1 Our community nursing roles Community Nursing Services provide nursing care to house-bound patients within the community. Our aim is to help patients to remain healthy and independent for as long as possible, and to support high quality palliative and end-of-life care. Our community nursing services are organised into 4 main areas: Planned care Unscheduled care Intensive Community Support Specialist We provide a 24-hour service. This includes routine planned care as well as urgent unscheduled care. We deliver care in people s own homes for those who are housebound, and in GP practices, health centres and community hospital clinics for those who are mobile. All of our patients will be registered with a GP in Leicester, Leicestershire or Rutland and be at least 18 years of age. Our staff work closely with GPs and community hospitals. We aim to make sure that our patients are cared for in the most appropriate place by the most appropriate healthcare professional. The care and treatment we provide includes: Wound management Care of patients with leg ulcers Catheter care Bowel care Eye drops for patients requiring surgery who have no support at home Intravenous therapy and the care of lines such as Hickman lines Injections Insulin administration and diabetes monitoring for patients who are unable to do this themselves Rehabilitation in conjunction with our therapists The provision of equipment Education to help patients and carers to care for themselves. 2 Specialist nursing services include: Support and care for patients with long-term conditions such as respiratory conditions and heart failure Continence care Palliative and end-of-life care Tissue viability How does our service work? A registered nurse visits the housebound to assess health needs and agree a plan of care with the patient and their family. Our teams, led by district nurses provide general care in people s own homes and in community clinics. Patients with a long-term condition requiring specific expertise may be cared for by a specialist nurse or clinical lead nurse. Our unscheduled care nursing teams can provide care if patients have short-term illness or deterioration of an existing condition, allowing treatment at home to avoid admission wherever possible. They also provide short periods of rehabilitation at home to help patients regain maximum independence following an acute illness or injury. The teams are made up of nurses, healthcare assistants, physiotherapists and occupational therapists. Our Macmillan clinical nurse specialist service and our Hospice at Home team support high quality palliative and end of life care including care in the last days of life. Intensive Community Support The Intensive Community Support (ICS) Service facilitates the provision of enhanced care packages in the patient s own home of nursing, physiotherapy and occupational therapy with medical care provided by the advanced nurse practitioner (ANP). The aim of this innovative service is to respond to patients wishes for care to be delivered in their own homes. In addition this service allows patients to be discharged from acute/community hospital care in a more timely fashion or facilitates enhanced care requested by GPs. Page 2 of 14

3 Across all these services we employ qualified nurses at a range of levels. We also employ people in various unqualified and supporting roles in our nursing teams such as: Clinical apprentices Healthcare Assistants Phlebotomists Trainee and Assistant Practitioners We can offer you career development and progression in your role. All posts have performance based pay increments. There is also a progression structure around many of our key roles, for example you may join as a clinical apprentice, proceed into a band 3 Health Care Assistant, then if you make the grade become a band 3 trainee assistant practitioner, which on completion enables you to be a band 4 assistant practitioner. To give you an idea of what these roles involve, we have included below a summary of some recent adverts for these different roles. They are not jobs that are all being recruited to right this moment, nor are they all the different jobs that we recruit to, but we hope the below gives you an idea of the different types of opportunity we can offer to you regardless of your stage in your career in healthcare. 4 Job Title: Community Phlebotomist, Band 2 An opportunity has arisen to recruit Community Phlebotomists to work within the Community Nursing Service within City (Leicester) and County (Leicestershire). We are looking for a motivated person who would like to work as part of the Community Nursing team providing an efficient and effective Phlebotomy service (venepuncture and simple diagnostic testing) to house bound patients with many different medical conditions. Candidates must be enthusiastic, self-motivated, flexible, have excellent communication skills and enjoy working as part of a team as well as alone and have experience working with the elderly. You will have Phlebotomy competences completed and endorsed (within a nationally recognised health care setting), experience within a care setting (NHS or independent sector) and current IT skills. You will need to have an NVQ Level 1 or 2 in Health and Social Care. The successful applicant will be required to work flexible hours, be dependable and reliable. The focus of the role is to ensure the smooth delivery of a Community Phlebotomy Service to house bound patients in liaison with GP practices and Community Nursing Services. Job Title: Health Care Assistants within our community nursing teams, Band 3 Based across Leicestershire Partnership Trust which includes vacancies across City and County Hours: hours per week The pattern of working will be on a roster basis and will include a shift pattern to cover the service 0800 to 2200 hours 7 days a week including bank holidays and weekends. 