We are looking for a nurse with comprehensive practical post registration experience in the chronic management of haemodialysis patients to work alongside the existing team within our small satellite unit. The post holder will have the confidence and ability to develop their knowledge and skills to work independently within a small team. We have a four stationed haemodialysis unit where you will be part of the nursing team delivering a personalised service to patients undergoing renal replacement therapies. This post has a high clinical input. This exciting and challenging role will afford you the opportunity to work as part of a supportive team that is focused on delivering high standard patient care. You will be highly motivated and articulate with good interpersonal and communication skills and the ability to relate to staff and the general public at all levels. Ideally you will have experience in all areas of nephrology nursing, including pre-dialysis management and peritoneal dialysis. Job Identification Job Title: Senior Staff Nurse Renal Services Responsible to: Assistant Director of Nursing (Hospitals) Department: Renal Directorate: Nursing Last Update: 6 th May Job Purpose To provide a Renal Dialysis service to the patients of Shetland. This will involve caring for patients receiving peritoneal dialysis as well as home and hospital haemodialysis. To support the development of renal services as a satellite site of NHS Grampian 3. Role of Department The primary function of the renal service is to provide acute and chronic haemodialysis services to patients in NHS Shetland along with providing and enabling systems of working to enable NHS Shetland to achieve its strategic objectives and the provision of high quality standards and delivery of care. The renal unit currently has four haemodialysis stations and the usual days of operation are Monday, Wednesday and Friday. The unit also trains and supports patients and their families in home dialysis therapies. There may be occasional requirement to work at the weekend. The renal nurses are involved in the pre-dialysis stage of the patient pathway. The nurses see patients and their families in clinic to discuss treatment choices and to provide support and education. The unit actively encourages patient involvement in their care through regular discussion and feedback of test results and progress.

Key Result Areas The Senior Staff Nurse demonstrates an understanding of and works within NHS Shetland local policies/procedures and supports managers on dissemination of policies and procedures. Manages their own workload often with conflicting and changeable deadlines on which there is little or no control. Sorts, distributes and prioritises incoming and post using judgement, experience and a sound understanding of organisational objectives, This may include responding directly to some issues. Identifies personal opportunities for learning/development and recognises and assists with the development of other members of the team to meet service and individual needs Participates and includes all members of the team when carrying out appraisal recognises and evaluates own knowledge and practice as a Band 6 Senior Staff Nurse Maintains best practice utilising and ensuring all members of the team are aware of and use current evidence based practice. Participates willingly and suggests areas for learning and development within the environment. Recognises and encourages learning and development opportunities within the workplace e.g. study sessions and clinical practice learning. Facilitates and monitors staff development which encourages safe and effective working within local and national policies and guidelines. The role of the Senior Staff Nurse is to ensure all Health and Safety policies are adhered to ensuring best practice in Health safety and Security. Promotes the patient safety goals specific to the Shetland Renal Service. Ensure awareness of health and safety issues to identify actual and potential risks and take appropriate action. Demonstrates and provides evidence within risk management e.g. manual handling assessment. Undertake work activities consistent with legislation, policies and procedures. Assists and ensures other members of the MDT follow correct policies and procedures. Follow risk assessment procedures to identify and report any health safety and security problems ensuring appropriate action is taken. Support and facilitate others in maintaining health, safety and security. To ensure and assist in service improvement within the clinical area. In collaboration with team members promote positive change to current practice implementing policies and procedures.

5 Support and actively encourage changes to own working environment. Act as a role model actively encouraging all members of the team to promote and accept change. Evaluates own and others work and supports all members of the team in identifying current service provision through appraisal and audit. Assists with audit and discussion including all members of the team to improve current practice. Support all staff to identify the need to modify or change current practices which no longer reflect evidence based practice. Maintain quality in own work and encourage others to do so, supporting the Renal Service in improving services. Works in a way which promotes best practice and addresses NHS Shetland s policies and procedures/legislation. Acting as a role model for the team. Recognises own limits and capabilities being responsible and accountable for own actions and that of the team. Uses holistic evidence based practice knowledge to develop referral pathways for patients with complex needs requiring other interventions and enabling person centered approaches to be integrated into patient assessment and care planning processes. Demonstrates the ability to critically appraise current evidence and its application where appropriate. Uses expert knowledge and information relevant to care provision to enhance the experience of the patient e.g. liaises with all stakeholders to ensure that all information is used to plan the care provided. Interprets all information in a knowledgeable way using skills of differential diagnosis. Uses analytical skills and clinical reasoning to understand and use data in clinical practice.

