilson, SCN, joined her, and the program grew to 15 counties. Two years later, a nutritional counseling component was added with a third staff person who delivered help to patients through home visits, phone calls and mailings. "The program is and has always been very grassroots," said F. Rose Rexroat, community services and virtual care manager. "The sisters were instrumental in figuring out and helping these patients get what they needed and putting them in touch with community resources, and really focus on not only making sure patients have the resources they need to stay home and be independent, but allow them to increase their self-management skills, avoid hospitalization and coach them to avoid high-cost emergency visits." 4

5 Program Success Monitored By Examples of Program Success Dietician Barbara Baumgardner mostly sees patients in hour-long sessions at local clinics in 13 rural counties. Many are dealing with cardiovascular ailments and hypertension, or suffer from health concerns related to obesity and diabetes. Much of Baumgardner's time is spent educating patients about the importance of exercise and eating healthfully, developing personalized meal plans and helping them to overcome obstacles."some have concerns that eating healthy is expensive," she said. "I try to show them how they can get good, clean food, like high-fiber beans and seasonal fruits and vegetables, cheaply. "Baumgardner says obesity has become even more widespread in Appalachia then it was when she first began her outreach practice nine years ago. "Patients also tend to be sicker," she said, "because they don't have the ability to pay and wait longer to come for help. When they find out our service is free and we come to them to deliver care, they are just so thankful." Count Nelda Marshall among the grateful patients. After losing 80 pounds following her gastric bypass surgery, then gaining 16 in the few years that followed, she has taken off five since June when she began seeing Baumgardner. "Barb has gotten me back on track and keeps me motivated. She is my stabilizer," said Marshall. "When we meet, we discuss everything I am eating and critique how I am doing and see what might help. She found me a low-carb yogurt that I can use for a snack. I am crazy for yogurt, but the kind I had been eating had too many carbs. She has made all the difference in helping me be healthy." Nursing & Allied Health Scholarships Description Nursing & Allied Health Scholarship: Saint Joseph Hospital began with dedicated nurses in It is imperative to recruit and retain the best and brightest team of health care professionals as they earn nursing degrees and advanced credentialing. Nursing Scholarships reap long term benefits and rewards for both the donor and the scholarship recipient. In 2001, the Foundation created the Nursing and Allied Health Scholarship Fund. The program provides one and two-year scholarships to deserving students at area colleges and universities.to date, the Foundation has provided 34 Allied Health Scholarships and 87 Nursing Scholarships totaling over $392,350. Population Served,, Clinical Pastoral Education (CPE) Description Category Population Served Through the Clinical Pastoral Education Program Saint Joseph Hospital has been a leader in pastoral care by facilitating a specialized (CPE) program that hires and trains future chaplains who minister to patients, families and employees. CPE helps future chaplains to integrate their theological and pastoral training into real life situations of human needs and suffering. Having this educational opportunity allows us to train future pastoral care providers in our ministry which is a requirement for their seminary degree. As part of the Core Values of Saint Joseph we continue to drive the mission of Lexington s oldest hospital: Reverence for those we serve; Integrity in what we do; Compassion for those less fortunate; and Excellence in the service delivered. 6 Grateful Patient Program Description Provides capital, technological and educational needs including the Transitional Care Clinic, Continuing Care Hospital, Saint Joseph Cancer Center, Saint Joseph Heart Institute and Women and Children s services. Immediate needs include addingtherapeutic Kentucky art as considerable research shows art is not only aesthetically pleasing but it also improves patient moods, helps shorten the length of stay, and assists the healing process at a quicker pace.a serious diagnosis often leaves patients feeling lost, sad, depressed and full of emotions they need to express. With these emotions in mind, art can provide a creative outlet for patients to share their emotions. We value art as treating the mind, body and spirit. Population Served,, Mamm!!! Description Free Mammograms:, Mamm! If the walls of our KentuckyOne Health mammography locations could talk, they would tell stories of fear and relief, heartbreak and courage, faith and healing. Stories told from grandmothers, mothers, wives, sisters, aunts, daughters, neighbors, colleagues and friends., Mamm! began as a need the Foundation identified three years ago. Underserved men and women were foregoing mammography due to financial pressures. The Foundation put together a funding plan to connect donors, grants, and cancer related organizations collaborate and work together. In all over $60,000 was raised last year and as a result of the popularity of, Mamm! funds will be exhausted before all the people that need our help can be served. When you walk in the Susan G. Komen race, buy a pink Kentucky license plate, attend the Evening With The S*T*A*R*S gala, your donation helps provide resources that save lives right here at home. Over 570 Kentucky women will die of invasive breast cancer this year. Thanks to our pink partners and generous donors assistance is available for screenings regardless of ability to pay. Dr. Kimberly Stigers, Foundation Board Member and Radiologist at the Saint Joseph Breast Center has personally witnessed the impact this funding has provided her patients., Mamm! has helped us develop a seamless way we can help patients and families maneuver through the breast screening, diagnosis and treatment plan process. We are grateful and happy to serve all patients with the best care possible and this funding has and will continue to save many lives. Breast cancer is non-discriminating. It has hit close to home with many members of the KentuckyOne Health family personally affected. Population Served,, Program Comments CEO Comments Because of the ever changing needs there is a constant need to raise dollars privately and reinvest in our people and facilities.there are several programs and services that the Foundation proudly supports that build 6

7 healthier communities and improve the qualities of lives for our patients and their families. Also known as Lexington s heart hospital,saint Joseph has pioneered many firsts in the health care community. Beyond cardiology, the hospital is also nationally recognized for treatment of orthopedics and stroke. These and several other programs meld to establish Saint Joseph as a nationally ranked healthcare facility. More than 50 Kentuckians die of cardiovascular disease each day making it the state s leading cause of death. The Heart Institute is central Kentucky s pioneering heart and vascular care center. The Institute is home to an innovative new cardiac catheterization lab, electrophysiology lab and the first fully accredited noninvasive services department. Our team of nationally renowned cardiologists and cardiovascular surgeons perform the full spectrum of invasive, noninvasive and surgical procedures. In 2011, da Vinci robotic technology was added to cardiac surgical options. Many traditional heart bypass or valve replacement surgeries can now be accomplished through three small incisions rather than the traditional 10 incision through the breast bone. The result is less pain and faster recovery times for many of our patients. 7
