1 HMP Woodhill, Milton Keynes 1. Main purpose of the role (Salary as advertised) 37.5 hours per week Permanent Westminster Drug Project s (WDP) HMP WOODHILL is an integrated substance misuse service operating within HMP Woodhill. WDP WOODHILL provides an integrated drug treatment service, delivering clinical and psychosocial interventions to address the needs of service users, supporting and enabling them to achieve individualised recovery based goals and aspirations. Our service works as part of a larger multi-disciplinary team and relies on close partnership working to provide our clients with a holistic care approach. WDP develop individualised recovery plans, providing a safe therapeutic environment for individuals to address their drug and alcohol problems from the point of custody, through sentence and to maximise their potential for remaining abstinent on release into the community. Within the service, we manage a clinical caseload with a complexity of needs. We provide 24/7 clinical care, with a team of specialist practitioners, comprising of specialist substance misuse doctors, non-medical prescribers, substance misuse nurses (both RMN and RGN), pharmacy technicians and psychosocial recovery practitioners. The post holder will be a Registered Nurse RMN or RGN who will report to the Clinical Nurse Manager. They will have working knowledge and experience of drug and alcohol detoxification and treatment programs with an understanding of both clinical and psychosocial aspects of care. The main duties of the role will be: Supporting caseload management of the wider team. This will involve assessing, planning and implementing clinical care whilst managing clients general health needs Responding to clinical and medical emergencies, managing crisis as first line intervention and assessing risk and safeguarding The safe and efficient administration of controlled drugs and general healthcare medications to clients in compliance with medicines management guidelines. Role-specific responsibilities Clinical 3.1 To understand and implement WDP s policies and procedures. 3.2 To work as a member of the multidisciplinary team that provides a range of specialist services to problem drinkers and problematic drug takers attending the service, Page 2 of 11

3 including harm minimisation approaches, motivational interviewing, relapse prevention, individual intervention, health education and detoxification programmes and group work. To make continuous assessments, under supervision, of the psychological, emotional, social and rehabilitative needs of prisoners, taking into account the needs of family, local community, and environment. To develop systematic plans for care and evaluation of treatment plans. To be able to assess and case manage clients who inject drugs, in particular assessing injection sites and be conversant with the precepts of harm minimisation, taking a predominant role in providing duty assessments to clients who are new to the service. To develop and adhere to working relationships and or protocols with drug treatment services, probation, prisons, courts and community services. To provide case management and direct clinical care to a defined number of clients as agreed with the team manager and lead nurse and under the supervision of a designated professional within the team. This includes assessment, planning and providing specialist care. It is expected that the caseload will consist of clients with needs of a low to medium complexity. 3.7 To refer clients to the relevant professionals or treatment providers To work with other professionals or treatment providers with assessing / reviewing of client s needs. To use agreed and established DATA and Outcome Monitoring Tools and any other relevant quality measures Supervise volunteers and student placements to deliver WDP s service objectives To manage the transfer of individuals between agencies and services To ensure that all work is performed in accordance with organisational policies and procedures, including the incident, accident, and complaints reporting. To ensure the highest quality of service is delivered by following local and national quality and performance standards, guidance, protocols, procedures and practice guidelines. To ensure full risk assessments and risk management is delivered effectively, under supervision. To open, maintain, close and actively engage and support the Assessment, Care in Custody and Teamwork (ACCT) process of risk management within the establishment, ands take part in reviews as appropriate. To support and encourage the screening and vaccinating of clients on own caseload against BBVs. To respond to requests to provide clinical advice and support to other colleagues within the healthcare setting and to other agencies concerned with substance misuse. Page 3 of 11

4 To support individuals about their substance use, using motivational interviewing, relapse management and other evidence-based interventions. To respond to clinical and medical emergencies and managing crisis as first line intervention and be aware of potentially unpredictable situations that may arise with this client group. Working in partnership with Hotel 1 (NHS emergency healthcare response) to manage medical emergencies To deliver psycho social interventions supporting Service Users in there recovery Quality Control Oversee and maintain clinical governance procedures in line with the CQC, WDP and NMC requirements. nsure that all information in relation to safeguarding is maintained and reported in line with local policy and procedure. nsure that all service delivery is in line with the service specification and service contracts, and meet agreed targets. To develop and implement measurable quality assurance targets for the service, including outcome monitoring and service evaluation. To develop and maintain quality assurance processes that measure and report performance against these targets, including procedures for service user consultation, systems for routine data collection and reporting, and the keeping of accurate records with due regard to confidentiality. nsure that all complaints are dealt with promptly and reported in line with CQC and WDP requirements. You must adhere to and uphold the Nursing and Midwifery Council (NMC) Code of Standards of conduct, performance, and ethics for nurses and midwives. To ensure services and duties are delivered in compliance with the law and relevant national and local policies, standards and guidance, including the CQC, NTA, NIC and other quality standards. Be familiar with and comply with, WDP policies for infection control and hand hygiene in order to reduce the spread of healthcare-associated infections. This will include a requirement to attend mandatory training in Infection Control and be compliant with all measures known to be effective in reducing healthcare-associated infections, including correct uniform and dress code policy, the use of personal protective equipment policy, safe procedures for using aseptic techniques, and safe disposal of sharps an clinical waste. Being responsible for the health and safety of colleagues and service users through upholding organisational infection prevention and control policies in compliance with CQC essential standards. To manage daily medical needs of clients within WDP WOODHILL including facilitating appropriate referral and appointment bookings for partnership agencies and WDP Page 4 of 11

5 WOODHILL clinical and psychosocial services To facilitate staff supervision as required. Service Communications 3.33 To participate in multidisciplinary meetings, both on site and in the community relating to client management To develop close links with partner agencies and services To liaise and communicate clinical information with partner agencies within the strict guidelines / protocol on shared confidential information between WDP and named partner agencies. To promote an open dialogue culture where a positive culture on improving communication, and maintenance of a healthy therapeutic relationship with clients are enhanced and valued. To contribute to the promotion of team cohesiveness, multidisciplinary working and at all times work collaboratively with colleagues in all parts of the organisation to achieve healthy and effective communication. To work collaboratively in developing professional links with other statutory and voluntary service providers, ensuring a corporate approach is adopted. To provide specialist advice, when required, on drug/alcohol misuse to other colleagues in relation to the assessment, treatment planning and implementation of interventions to substance misusing clients. To participate in a range of partnership arrangements both internal and external to the service/unit while remaining within the boundaries of good practice. To liaise closely with operational staff and involve them in decision making process of clients move on and off WDP WOODHILL allocated house unit. To ensure that recording of clinical case notes is of a high standard and in accordance with NMC record-keeping guidelines, WDP information governance policy and data protection act and is undertaken within the required periods. Raise drug and alcohol and recovery awareness within the community, sharing your knowledge and experience by developing packages that train and educate establishment, community and other interested groups. Performance 3.44 To be responsible for the effective management of own caseload To maintain accurate and up-to-date documentation of all service clients records, ensuring the confidentiality of such records of information are in line with the Data Protection Act and Information Sharing Protocol. To ensure that client information is entered in the service database system and to ensure activity input to NDTMS data forms is accurate and timely. Page 5 of 11
