2 Mission & Impact Statements Mission To serve as a connection point for the residents and resources of 37138, offering assistance with basic needs and encouragement for life challenges. Background In 1985, several of the Old Hickory churches decided to pool their resources to help the needy within the Old Hickory area. With the help of the Old Hickory Ministrial Alliance, the Christian Community Ourtreach was chartered with the State of Tennessee on February 20, The Center was established for the purpose of providing a central location among the the Old Hickory churches, civic organizations and governmental bodies to provide emergency food, clothing and financial assistance as well as referral to services to persons in need. This assistance is available only to the Old Hickory area of Davidson County, zip code CCOC is a cooperative ministry of 12 churches in the zip code including: Berryville Baptist, First Baptist Hopewell, Old Hickory Church of Christ, Old Hickory Church of God, Old Hickory First Baptist, Old Hickory First Presbyterian, Old Hickory Nazarene, Old Hickory United Methodist, Rayon City Baptist, Ruth Ensor United Methodist, Temple Baptist and Old Hickory Community Church Impact In fiscal year 07/01/16 to 06/30/17, CCOC's accomplishments included: 1) Providing help for 912 families in the Old Hickory area - 2,446 people 2) Providing 790 food boxes, 3) Helping with 251 utility bills, 4) Holding a luncheon and fashion show from the clothes dontated to the thrift store raising $1, dollars. 5) Providing 106 Christmas boxes to those who had needs 6) Helping 176 children in 76 families have toys for Christmas In 2017 our goals include: 1) Enrich service's to Old Hickory residents, 2) Make every CCOC client feel their self-worth, 3) Improving our facility located at 209 Bridgeway Ave, Old Hickory. 4) Expanding our property to accomadate the growing needs of the CCOC service needs 5) Continuing the website to give the residents of Old Hickory information about the Center 6)Through a more efficient and effective organizational structure, allow for any unexpected occurrences, a nonprofit business plan with 3 to 5 year strategic and spiritual structure will continually increase the amount of assistance to an ever increasing need for the residents of the zip code. 7.) Additional organizational structure of the staff consisted of a program director to meet the needs of the clients with more productive services. Needs Christian Community Outreach's most pressing needs are: 1) Food donations 2) Volunteers for Thrift Store and to help with food boxes Monday-Friday 10:00 am - 1:00 pm and Thursday nights from 5:00 PM - 8:00 PM 3) Internal facility improvements to accommodate the clients with an enriching atmosphere. Other ways to donate, support, or volunteer You may make checks out to CCOC or give cash to CCOC. Board Chair Statement This is an organization that is unique. Churches of various denominations work as one unit to provide for vulnerable households in crisis. 4 Programs Programs Emergency Food Box Description CCOC provides emergency food for families. We provide 3 boxes every 6 months. In FY 2017, CCOC provided 790 food boxes serving 2446 people. Later, we learn that they have found new jobs and are successfully providing for themselves once again. Emergency Finanical Assistance Description CCOC will provide assistance with past due utilities twice a year to families in need. In FY 2017, CCOC helped with 251 untility bills. Budget 57, It costs over $ per household to help with assistance. Many time these late bills have accumalated to the point that the utilities will be shut off. This then creates more cost to these folks that are having trouble with paying thier utility bills. The Thrift store will provide free clothing 2 times a year (4 outfits or 8 pieces per person each time). At other times, donated clothes are available to be purchased at a nominal cost. CCOC is not a Second Harvest Food Bank distribution center. Due to the increased cost of food and the poor ecomonic situation, CCOC has had to buy more food products for our services. United Way has also told us since we give butter, milk and eggs in a food box that is is considered a healthy food box. 4

8 Title Experience/Biography CEO Comments The CCOC board is composed of up to three members from each supporting congregation. They may serve as long as the congregation appoints them to the board. Our volunteer pool is dedicated to serve on the days appointed. Volunteers from Old Hickory Credit Union work on special projects including the food drive, loading and organizing food. The local schools have food drives for the center. Each member church and civic organizations support us with donations each month. 8

The board has approved buying some property adjoining the current property on a tax sale. The goal for the future is to build a new building to seperate the thrift store from the operation for food and utility client needs. The current need is more parking space.
