2 Mission & Areas Served Statements Mission Statement Mercy and Truth's mission is to provide health care to the underserved and uninsured of our community. Background Statement Over the past several years Mercy and Truth Medical Missions has served over 24,000 residents of the greater Kansas City area. We have stretched our "thinking" and added more clinics and grown our international department to increase our services to the marginalized and uninsured. We view community involvement and relationships as vital to our success as an organization and key to our sustainability. To date MTMM has established two safety net clinics within the Kansas City Metro area. Both clinics are established to provide health care to clients regardless of their geographic residential location, status in the United States, income, or ability to pay. Our services are comprised of primary care for all ages including specialty services such as women's health, laboratory, acute and chronic illness, pediatrics and preventative health care. In addition to our local efforts, internationally we have sent well over 75 volunteer teams to aid in developing countries. The teams have delivered health care and education to communities that are geographically diverse, ranging from the bush of Africa to the raging waters of the Amazon. Each international team member serves in their health care specialty whether it is nursing, midwifery, primary medicine, or non-medical professionals. Impact Statement Mercy and Truth continues to faithfully serve the uninsured and under-served of our community in the Kansas City metropolitan area and in various countries around the world. Last year, our two clinics in Kansas City, Kansas and Shawnee Kansas served over 5,103 unique patients. Approximately 68 % were uninsured and 32% had Kansas Medicaid. Mercy and Truth had a 54.54% increase in number of patients served over the prior year, and a 1,300 % increase the previous year. Mercy and Truth added a full time physician to the staff which expanded our service hours. Dr. Alex Kieu is building relationships with our patients and forming partnerships with other organizations. Mercy and Truth is also looking to add a physican to its Shawnee location. At the start of 2016, we opened up with newly renovated facilities in Kansas City, Kansas. These renovations were funded by individual donations and we now have a state-of the art facility and examination rooms. During the final quarter of 2018, we will be relocating our Shawnee location and more than doubling our clinic space. Top 3 accomplishments for 2017: Provided quality healthcare to the underserved and uninsured while increasing patients that were seen. Researched larger locations for Shawnee clinic and utiized tax credits to fund expansion. Built strong referral and relationships with other organizations in Wyandotte County. Top 3 Goals for 2018: 15% increase in number of clients served in Wyandotte and Johnson County Expand Johnson County Clinic facilities to keep up with anticipated growth. Increase strategic collaborations with other community organizations to facilitate referrals and better service for our clients. Needs Statement Mercy & Truth Medical Missions has identified 5 priority needs to improve our organization and our service to our community and world. 1.Financial operating support of primary care clinics to offset the "inability to pay" clientele. 2.Funding for a full time medical doctor at the Shawnee KS clinic facility. 3.Passionate staff interested in serving marginalized populations in Kansas City. 4.Funding to aid the high volume of unemployed who still need health care. 5. Funding for an International Outreach Scholarship to assist medical professionals interested in travel but are unable to serve due to financial difficulties. 2
3 CEO/Executive Director/Board Chair Statement Mercy and Truth is thrilled to have a newly renovated clinic in Kansas City Kansas where we can serve more patients and serve them more efficiently. With the addition of a full time physician, we are now open over 40 hours per week and are providing quality care to every patient that comes through our doors. In the fall of 2018 we will move our Shawnee clinic to a new, larger location where we can see more patients in an updated environment. This will allow for increased access to care for the 45,000 Johnson County individuals who have no health insurance. The second clinic is located in Shawnee, Kansas. Johnson County has 45,000 uninsured individuals with only two safety net clinic options. With our increased clinic space, we can serve more of these residents. 3
4 Programs Programs Core Operation Expense Description Category This project addresses the fine balance of core operating expenses of the safety net clinic sustainability while providing health care to povertystricken communities and to those who cannot afford health care insurance. Safety Net clinics serve uninsured or underserved clients. Clinics that offer low cost, affordable health care with a sliding scale for those who fall at or below the federal poverty guidelines for income. Services provided at the clinics include primary care for all ages, full service laboratory, immunizations, and vaccines for children program, acute and chronic care, and preventative health care in education and client evaluations. Our specialty services include women s health, pediatrics, and diabetes. The cleints inability to pay for needed health care services is not only faced in our current economic downfall but as a growing national concern in