2 Job Title: Reports to: Salary/ Grade: Location: Key Working Relationships: Emergency Nurse Practitioner (ENP) / Advanced Nurse Practitioner (ANP) / Emergency Care Practitioner (ECP) Urgent Care Centre (UCC) Service Manager (administratively) UCC Lead Nurse (clinically) Band 7 Beckenham Beacon Urgent Care Centre, Croydon Road, Beckenham, BR3 3QL Lead GP, Service Manager, Clinical Director Job Summary: The post holder will autonomously undertake assessment and treatment of patients in the UCC who may present with complex and/or chronic illnesses and/or injury; to determine a clinical diagnosis, treatment and appropriate discharge as indicated as well as maintaining patient records. The post holder will be a source of clinical expertise to less experienced staff, enhancing the nurses skills and knowledge, and work in partnership within a multi-disciplinary team. To contribute to meeting the identified Key Performance Indicators within the time frames identified. To ensure a high-quality patient centered service. Key Responsibilities: Clinical responsibilities To independently undertake highly specialised clinical assessments, diagnose, treat and discharge patients with a wide range of minor illnesses and injuries, including management of complex cases To ensure patients are fully involved in the design and implementation of their treatment plans To ensure effective coordination and liaison with multidisciplinary teams across Primary and Secondary Care Services, statutory and voluntary disciplines/groups in order to provide a seamless service to the patients and their carers and prevent hospital admission where urgent care is required To refer appropriately to in-house UCC GP s where a second opinion is required To adapt styles of communication appropriately to promote empowerment, selfmanagement, and health education for patients To attend clinical case conferences, review, and clinical meetings as required To be a source of clinical expertise to less experienced staff within the team To delegate duties/tasks to other members of staff where appropriate Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relatives, and carers To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure Service delivery responsibilities: To ensure that patient records and documentation are maintained in line with NMC standards, Greenbrook Healthcare policies and use of the electronic patient records system. This includes: o Documenting full and accurate records of assessments, investigations, procedures, treatments, and aftercare. o Encouraging colleagues to maintain good and complete records. o Working closely with the reception team and the administrator, ensuring all records pertinent to patients are scanned and filed as appropriate. o Contributing to the development of computer-based patient records 3

4 o Contributing to the Read-Coding of patient data Work within local protocols, guidelines, PGDs (Patient Group Directives). To participate in audits and use the results, current research, and government guidelines to make recommendations for changes to service delivery and clinical practice, and to share knowledge with other healthcare professionals. To contribute to the maintenance and improvement of communication networks with all disciplines including community healthcare teams, patients, acute hospitals and relatives/carers in order to ensure continuity of care. To collect data, and provide service information to the Service Manager as required. To contribute to the development and establishment of systems to gather feedback from patients. To ensure that Service Manager, Lead Nurse /GP are kept informed on service issues and potential risks. To work with the Service Manager and UCC Lead Nurse /GP in delivering and promoting the needs of the service To work with the UCC Lead Nurse /GP and Service Manager to meet the Clinical Governance needs of the UCC; including investigation of incidents (including serious incidents) and complaints, where required, sharing learning that arises from the investigations within the service with the UCC team. To provide clinical supervision /mentorship (where appropriately trained) to junior practitioners. To remain updated in the use all equipment used in the UCC and to report any technical problems or issues with that equipment. To participate in nurse / team meetings to discuss day to day operational issues. Personal & professional development To be a skilled practitioner in routine and specialist skills. o Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)/Health & Care Professions Council (HCPC). o To keep abreast of developments in nursing practice and topical issues ensuring that skills are maintained and enhanced through continued professional development. To collaborate in the continuing development of the ENP/ANP/ECP role to include:- o Level of autonomy in practice. o Progression from a role focusing on minor injuries to include minor illness where not previously trained. o Appropriate education and preparation for the role to develop. o Show self-motivation toward expanding practice and developing advanced skills and knowledge. 5 o Be an effective role model and expert resource for nursing staff and other members of the multi- disciplinary team. o To maintain clinical excellence within the department and provide education and training both formerly and informally for colleagues. To initiate and /or participate in research if required. Policies & Guidelines Awareness of and compliance with all relevant company policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, to ensure that the UCC complies with NHS/NHS England/CCG contractual obligations in relation to patient care and meets contractual key performance requirements. To participate and contribute to operational policy making, attending appropriate meetings, and representing the UCC as required. Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate Health & Safety To act responsibly in respect of own and colleagues health, safety, welfare and security, following safe work practices and complying with Health and Safety Policies. Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken. Where appropriate, oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements Apply infection control measures within the UCC according to local and national guidelines Using personal security systems within the workplace according to UCC and organisational guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Reporting potential risks identified Equality and Diversity To support the equality, diversity and rights of patients, carers and colleagues, to include: o Acting in a way that recognises the importance of people s rights, and interpreting them in a way that is consistent with practice procedures and policies, and current legislation o Respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues o Behaving in a manner which is welcoming, non-judgmental and respects the circumstances, feelings, priorities and rights of every individual. 5

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team s performance. Work effectively with individuals in other agencies to meet patients needs. Effectively manage own time, workload and resources. Accepting individual responsibility for meeting required standards and for following quality / safety processes and procedures (as defined in national or local Codes of Practice and local service standards) and be conversant and comply with current emergency resuscitation techniques, infection control and fire regulations and procedures both in theory and practice. SUPPLEMENTARY INFORMATION Health Clearance Health clearance is required for this appointment. Disclosure and Barring Service (DBS) Check A DBS check (previously Criminal Records Bureau check) will be required before appointment for all posts with access to children or vulnerable adults. In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, staff and other healthcare workers. They may also have access to information relating to the company as a business organisation. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the service may only be divulged to authorised persons in accordance with the Data Protection Act, and policies and procedures relating to confidentiality and the protection of personal and sensitive data. The post holder must always keep all patient-related information confidential; this includes if the patient is deceased and if the post-holder is no longer employed by Greenbrook Healthcare. Data Quality/Security The post holder is responsible for ensuring that he/she maintains the integrity and quality of both computerised and manual data. Clinical Governance / Raising Concerns 7

The post holder has a personal responsibility to keep up to date and, if s/he encounters unacceptable practice, to raise this in a responsible and appropriate manner, if necessary through the whistleblowing procedure. Codes of conduct and professional standards All staff should adhere to any codes of conduct or professional standards set by the regulatory bodies with whom they are registered or by professional bodies of which they are a member. Uniform and Dress Code All nursing and Emergency Care Practitioners should adhere to infection control principles and must wear a uniform when on duty. Smoking Smoking is not allowed on any part of Greenbrook Healthcare premises. Policies & Procedures The post holder is expected to comply with Greenbrook Healthcare s policies and procedures. Personal Property Greenbrook Healthcare is unable to accept responsibility for articles of personal property lost or damaged on its premises whether by burglary, fire, theft or otherwise and staff are advised to insure against all risks. 8

Up to date knowledge of DOH and NICE guidelines. Knowledge of use of IT/library searches knowledge Diagnosis and management of minor injuries including fractures, burns, lacerations, minor eye injuries, soft tissue injuries Ability to work with and use PGDs Diagnosis and management of minor illnesses (where appropriately trained), giving evidenced based advice and management, and where required appropriate prescribing (including using Application form / Interview Test 9
