2 DATE: 21 st September 2015 An excellent opportunity has arisen for a Health Care Assistant to join a friendly and busy GP Practice. Previous experience in phlebotomy is preferred, however training would be provided to a suitable candidate. The role would require the candidate to be flexible, and be willing to work mornings and afternoons plus holiday and sickness cover. 3 JOB DETAILS TITLE: HOURS OF WORK : Health Care Assistant To be arranged SALARY: 15,392 to 16,354 Band per hour Band per hour Band per hour REPORTING TO: Team Leader Clinical Services DATE: March 2014 ABOUT US The current list size of Danetre Medical Practice is approximately 12,800. Any increase over the last ten years has been due mainly to; natural change (more births than deaths); building development within Daventry itself and more recently migration from the rest of the UK and overseas. The boundaries of the Practice show we are large in terms of geographical size, with the majority of the land classified as rural. The practice itself is located in close proximity to Daventry Town centre in a modern building adjoining Danetre Hospital (a community hospital which opened in September 2006) offering a wide range of services and delivering a high standard of care. Also attached to the Practice operating independently is Boots Chemist. Currently the practice works with 7 x full session GP s, 220 hrs of clinical nurse & HCA hours. Attached staff include; Health Visitors, Midwife and District Nurses. The role of the Health Care Assistant HCA s are an important part of delivering care in general practice. Current policy is increasing the shift of care from hospital to General Practice providing HCA s with a really exciting career choice. 4 Our Seven Goals To be healthcare professionals who are committed and dedicated to providing patient care To be recognised as a Practice that performs over and above what is expected of them to ensure high quality patient care To have an organisation that promotes a culture of inspiration, maturity and accountability To mange resources and build organisational capacity to enable the delivery of diverse local services To engage with patients by listening to them and involving them in the decision making process To ensure the Practice looks at the holistic picture in their ambition to be a leading provider of medical care To grow profitability and maximize income so that a cost effective healthcare service is provided JOB PURPOSE The post holder is responsible for assisting the nurses, ensuring the delivery of safe and effective nursing services to the whole practice population. As a member of the clinical nursing team the post holder is responsible for his/her own practice, whilst being accountable to the senior trained nurse on duty. Supported by the clinical team within the practice they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. This part time role will be required to cover in periods of holidays, absence and sickness, and when the business need arises. KEY RESPONSIBILITIES AND ACCOUNTABILITIES The post holder will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures whilst managing their own workload to deliver the practice priorities. Clinically, the focus of the role is to undertake clinical activities within the clinical team as requested. Eg. ECGs, health checks, dressings and wound care etc, as delegated by the practice nurse on duty and according to training and competencies. The role will also be evolving as the balance of care shifts from hospital to general practice.

5 CLINICAL Undertake Phlebotomy for individual patients. Undertake screening and diagnostic procedures, promoting and being involved in Health Education activity e.g. height, weight, BP. Processing of samples e.g. urine. Administer First Aid Receive and refer patients to trained nurses when appropriate Understand and adhere to the Policies and Procedures of the Practice Update the computer by coding activities appropriately and at the time of the consultation using templates where appropriate. Comply with computer based information requirements Ensure all items of equipment are maintained in good order, and ensure that all procedures for reporting hazards or unsafe equipment are known, understood and carried out by all staff in the nursing team Participate in a duty rota system within the clinical team to ensure the service requirements are met Ensure economical and careful use of property and resources within the clinical team are used economically and effectively, including ordering of dressings and other surgery equipment. Provide cover for colleagues by undertaking extra shifts when required. COMMUNICATION Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Utilise communication skills to support patients to adhere to prescribed treatment regimens Anticipate barriers to communication and take action to improve communication Estimate and maintain effective communication with individuals and groups within the practice environment and with external stakeholders Act as an advocate when representing the patients and colleagues viewpoints to others

6 QUALITY Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Prioritise, organise and manage own workload in a manner that maintains and promotes quality Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation In partnership with the clinical team, collaborate on improving the quality of health care, responding to local and national policies and initiatives as appropriate Support and participate in shared learning across the practice and wider organisation Participate in team meetings. Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate PERSONAL AND PEOPLE DEVELOPMENT Take responsibility for own development, learning and performance participating in acting as a positive role model Support the development of others in order to maximise potential Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Encourage others to make realistic self-assessment of their application of knowledge and skills, challenging complacency and actions that are not in the interest of the public and/or users of services Participate in planning and implementing changes within the area of care and responsibility Contribute and participate in the development of local guidelines, protocols and standards Participate in the engagement of practice-based commissioning or similar initiatives Keep up-to-date with new developments locally and nationally identifying those that will enhance the team s work Ensure awareness of sources of support and guidance (eg patient advice and liaison service PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring to the trained nurse where appropriate. TEAMWORKING Understand own role and scope in the organisation and identify how this may develop over time Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working Ensure clear understanding and utilisation of referral mechanisms within the practice Accept delegation from nurses, prioritise own workload and ensure effective timemanagement strategies are embedded in own practice Work effectively with others to clearly define values, direction and policies impacting upon care delivery Participate in team activities that create opportunities to improve patient care Participate in and support local projects as agreed with the practice management team

7 MANAGEMENT OF RISK Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines Undertake mandatory and statutory training Apply infection control measures within the practice according to local and national guidelines Apply policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all Participate in the local implementation strategies that are aligned to the values and culture of general practice ADDITIONAL RESPONSIBILITY Attend training courses identified as essential or beneficial to the role and take responsibility for identifying own learning needs. Ensure all significant events are reported. Maintain records of specific adverse incidents, complaints and near misses. INFORMATION GOVERNANCE Information is vitally important for the safe clinical management of patient care and the efficient administration of services and resources, including our workforce. Information Governance is a framework to enable the Practice to handle personal and corporate information appropriately. It is the responsibility of our entire workforce, regardless of employment status, to ensure they abide by the requirements of Information Governance as set out in the Employee Handbook and the Information Governance Policy. CONFIDENTIALITY In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers. They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data. HEALTH AND SAFETY The post holder will be required to comply with the duties placed on employees of the Practice as set out in the Health and Safety at Work Policy and related procedures. The post holder has a legal obligation to make positive efforts to maintain their own personal safety and that of others by taking reasonable care, carrying out requirements of the law and following recognised codes of practice.

Assist patients from marginalised groups to access quality care CRIMINAL RECORDS BUREAU (CRB) DISCLOSURE The post holder will have direct contact with patients and carers therefore a disclosure check will be undertaken by the Criminal Records Bureau. The Practice will normally require an enhanced CRB check to be undertaken. This is a mandatory requirement and a condition of the employment offer. REHABILITATION OF OFFENDERS ACT 1974 This post is not exempt from the provisions of the Rehabilitation of Offenders Act Therefore the post holder is required to disclose any spent convictions. Educated to GCSE Standard or equivalent Knowledge Knowledge of Team working Awareness of own skills and limitations Willingness to study and learn new skills Experience Minimum 2 years Experience in health care setting Desirable Has completed recognised course in Phlebotomy NVQ level 3 Has completed a recognised course in spirometry Awareness of local and national health policy Awareness of issues within the wider health economy System One experience Experience of working in General Practice. Limitations to Authority o The post-holder shall operate within the parameters of the Strategic and Operational Plans and within good practice as understood within Primary care. Accountabilities o Accountability in this relationship is mutual. The Clinical Manager is accountable to the General Practice Nurse to provide support, guidance and resources and to relate to the post-holder in a manner consistent with the Practice values. The Clinical Practice Nurse is responsible to the Clinical Manager and Management Team for performance according to the expectations of the post and for operating within the authority delegated.