Amercia. Tough times not only for clients but also the health care sector. Decreasing revenues due to reduced state and national funding, clientele inability to pay and decrease general support make it increasingly difficult to complete core operations successfully. We have two locations strategically established geographically to serve those in greatest need across both Wyandotte and Johnson Counties. Our clinics provide health care to clients regardless of their geographic residential location, status in the United States, income, or ability to pay. Our services are comprised of primary care for all ages including specialty services such as women's health, laboratory, acute and chronic illness, pediatrics and preventative health care. As Mercy and Truth continues to grow, our objective is to increase our scope of care to communities throughout Kansas City and develop a rapport that invites those with limited resources to find assistance through our care and services. Health Care, General/Other Ambulatory & Primary Health Care Adults, Children and Youth (0-19 years), Adults 1. Increased access to health care - more people are being served because of the clinics inception. 2.Decreasing wait time to be seen. 3.Community awareness 4. Managing budget - opening a clinic in the core of Wyandotte County revealed a much poorer community therefore making budgetary constraints, thus initiating creative strategies for success 1. Increased access to health care 2.Every child will receive annual health care visits including dental care 3.Preventative care, annual visits 4. Reduction in preterm labor - due to health care education received during preconception and during prenatal visits. 1. Data collection of total number of clients seen by a provider, including age, gender, residential location specialty care and those not seen by a provider. 2.Statistical analysis of clients served 3. Statistical analysis of referrals to specialties including medical assistance programs. 4. Budget management monthly on actual revenue compared to lost revenue based on the following factors: inability to pay, bad debt, sliding scale reduction and Kansas Medical Assistance program reduction. 5. Data collection on client income compared to the federal poverty guidelines. 6. Monitoring the cancellations and no-show clients with qualitative interview for quality control improvement markers. 7.Patient Satisfaction: Surveys (HRSA format). Success can be assessed in a variety of ways. 1.Becoming a "Medical Home" 2.Providing preventative health care services 3.Providing specialty care such as asthma, prenatal and dental. 5
6 International Outreach Description Category In addition to local services, Mercy and Truth Medical Missions is dedicated to serving indigent populations internationally. Our work recognizes the plight of the poor and marginalized by seeking ways to combat structural violence and injustices that leave so many across the world destitute and consequently disadvantaged. Our efforts to fight these injustices are centered on the provision of medical services. Mercy and Truth desires to extend medical care and education to communities by forming teams of medical and non-medical professionals to go and serve. We seek to utilize our skills and resources to help bridge the gap of inequalities plaguing developing nations. As an organization, Mercy and Truth has sent over 75 medical teams to over 50 countries since its inception. We work throughout the year, traveling to areas including Mauritania, Malawi, Myanmar, Kenya and others. As our international department continues to grow, our desire is to expand our approach in serving developing nations to help provide sustainable measures that empower individuals and highlight the strengths within their communities. Mercy and Truth hopes to equip individuals to stand in opposition to injustices that afflict their community and subsequently establish a healthier population. Health Care, General/Other Preventive Health Population Served US& International,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Through our international outreach program, we intend to send six medical teams to developing nations to serve and provide necessary medical services. This would be a 50% increase from the previous year. Additionally we intend to organize three scout trips within the next year to ascertain information for additionally areas we can go and serve. Lastly, we intend to create an assessment tool that will assist on scout trips to gather pertinent information about needs and resources of the communities to help guide our services internationally. Our overall goal for our international outreach program is to improve health in developing nations. Measurable outcomes to meet this end include, decrease in prevalence of preventable infectious diseases in the areas we serve by 50%. Additionally, we hope to increase our presence in developing nations throughout the world to utilize our resources abroad. We hope to increase our international outreach trips by 50%. Lastly, we intend to create educational programs that can be utilized to assist in the prevention of disease and promotion of health in the areas we serve. To ensure success of our program we plan to create an evaluation tool to monitor the effectiveness of our educational tools in communities we serve. Additionally, we intend to utilize health records at community level facilities to monitor the incidence of infectious diseases. We have successful sent three medical teams to international sites to provide medical services to communities without access to necessary care in the past year. On average we assisted approximately 700 patients on each trip. We provided care that addressed needs including, infectious diseases, wound care, women's health education, and chronic conditions including diabetes and hypertension. 6