7.5 hours a shift. Are we looking for you? If you wish to be part of a team delivering delegated nursing care to patients within their own home, providing effective high quality delegated care to the local population. Are you someone with a strong commitment to national quality standards and integrated working within a community setting? Previous community experience would be an advantage; however further training will be given to successful candidates able to commit and demonstrate LPT values; Compassion; Respect; Trust; Integrity. Our community teams provide a range of services to prevent un-necessary admission to hospital, to support early discharge home, to provide care to house bound patients, End of Life care and to provide rehabilitation programs at home with the ultimate aim for most patients of independence and self-management. The team also works with the acute sector on admission avoidance and provides appropriate advice on discharge and the available supporting services. Leicestershire Partnership Trust is committed to developing its workforce. The band 3 Health Care Assistant post is an ideal starting point to move on in time into a trainee assistant practitioner and ultimately an assistant practitioner role. These roles are a new initiative to increase the number of qualified nursing staff available to look after our patients, and offer an ideal career progression to those who want to take their career in care further, but do not at the moment want to return to full time studies. 5 Our intention is to recruit most of our trainee assistant practitioners from within our existing workforce. Whilst it is not guaranteed that everyone in our Health Care Assistant teams can move into one of these trainee posts, being successfully appointed to one of our band 3 Health Care Assistant posts means you would be well placed to apply for the trainee course as they become available. Job Title: Trainee Assistant Practitioner, Band 3 Are you dedicated, enthusiastic, professional, and want to make a difference? Leicestershire Partnership NHS Trust Community Health Services Division (CHS) is responsible for providing high quality clinical care to our community patients across the City and County. To further enhance our existing provision, Leicester Partnership NHS Trust CHS are looking to recruit enthusiastic and experienced Band 3 Trainee Assistant Practitioners (progressing to Band 4 upon completion) to join the Community Integrated Teams within LPT. These are multiple posts and will be based within an area of the City or County to provide excellent nursing care to a caseload of delegated patients. You will have the ability to work independently and autonomously, providing assessment, support, care, treatment and advice to individuals within the guidelines and protocols identified for the role. You will be expected to plan and implement treatment and care to patients within professional guidelines e.g. dressings, injections and monitoring. You will meet the educational level specified on the person specification and you will have significant experience of working in a care setting, preferably within Health and Social Care and experience of working under distanced supervision. 6 Job Title: Assistant Practitioner, Band 4 Base: Leicester, Leicestershire and Rutland An exciting opportunity has arisen for a responsible and experienced health care professional to join community nursing services. The successful candidate will: Provide effective; evidence based general and specific care for patients under the indirect or remote supervision of registered practitioners. Plan, organise and deliver clinical care as agreed with the supervising practitioner. Use personal effectiveness and knowledge to contribute to the development and delivery of a quality service. Ensure a high standard of care for all patients by effective communication with patients, carers and the wider health care team. This is an excellent opportunity for someone who is seeking to broaden their range of skills and knowledge in caring for people in their own environment as part of a multidisciplinary team. The post holder will participate in the delivery of skilled nursing care to patients within their own home and will carry a caseload of delegated patients. The successful candidate must be someone with a strong commitment to working in an integrated manner within a community setting. You will possess a level 5 qualification in care and have a proven ability to assist in the delivery of care programmes within the patient s own home. Relevant clinical health care experience within a community setting is essential. Community nurses provide a range of services to prevent unnecessary admission to hospital, to support early discharge home, to provide care to house bound patients and to provide end of life care at home. You must also demonstrate a commitment to national quality standards, evidence based practise and integrated working within a community setting and across community teams. Previous community experience and a wide range of clinical experience would be an advantage however further training will be given. Job Title: community nurses intensive community support Band 5 We have a great community nursing service, and we are looking to expand it significantly. To support this expansion, we have a number of new vacancies for Band 5 Registered General Nurses to work on our Intensive Community Support service as we continue to extend our proven services, moving care away from acute hospitals and into a community setting. These roles are within our service that gives nursing, physiotherapy and occupational therapy care to patients in their own home, reducing the time that these patients would otherwise spend as inpatients in hospital. We know from experience that patients recover more quickly than they would in hospital, and patients love the fact that they can remain surrounded by their family, friends and possessions in their own homes. We now need more people to join our teams so that we can offer this to significantly more people. You will work in a multi-disciplinary team alongside other nurses, physiotherapists and occupational therapists. We will trust you to use your clinical judgement to best meet the needs of your patients. However, you will at all times have the back-up of more experienced team leaders and Advanced Nurse Practitioners. You will be a qualified nurse with a passion for providing care to patients. You will be forward thinking and be excited by taking services forward to