2 Mission & Impact Statements Mission Our mission is to empower individuals with intellectual disabilities to be contributing and participating members of the community. Background New Horizons Corporation was established in 1971 by Wanda Burkett, founder and former Executive Director, and a group of concerned parents in the Nashville area. From those humble beginnings in the fellowship hall of a local church, New Horizons has grown to now serve over 200 adults with a developmental disability, and has placed well over 500 people in productive jobs in the community. For many years now New Horizons has been a leader in advocacy for the developmentally disabled and continues to advocate for individuals with developmental disabilities. Impact New Horizons Corporation endeavors to make a positive difference in the lives of people through offering quality supports and services that are individual and family centered, stable and flexible, satisfying to the person served and his or her family and able to meet individual needs. The agency supports each person we support, with his or her own uniqueness, to experience a good quality of life in a safe, healthy environment. Needs The change in day service focus from the supported workshop model to community based day services and competitive employment for the people we serve has created new challenges for New Horizons. In order to meet those challenges, we have identified these needs: Excellent recommendations for new board members in these areas: the arts, disability healthcare, music, and marketing Local employers who will work with our employment specialist to offer jobs in the community that our enthusiastic disabled adults would love to fill. Partners in our community to help us utilize the assets we have in new ways to accomplish our mission. This could include alternate uses of our current workshop (possibly as a training center for job skills), new business or funding opportunities that would cover our current revenue shortfall, or imaginative ideas we have not yet thought of yet. 4 Programs Programs Residential Services Description Residential: Supported Living model provides 24 hour staff in a home with 1-2 roommates. Family-based: the individual lives with a family in their home. This is similar to adult foster care. Budget Category Population Served Human Services, General/Other Residential Care for Individuals with Disabilities Adults, US, Production Center Description The production center provides pre-vocational skills training to prepare the disabled people we support for work in the community. Examples include the Nashville Zoo, movies, bowling, activities at community centers, mini-golf, and many others. Budget Population Served,, Employment Description Our employment specialist works with each person to discover their employment goals and experience and helps them through the Vocational Rehabilitation process, ultimately resulting in a job placement. Follow up and job coaching help the person retain the position and excel. 12 Tennessee Department of Intellectual & Developmental Disabilities (DIDD), private pay, community support from grant development and individual donors. The needs of the agency to continue to provide high quality training and supports for consumers in the work place and residential housing continue to be a fiscal challenge. The need to provide service supports to individuals in employment and community settings rather than a workshop setting as well as the rising wage rates in the Nashville labor market make it a challenge to hire and retain high quality staff are a challenge in the day services and residential programs.
