1 JOB DESCRIPTION Title of Post: Lead Clinician for Adult Community Speech and Language Therapy Service Band of Post: Band 7 Directorate: Reports to: Accountable to: Initial Base Location: Type of Contract: Hours: Primary Care, Elderly & Nursing Assistant Manager for Adult SLT Services Lead Professional Speech & Language Therapist for South Eastern Trust Base to be confirmed however will work across SET area Permanent Full-time 37.5 hours Job Purpose The Community Adult Speech and Language Therapy Service in SET aims to ensure that all adult clients with an identified Speech and Language Therapy need can access a service to assess and support their communication and/or swallowing needs. This service also aims to ensure adequate public health awareness to educate the wider population in both communication and swallowing problems. This includes direct interface with family and carers. The post holder will have a lead responsibility for the planning, design, delivery, development and evaluation of a specialist Speech and Language Therapy service within the Community Adult Speech and Language Therapy Service in SET. They will work as part of the wider adult Speech and Language Therapy Service team and in close liaison with Multidisciplinary Teams, statutory and voluntary agencies as well as the SLT service leads/expert practitioners, to promote and develop a collaborative approach to the management of adult clients with communication and/or swallowing needs. Main Responsibilities To have lead responsibility for assessment, diagnosis, prioritisation, treatment and management of patients referred to the Community Adult Speech and Language Therapy Service. This will include highly specialised assessment and intervention techniques for management of communication and swallowing difficulties in this client group, including complex cases. These services will be delivered on a domiciliary and/or outpatient basis depending on the needs of the client. Page 1 of 14
2 Refer clients to the Clinical and Service Lead Speech and Language Therapists as appropriate for further specialist assessment and advice in liaison with Line Manager and in line with Trust care pathways. Assistant in/undertake objective swallowing assessments dependent upon level of experience To have lead responsibility for the planning and management of the caseload in conjunction with the Assistant Manager for Adult SLT services and other senior staff, as appropriate To ensure close working with colleagues within the wider adult Speech and Language Therapy Service, including colleagues in specialised MDT teams, in the planning and implementation of joint programmes where appropriate. In conjunction with the line manager, will have responsibility for the planning, design, implementation and evaluation of specialist swallowing advice and training for clients, carers, Trust staff and private providers Monitors and evaluates the outcomes of the clinical practice and co-operates with the Clinical and Service Lead Speech and Language Therapists and Assistant Manager for Adult SLT services and other senior staff in the development of efficient and effective models of clinical intervention and service delivery. Lead on the development of relevant advanced professional knowledge and clinical expertise. Support, guide and supervise junior Speech and Language Therapists and Speech and Language Therapy Assistants across the SEHSCT Adult SLT Service Offer on the job training and delegation of training programmes to support staff. Demonstrate through clinical practice added knowledge within the specialised area of work Provide a second opinion to junior Speech and Language Therapists. To assess for the use of AAC technologies to support and supplement communication and to provide training to clients, carers and other relevant individuals in their use. To evaluate service delivery suggesting/proposing SLT policies and guidelines within the clinical area, and in conjunction with the line manager develops these SET SLT policies and guidelines based on Trust, local and regional policies and strategies Page 2 of 14
3 Clinical/Professional/Specific Managerial Responsibilities The post holder will be expected To be responsible for the administrative work involved in the operation of the post including the establishment and maintenance of appropriate data bases and records. To collate and submit clinical statistics as required by the Trust. To compile reports as appropriate for other health and care staff. To participate in standard setting for clinical audit within this clinical field. To adhere to the principles of the Data Protection Act and Trust policies and procedures. To provide professional advice to the Clinical and Service Lead Speech and Language Therapists and Assistant Manager for Adult SLT services on issues relating to clinical and social care governance. To contribute to the Speech and Language Therapy Department business planning process. To adhere to the clinical responsibilities under the clinical and social care governance guidelines within the Speech and Language Therapy Department. To delegate duties when appropriate. To sit on interview panels when required. To be aware of the necessity to promote the Speech and Language Therapy Service and all services within the region. To adhere to professional standards and guidelines. To be responsible for the safe and responsible use of equipment used both personally and by service users. To be responsible for facilitating those clients with impaired communication in decision making relating to the client s personal health and social care needs and personal choices. Learning and Development Responsibilities: The post holder will be expected To provide clinical placements for undergraduate Speech and Language Therapy Students and other students as deemed appropriate. 4 To assess, monitor and ensure the provision of training to clients, carers, other staff/colleagues in relation to the management of speech, language and communication needs. To maintain an up to date knowledge base through reading of relevant articles and journals to ensure clinical competency and be responsible for continued personal and professional development. To regularly complete audits and service evaluations to help deliver a high quality service delivery with positive outcomes for service users. To liaise with clients/carers and other relevant professionals to monitor service user views in relation to all aspects of service delivery in meeting the Personal and Public Involvement agenda. To contribute to in-service training throughout the Trust and to contribute to lectures, seminars and conferences as requested. To ensure all Speech and Language Therapy Assistants receive appropriate supervision and training as appropriate. Commitment to maintaining highly specialised skills/knowledge. To participate in annual staff development reviews and to have responsibility for conducting staff development reviews with junior SLTs and SLTAs. To advise other professional groups on research as requested. To be committed to the post through high standards of clinical practice, data collection, systematic evaluation of therapeutic approaches and outcomes and the development of evidence based practice. To communicate and collaborate with other professional/groups on project findings both verbally and in writing. To initiate and undertake research and development projects as they pertain to the Adult Speech and Language Therapy Service. To advise the Clinical and Service Lead Speech and Language Therapists and Assistant Manager for Adult SLT services of training needs of junior staff and assistants, purchase of appropriate equipment etc. To ensure robust interface arrangements are in place across statutory/voluntary/community sectors. Liaison Responsibilities: The post holder will be expected To liaise closely with clients/carers of clients and to provide specialist advice and support for clients/carers on communication and/or swallowing needs. 