provide the most appropriate care to patients in the best setting for them. Working on your own initiative and trusting in your own abilities and judgement will be things that you relish, but equally you will need to be aware of when to ask for support or input from colleagues. You will also be fully able to work within a multi-disciplinary environment, working with colleagues within physiotherapy and occupational therapy to support patient s recovery as effectively as possible. Job Title: Tissue Viability Nurse, Band 6 This is an ideal opportunity for someone who is seeking to broaden their range of skills and knowledge in caring for people in their own environment as part of a multi-disciplinary team. This post will support the existing CHS Tissue Viability Team to monitor and evaluate the wound care services across all areas of care within LPT. Applications are actively encouraged from all divisions across LPT and the successful candidate would ideally be able to demonstrate experience of delivering training programmes for wound care. The applicant must be someone with a strong commitment to: working in an integrated manner within the community setting and supporting multi-disciplinary team development. You will have a proven ability to promote patient independence, a sound theoretical knowledge under pinning highly skilled assessment and nursing skills, demonstrate a Page 8 of 14

9 flexible approach to patient care and be adaptable to new initiatives in a culturally diverse setting. You must also demonstrate a commitment to national quality standards, evidence based practice and integrated working within a community setting and across community teams. Post registration experience and relevant academic qualifications in specialist subjects of wound care/leg ulcer care would be an advantage. Job Title: Respiratory Specialist Nurse, Band 6 An exciting opportunity has arisen for a Band 6 Respiratory Nurses to join our Long Term Conditions respiratory team mainly covering the City. This role will include management of patients with Chronic Obstructive Pulmonary Disease within GP Practices, Clinics and peoples own homes. Applicants will be enthusiastic, motivated and have excellent communication skills. Experience in respiratory management of patients is essential as is working in the community setting. The successful applicants will be working within a multidisciplinary team supported by a respiratory team lead. A car driver is essential as the post involves travelling across the city. 10 Job Title: Clinical Nurse Lead, Band 6 An exciting opportunity has arisen for a Clinical Nurse Lead to join our integrated nursing and therapy teams. We recognise that strong clinical leadership is essential for the successful delivery of our vision and the post holder will be responsible for clinically leading and motivating a multidisciplinary team. The successful candidate will liaise closely with GP practices regarding patient care, take clinical responsibility for patients and manage those who are complex. You will be flexible and adaptable and thrive and respond positively to a challenging and changing environment. You will have the ability to effectively promote and lead change. Working closely with the Operational Lead you will ensure that performance standards and best practice are achieved, and you will clinically lead the team in fulfilling our coordinated care objectives, such as engaging patients in their care, ensuring they feel in control and understand their care plan and that their experience is positive as a result of good customer service and effective communication. As a first level Registered Nurse with significant post registration experience, including working at a Band 6 level, you will be able to demonstrate relevant experience and the appropriate competencies for the role. 11 Job Tittle District Nurse, Band 6 Leicester, Leicestershire and Rutland, Full time 37.5 hours. Monday to Sunday between 08:00 and 17:00. For the role of Coordinated Community Health Services (CCHS) across Leicester, Leicestershire and Rutland we require an enthusiastic District Nurse to lead and implement the District Nurse (planned) Team. Part of this roll out includes removing the unscheduled care form the planned care teams. The Unscheduled Care service is a patient pathway improvement within CCHS which removes episodic patient care from the existing District Nursing service and places it centrally within an unscheduled nursing service, thus ensuring planned patient care. This requires strong leadership to ensure teams of community nurses efficiently deliver patient care geographically across Leicester, Leicestershire and Rutland. Excellent communication skills between the planned and unscheduled teams are a necessity to support delivery of the service. Current District Nurse students are also encouraged to apply. We will be developing the role of the ANP in:- Sub-acute care providing medical care for higher acuity patients Community hospitals wards providing medical care for a diverse in- patient case mix Rehabilitation wards providing medical support for patients undergoing rehabilitation Intensive Community services virtual ward concept Physical Health assessment in Mental Health wards Community Frailty units assessment unit with diagnostics to support clinical diagnosis and decision making This diverse clinical case mix requires practitioners with comprehensive assessment and medical management skills to deliver safe effective care. We have a number of exciting training opportunities for motivated personal. For our Band 7 you will need to demonstrate extensive Post Reg experience, recognised qualification/verified portfolio evidence of clinical assessment and decision making skills. You should possess the Independent Non-Medical Prescriber Qualification (V300)or be willing to undertake this. Willingness to study at master s level in Advanced Nurse Practice is required. Possess or be working towards an MSc in Advanced Nurse Practitioner/verified portfolio of evidence and have completed the Independent Non- Medical Prescriber Qualification (V300). Extensive clinical experience in management of long term conditions/acute care is beneficial.
