2 Statements & Search Criteria Mission To enhance the health and quality of life of older adults and adults with disabilities by providing a structured, supportive, supervised, enriching and peer-interactive environment which can support wellness and promote socialization and emotional well-being. Background In 1975, well before the community at large was thinking about the needs of a growing elderly population, two visionary pastors- the Reverend Dr. Dwight Jones of First Baptist Church, South Richmond, and the Reverend Don Raby Edwards of St. Stephen's Episcopal Church, saw the need for a program where the well elderly and recently retired individuals could come together daily for companionship and socialization. They founded the South Richmond Senior Center to meet this need. Over time, the Center evolved to meet the needs of the participants and community, becoming today's licensed, Medicaid-certified South Richmond Adult Day Health Care Center. In 41 years of continuous operation, SRADCC has observed a steady increase in the level of care that participants need at the time they begin services. Based on this trend and national statistics, we expect that an even larger percentage of our participants will enter services with significant ambulatory difficulties and laterstage cognitive and physical illness and disabilities. With the anticipated Age Wave and resulting increase in the number of individuals who will require some type of care as they age, the need for community-based services such as adult day health care will continue to grow. Adult day services play an important role in meeting the care needs of today s population and may hold the answer to the pressing question, How can we meet our future care needs in a fiscally efficient and ethically responsible manner? SRADCC is a key component in spearheading the Greater Richmond Age Wave Plan to ensure that our region is prepared and able to respond to this population's health and wellness needs. Impact Accomplishments: Maintained a podiatry clinic in partnership with Earnest Foot and Ankle, PLC, which is providing foot care to 10 participants, helping identify potentially debilitating conditions, reducing the potential for falls, and avoiding complications associated with diabetes and other conditions. Hired a clinical nurse manager who expands on-site health assessments, expedites Medicaid applications, and troubleshoots barriers to Medicaid enrollment. Made significant facility upgrades and repairs, including new plumbing, new heating and air conditioning, magnetic locks for bathroom stalls and upgraded commodes and sinks. The improvements increased participants comfort and safety and increased efficiency. Established a relationship with the Hull Street Library to provide activities to participants. Established a relationship with Blackwell Elementary School to provide inter-generational activities. In FY , the Center served 29 participants, enabling them to remain living in their homes and communities, where they want to be, preventing 8,191 days of institutional care at an estimated savings of over $1.4 million to caregivers and the community. Goals: Continue to increase caregiver and staff knowledge about healthy eating, diabetes prevention, screening, and care, which will help stabilize blood sugar levels and increase adherence to prescribed therapeutic interventions. Provide additional information and experiential learning activities for caregivers on stress reduction. Establish an art therapy program. The Center will host residents of a local assisted living facility, who will participate along with Center participants. Caregivers and staff of both locations will receive training. The Center is also working to establish a massage therapy program. 2
3 Needs The community's need for Adult Day Health Care Services is growing rapidly. Within the next 20 years, adults over age 60 will outnumber school-aged children for the first time in history. This dramatic demographic shift, known as the Ave Wave, brings with it significant changes. Adult day services are a critical component of our community's Age Wave readiness. The Center serves vulnerable older adults, the vast majority of whom are low-income, African American women ages All experience significant impairment in one or more activities of daily living and have a high prevalence of multiple, chronic conditions including dementia, diabetes, heart disease, hypertension, and psychiatric diagnoses. Their family caregivers need respite to work, maintain other life roles, and reduce caregiver stress and burnout. To meet these needs, the Center seeks: 1. Participant scholarships to offset the gap between participants' ability to pay and the cost of services, an average gap of $600 a month per participant; 2. Ongoing funding for salary and benefits ($60,000) for a full-time clinical nurse manager who provides ongoing, on-site health services; 3. Volunteers for center-based activities, caregiver support group meeting speakers, office assistance, board and committee positions, fundraising, etc. CEO/Executive Director Statement The provision of Adult Day Health Services continues to be rewarding, fulfilling, effective yet challenging. The services provided are vital, necessary and make a significant difference in the lives of participants. Caregivers validate the benefit, and effect these services have on their loved ones, and are willing to share their feelings and gratitude regarding the positive impact of Adult Day Health Care on the participant and the family. We continue to expand the vision for South Richmond Adult Day Care Center to include additional health services to meet the needs of a changing older population with complex medical challenges. Although it is a challenge, we are confident our participants will benefit from these services. As the population ages, we must be ready to meet their needs and ensure these individuals can remain in the community with their families and caregivers, and receive community based services which are beneficial and cost effective. 4 Programs Programs Adult Day Health Care Center Description South Richmond Adult Day Care Center (SRADCC) is a communitybased, structured, supervised program serving older adults who are cognitively and physically impaired and can no longer remain at home alone. SRADCC offers care and services to reduce isolation and promote mobility, wellness and socialization. Participants benefit from regular health assessments and monitoring, music, arts and crafts, exercise, games, discussion groups, special celebrations, mid-day meals and nutritious snacks to improve functioning, memory, and strength. Students come from two elementary schools and two preschools for intergenerational activities. Caregivers receive support, education, links to services, and respite enabling them to provide quality care and work or maintain other life roles with less stress and guilt. SRADCC also helps to ensure that the community has health supports, services, information and referral, and education that provide for the changing needs of the population as it ages. Budget $303, Strategy Long term Success Short Term Success Yes Our participants experience reductions in unnecessary hospitalization and/or institutionalization. A minimum of 80% of participants will remain living in their homes and communities as healthy, well, and independent as possible despite being eligible for nursing home placement. A day of care at SRADCC costs an average of $50, compared to an average of $198 for a semi-private room in a nursing home, a level of care for which all participants qualify. As a result of SRADCC services, in the last fiscal year, 8,191 days of institutionalization were prevented, at a savings of $1,439,589. Also, 94% of caregivers report that SRADCC services enable them to work and maintain other important life roles. 4

Prior to her position at the center she was the Director of Wellness at Lucy Corr Village in Chesterfield, Virginia. Ms. Watson holds a Master's degree in Public Administration, a Bachelor's degree in Therapeutic Recreation along with a Master of Certificate in Project Management. She has over 17 years of healthcare experience with a significant focus on Alzheimer/dementia research, therapeutic recreational program and implementation, and best practices on how to serve this unique population and their families. Many older adults are not able to monitor and care for their own feet and professional care can be cost-prohibitive. Medicare and Medicaid do not cover routine foot care. IVNA provides on-site vaccinations to participants, staff, and community members. Imani Mews apartments residents participate in education opportunities at SRADCC. SRADCC is a field placement site for students from Bon Secour College of Nursing, Virginia Union University School of Social Work, Virginia Commonwealth University's Occupational Therapy and Social Work programs, and St. Catherine's University Occupational Therapy Assistant program. Students receive valuable community-based geriatric care experience, leading some to pursue careers in gerontology, a field with rapidly growing demand. 7