7 Provision of Services to Resettled Refugees Description Category As part of our local services, Mercy and Truth Medical Missions works to assist resettled refugees who have suffered unimaginable pains due to ethnic strife, religious persecution, and economic disparities by providing medical services in a welcoming and culturally appropriate way. We work in partnership with Catholic Charities to provide initial physical screenings to resettled refugee populations, as mandated by state and national principalities. Mercy and Truth recognize that the need for services directed towards resettled refugees is great in the heart of our city. It has been estimated that approximately refugees were resettled in Kansas City, KS annually. (This number has dramatically decreased recently due to the political environment.) The vast majority of these refugees migrated from Burma, Bhutan, and Somalia, followed closely by Iraq and Vietnam. To accommodate the extensive differences of ethnicities and cultures, we work with certified medical interpreters to ensure the care we provide is equitable to all our clients and culturally appropriate. As Mercy and Truth continues to grow, our intent is to increase our preventative and primary care services to refugee and immigrant populations. We also hope to be recognized as an organization that provides medical care to all the people. Health Care, General/Other Preventive Health Population Served US& International,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Mercy and Truth attends the health refugee meetings that are currently hosted by Catholic Charities. This meeting involves social workers, community health workers and medical providers. As an organization, our short-term goal includes increasing access to healthcare resources for newly resettled refugees. Our long-term goal is to increase our services to resettled refugees in Kansas City, Kansas by 20% over the next five years. We intend to expand our services to serve as a medical home for all refugees in Kansas City, Kansas. In addition to initial health screenings, we intend to provide primary medical services to refugee clients we serve. To measure the success of our short-term and long-term goals we will continue to offer successful screenings. We will evaluate data obtained from annual reviews of medical documents to ensure effective screenings and prevention of transmission of communicable disease, development of chronic conditions, and prevention of poor health behaviors. We also intend to use an evaluation tool to measure the success of health education courses provided my Mercy and Truth to assess behavior change. In 2018, we are planning to see 200 resettled refugees at our Kansas City, Kansas Clinic. We provide screenings, preventative care, and treatment for chronic conditions including, hypertension and diabetes. We also assisted in providing necessary health care referrals for patients with conditions including, pregnancies, neurological deficits, heart abnormalities, and other cases needing surgical interventions.overall, we have been successful in providing health care services and assisting the refugees in navigating the U.S. health care system to newly resettled refugees in Kansas City. 7
8 CEO Comments Our mission, "to provide healthcare to the uninsured and underserved has proven to be an effective and positive statement for meeting health care needs in our community and world". Not only has our "business" base expanded but also our service to people within our community. Since its inception, MTMM has demonstrated that our passion, coupled with our faith, can influence our community and world and will ensure our success. Partnerships with local churches, organizations, and community groups have aided MTMM. To date, MTMM operates 2 separate clinics in the greater Kansas City area. In addition, we routinely send approximately 6-8 volunteer medical teams annually to provide health care in developing countries. The growth and development of MTMM causes us to focus on the strengths of our relationships and our desire to listen to our clients so that we can meet their needs. 8

Organization Comments SUSTAINABILITY: Based on Mercy and Truth Medical Mission s model of providing affordable health care - we have tremendous confidence in our long-term sustainability. Our projects have been completely sustainable, keeping administration costs well below 10% of overall budget. Any requests for funds have been for special projects needs and start-up funds to purchase additional properties, equipment, and services. Having a fee for service, accepting Kansas Medicaid, and in future adding Medicare and private insurance will help us maintain our project ongoing without the aid of additional grants or funding requests. We plan to continue to utilize the tremendous resources available to us as a non-profit organization, specifically volunteer support and in-kind services, in order to keep our costs to a minimal level. Partnerships with outside organizations in providing in- 14
15 kind gifts, volunteers and other foundations has demonstrated that our work at MTMM is sustainable, ongoing, and growing. We strongly believe that MTMM clinics will be hope to the hopeless here with the urban and suburban core of our city.