7 Co-ordinates the delivery of care. E.g. care management or case load. Identifies trends in care delivery to support profile and inform the development of services. Monitor and evaluate the quality and safety of care. Review care plans as required to enable complex health and wellbeing needs to be met e.g. case conferences, care discussion, multiagency meetings. Ensure that the patient relative/ carer is a partner within their care, documentation will reflect this partnership and will be involved in all decision making through informed consent e.g. altered body image, physiological change and function. Promote care plans to reflect the patients individuality and holistic intervention e.g. physical, social and psychological wellbeing. Care will be delivered through a multidisciplinary approach and reflect evidence based practice following NHS Shetland s policies, procedures and protocols. Support team members in the assessment of patients and will ensure that the needs of the patients are met appropriately. Encourages team awareness of local and national guidelines and be able to resource relevant members of the MDT required to deliver patient care. - his/her knowledge & skills and experience must reflect up to date research based care. Ensures that the team assess all patient associated risks, and action them in an appropriate and timely manner e.g. Waterlow, MUST, Manual Handling requirements. Takes responsibility to ensure that all aspects of care are continually evaluated and updated as necessary. Will assist in promoting effective liaison with all staff members, MDT and consultants as appropriate, discussing/ changing any aspect of the care plan considered ineffective. Ensures and supports all nursing documentation surrounding patient care and the environment is up to date, legible and adequately recorded. Ensure all staff identify personal development needs and collates information and helps to plan future learning and development needs within service. Responsible for ensuring own lifelong learning and development. Participates in the delivery and/or facilitation of educational programmes. Maintain formal assessment of the renal service environments suitability for learning. Ensures resources available to support varied learning and development opportunities ensuring a positive outcome for both the staff member and the service.

8 Formally reviews staff/patient responses to learning and development. Ensures all feedback used to enhance and progress the learning environment. Provide feedback regarding individual learning, development and progress via direct/delegated appraisal and objective setting. Supports the implementation of strategies to improve learning and development within the service. Maintain an active role in mentorship, teaching and supervision. Suggests workforce requirements to the Assistant Director of Nursing (Hospitals)/clinical nurse manager, this may include: - numbers of staff on a daily basis - rota/shift patterns - skills/skill mix of staff for day/night shifts Involved in the recruitment and selection process, selecting individuals following NHS Shetlands policies. Gives team member s achievable and realistic goals for development. Encourages all team members to participate in the service development, giving praise to recognise good ideas. Ongoing search for innovative ideas to improve the service provided by the service. Plans and coordinates work may include: - Continuous prioritising of patients and the workload in an often chaotic environment. - Managing multiple simultaneous admissions ensuring the coordination and effectiveness of staff. Delegates authority to more junior staff members as a development opportunity e.g.: encouraging junior staff "taking charge" of the service at defined times and the outcome expected by the service. Advise staff of the support that will be available to allow their development. Performance will be monitored continually with a formal PDP and review annually. Allocating and providing resources may include: - Study time - Financial support - Access to in-house development courses etc. - Peer support - Progress will be reviewed on a day to day basis and any issues addressed in a timely manner, implementing and documenting the timescale for resolution. Assignment and Review of Work This Job Description will be used as a basis for individual performance review between the post holder, and the manager. The Job Description covers only the key result areas and, as such, does not intend to provide a comprehensive list of objectives. Specific objectives will be reviewed each year, and may develop to meet the changing needs of the service. The postholder will need to take due account, in the way they achieve the key result areas, of Board policies and procedures. Ensure timely and appropriate action taken when dealing with emergency and potential emergency situations. 10. Most Challenging/Difficult Parts of the Job Working with patients varying needs. Meeting deadlines and prioritising work to ensure completeness when there may be conflicting pressures. Ensuring good communication links as a satellite site of NHS Grampian. 11. Communications and Relationships Internal: - Communicates with all disciplines of staff including managers, patients, carers, doctors and the wider multidisciplinary team as appropriate taking into account the diversity required in communication skills. Demonstrates an in depth knowledge of service delivery and skills associated with the renal speciality - Communicates effectively, demonstrating good interpersonal skills utilising verbal, written or electronic format ensuring individual needs are met. Can adapt communication styles to ensure effective delivery of information to all in varying and difficult circumstances e.g. breaking bad news and cultural differences. - Understand the existence of barriers to communication and takes appropriate steps to overcome these. - Provide opportunity for effective communication for all members of the MDT. External: - Liaison with others outside of the Board is of equal importance, including members of the public, other NHS Organisations, other Public Bodies and Voluntary Agencies and members from the Scottish Government teams (as appropriate). 12. Physical, Mental, Emotional and Environmental Demands of the Job - Moving and handling of patients and equipment - Concentration levels need to be sharp whilst dealing with a diverse workload and trying to achieve tight deadlines - Working environment mainly at Gilbert Bain Hospital and in Patients homes in Shetland as required. 13. Knowledge, Training and Experience Required To Do the Job - Registered Nurse with broad experience of haemodialysis and peritoneal dialysis - Broad experience within medical nursing - ENB 136 qualification or equivalent - Degree level qualification or working towards at time of application or able to evidence working at an equivalent level through experience and professional development - Experience at mentoring / preceptorship along with a teaching and assessment certificate - Experience of audit and monitoring of standards. - Experience in all areas of nephrology nursing, including pre-dialysis management and peritoneal dialysis - Ability to identify and manage a number of conflicting priorities and make decisions in order to manage this workload, taking corrective action where appropriate. - The ability to work flexibly and enthusiastically whilst remaining professional under pressure.

12 - The ability to work as part of a multi-disciplinary team but equally to work on own initiative to deliver to time the requirements of this post. - Excellent interpersonal skills with the ability to relate to staff, and the general public at all levels. - Ability to coordinate and communicate effectively with all members of the team. - High standard of presentation and accuracy. - Full driving licence (as you may be required to support home dialysis patients). 14.