5 To attend and participate in staff meetings. To initiate and participate in case conferences/best interest meetings as required. To meet regularly with the Clinical and Service Lead Speech and Language Therapists and/or Assistant Manager for Adult SLT services regarding supervision, annual appraisal etc. Compliance to Trust Policies and Procedures The post holder will be expected To be aware of current legislation and policies in relation to post. To participate in clinical audit and peer review. To work in accordance with the SEHSCT and SLT policies, procedures and guidelines including the Safeguarding Policy. To carry out any other designated duties which may be required relating to the particular needs of the service. The post-holder may be required to work weekends, evenings and statutory days. In response to continued service development the duties of the post may be conducted over a 7 day period. This job description may be subject to review in the light of changing circumstances. 8 Records Management All employees of the Trust are legally responsible for all records held, created or used as part of their business within the Trust including patient/client, corporate and administrative records whether paper based or electronic and also including s. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environmental Information Regulations 2004 and the Data Protection Act Employees are required to be conversant with the Trust s policy and procedures on records management and to seek advice if in doubt. Environmental Cleaning Strategy The Trust s Environmental Cleaning Strategy, recognises the key principle that Cleanliness Matters. It is everyone s responsibility, not just the cleaners. Whilst there are staff employed by the Trust who are responsible for cleaning services, all staff employed by the Trust have a responsibility to ensure a clean, comfortable and safe environment for patients, clients, residents, visitors, staff and members of the general public. Infection Prevention & Control All Staff should co-operate fully with regard to Trust policies and procedures relating to infection prevention and control. All staff should be aware of the Trust s Infection Prevention & Control Strategy and their local Directorate initiatives being undertaken to reduce infection and enhance care delivery for patients/clients. They should know their role, in keeping with key principles as Infection Prevention and Control is everyone s business. Staff, in delivery of all care must:- -Wash their hands thoroughly between each patient contact. - Be compliant with Standard Infection Control Precautions, Hand Hygiene and decontamination and other relevant infection prevention and control measures. - Be aware of the Trust s Infection Control guidance and the Northern Ireland Regional Infection Control Manual and ensure they obtain mandatory Infection prevention control training or other specific infection control related training as required. Hygiene & Prevention of Infection You will at all times be required to observe hygiene/infection prevention & control standards and notices in operation throughout the Trust. Hygiene and control of -infection are key elements in delivery of safe, quality services and you will, in certain jobs, be required to undertake an appropriate training course at the expense of the Trust. 9 Personal Public involvement Staff members are expected to involve patients, clients and their families in developing, planning and delivering our services in a meaningful and effective way, as part of the Trust s Personal Public Involvement (PPI) Strategy. Location Please note that it is a standard condition that all Trust staff may be required to work at any location within the Trust s area, as the needs of the service demand. Professional qualifications required by Royal College of Speech and Language Therapists. 2. Current license to practise. 3. Professional indemnity insurance. 4. Member of HCPC and RCSLT 5. Four years clinical experience, at least two years with client group. 6. Show evidence of advanced training and experience of communication and associated disorders with the client group at postgraduate level, e.g. MSc or equivalent. 7. Qualification or extensive training/experience enabling SLT to work independently with dysphagic adult patients. 8. Demonstrate specific practical knowledge of the highly specialist area relating to the post;- e.g. Knowledge of a wide range of neurological and physiological conditions and the concomitant communication and/or swallowing disorders. Knowledge of and experience in the use of AAC and IT to support and supplement clients communication Page 10 of 14
11 Knowledge of the ethical issues surrounding feeding in certain circumstances and the practical implications in terms of risk feeding. Knowledge of legislative changes and the implications for clients, in particular Mental Capacity legislation and the consequences of this in relation to decision making and capacity assessment. Knowledge of and experience in instrumental/objective swallowing assessments Knowledge of the social care structure and the roles undertaken by both statutory and non-statutory care agencies 9. Hold a current full driving licence valid in UK with access to suitable transport on appointment. This criterion will be waived in the case of an applicant whose disability prohibits driving but who is able to organise suitable alternative arrangements. Qualifications Diploma/Degree recognised by R.C.S.L.T. Registration as a practising member of R.C.S.L.T. Registration with H.C. P.C. Qualification and/or extensive experience allowing independent practice in dysphagia Relevant Experience Four years experience as a qualified Speech and Language Therapist, with at least two years experience with the client group. Advanced training and experience of communication and associated disorders and swallowing disorders within the client group at postgraduate level e.g. MSc or equivalent. Experience in providing training to clients, carers and other professional groups. Skills and Knowledge Knowledge of Health and Social Services reforms. Knowledge of minimum quality standards of speech and language therapy practice as contained CQLive. A knowledge of how to use outcome measures to evaluate intervention. Page 11 of 14
