This is achieved through leadership and clinical excellence for our staff and involvement, self-care and personalisation for our patients Overview of the Post To provide a comprehensive community service to clients with a neurological condition who have multi-disciplinary needs and have a GP within Sutton CCG. The Community Neuro Services include the Community Neuro Therapy Team (CNTT) and Stroke Early Supported Discharge (ESD) Team, providing neurological rehabilitation & intervention to patients with a GP in Sutton CCG. 2 The CNTT provides a specialised multi-disciplinary service to patients with neurological conditions in the community in order to safeguard independence by preventing inappropriate hospital stays, maximising rehabilitation and recovery after illness or injury, and minimising premature dependence on long term institutional care. Therapists in the Community Neuro team assess, differentially diagnose, rehabilitate and manage a caseload of adults with neurological conditions in the community setting. The CNTT physiotherapy service provides both a domiciliary & outpatient service. Physiotherapists in the CNTT team may work within either of these services or across the services. The ESD team provides a flexible and proactive therapy service to facilitate early discharge of patients from acute, inpatient settings for clients with a new stroke diagnosis who have on-going therapy needs The post is within the Community neuro service & therefore the post holder may either work within the ESD or CNTT team, with potential to rotate within the service. The neuro service is also looking at the prospect of developing rotational or integrated roles opportunities across the acute & community settings. Job Purpose To work as part of a multi-disciplinary team of therapists and rehabilitation assistants to provide specialist neurological intervention to enable and rehabilitate patients registered with GP s within Sutton CCG. To provide rehabilitation to patients with a range of neurological diagnoses who meet the Community Neuro Therapy Team or Early Supported Discharge Team criteria. To have clinical responsibility for assessment and treatment planning of a caseload of clients within the community, including those based in residential and nursing home facilities. To assist in the overall development and management of physiotherapy and community neuro services. Key Result Areas/Principal Responsibilities To develop a high level of clinical skill in community rehabilitation and longer term management of disability and impairment. To provide comprehensive physiotherapy intervention to clients in the community, making use of community facilities where appropriate. To be responsible for a clinical caseload working within a multi-disciplinary team ensuring clients receive optimum level of therapy within available resources. To carry out assessments, plan and implement agreed goal orientated physiotherapy intervention with client/carers in an appropriate community setting. To carry out multi-disciplinary screening assessment and discuss outcomes with team members. To keep accurate up-to-date records on patient care and equipment provision, which are in accordance with quality standards of the Trust/Service policies and The Chartered Society of Physiotherapy. To provide written and/or verbal reports to referrers and other agencies involved in clients care and participate in clinical meetings as necessary. Page 2 of 8

3 To assist in the development of the Physiotherapy Service to the specialist clinical area given that effectively contribute to the patient /care pathway, and to develop the skills required to undertake this. To develop skills in monitoring the effectiveness of intervention through goal setting, outcome measurement and evaluation of practice. To contribute to the development of information that enables clients to understand and self manage their condition whenever possible. To assist the Band 7 Lead Physiotherapist and Service Manager by monitoring caseloads, recording statistics and prioritising referrals as required. Core Development Duties & Responsibilities To participate in continuing professional development as identified through supervision and annual appraisal, including attending in-service training and courses as agreed. To participate in own supervision on a regular basis and work towards objectives set in appraisal. To keep abreast of new development within the clinical field as relevant to physiotherapy and community team working. To comply with Sutton Community Healthcare Services and physiotherapy service mandatory training requirements. To participate in the training of others, including education and support programmes for relatives/carers and giving presentations to other disciplines within the team. Research and Development To assist in the development of the physiotherapy service to ensure effective evidence based treatment to the patient and to develop the skills required to undertake this. To be involved in clinical innovation, using research evidence based practice and clinical networks. Page 3 of 8

4 To actively review efficacy of service delivery and facilitate setting up of new standards, or amend existing ones, in close liaison with the Band 7/ Service Manager. To be involved in monitoring and evaluation of physiotherapy intervention as part of the multi-therapy service To directly supervise physiotherapy students and rehabilitation assistants as appropriate To participate in clinical governance / audit / clinical effectiveness projects in this clinical area. Professional Responsibilities To participate in Individual Performance Reviews in accordance with trust and department standards. To keep up-to-date with the department and Royal Marsden standards and policies and facilitate dissemination of this knowledge to colleagues. To regularly participate in in-service programmes. To abide by the CSP rules of professional conduct and local professional and quality standards. To develop and maintain personal, clinical and managerial skills through supervision, CPD, peer support and a personal development plan agreed with the line manager/professional manager. To maintain a current knowledge of developments within the NHS and physiotherapy. To deputise for the Band 7 in their absence. To assist the Service Manager and other senior staff with the day-to-day running of the service, by carrying out duties as deemed necessary by the Service Manager. To work in accordance with the Standards and Policies issued by the Chartered Society of Physiotherapy, The Trust and Community Neuro Services. To attend any mandatory training and induction courses as designated by management. Present a positive image of The Royal Marsden Community Services. In instances where it is known that a member of staff has communicated information to unauthorized persons, those staff will be liable to dismissal. Safeguarding Children and Vulnerable Adults All staff must be familiar with and adhere to the Trust s child protection and safeguarding vulnerable adult policies and procedures. Emergency Planning In accordance with the Trust's responsibilities under the Civil Contingencies Act 2004 all staff are required to undertake work and alternative duties as reasonably directed at variable locations in the event of and for the duration of a significant internal incident, major incident or pandemic.
