2 Statements & Search Criteria Mission Statement Venture Community Services provides support to individuals facing diverse challenges through a compassionate and dedicated workforce using innovative practices that encourage independence, empowerment and opportunity. Background Statement Venture Community Services was originally incorporated under the name South Central Massachusetts Rehabilitative Resources in 1975, following the movement toward deinstitutionalization for people with developmental disabilities. A group of citizens began their journey by fundraising for two years to purchase two properties that would serve as communitybased residential programs for individuals transitioning out of institutional settings. These two homes became the very first residential programs in Worcester County for people with disabilities transitioning from Belchertown State School. After the success of their first two homes, the group decided to begin their own company and offer other services. The agency opened a group home in Webster, which became the first licensed adolescent group home in Massachusetts. The agency also started the first family support in-home teaching program in Worcester County. The agency continued to grow, opening more residential programs across the state. In 1999, the agency shortened its name to Rehabilitative Resources, reflecting the expansion of its services across Massachusetts. The organization continues to increase the number of programs offered to accommodate this growth and has been leading the way in designing and constructing handicapped-accessible homes. Over the years, the organization has continued to develop new programs that promote independence and a higher quality of life for individuals with disabilities, such as Supported Employment, Adult Foster Care, Shared Living, and Individual Supports. In January 2015, Rehabilitative Resources officially changed its name to Venture Community Services. This update represents the agency s commitment to remaining an industry leader in providing compassionate and cutting edge supports to individuals and families across Massachusetts. 2

3 Impact Statement Venture's most notable accomplishment in recent years was a hugely success re-naming and rebranding process, in which the organization changed its name from Rehabilitative Resources to Venture Community Services. In 2015, the agency celebrated 40 years of service to individuals and families with disabilities. While our historical foundation is sound, our future will see a new dawn shaped by a visionary leadership team and supported by the core values of honesty, respect, integrity, innovation, and compassion. Our ongoing commitment to innovative growth initiatives will ensure that individuals and their families have access to the opportunities they deserve. Venture is also proud to have recently expanded the Quality Assurance department with the hiring of additional clinical and behavioral specialists, which offers more support to program participants and additional training opportunities for staff members. The agency has also developed a comprehensive Employee Recognition Program, Employee Wellness Program and Assistive Technology Committee. In addition to continuing to provide compassionate, person-centered services that assist individuals and families in reaching their goals, Venture has identified the following objectives: 1. Open additional community day programs to increase the safety net for individuals not currently receiving services 2. Expand autism services and education 3. Collaborate with other like-minded organizations to expand services across the Commonwealth 4. Develop training services for community providers 5. Develop a community-based Autism Support Center featuring a resource library, family support groups, training, and education to families of children with autism in Worcester County at our new facility in Uxbridge 6. Foster partnerships with school systems to provide support for children, families, and transitioning adults 7. Increase transportation services to under-served areas 8. Enhance our therapeutic yoga program by adding more instructors and teaching additional adaptive yoga classes 9. Encourage greater community involvement and meaningful integration for program participants 10. Enrich the agency's Mentor Program to encourage employee growth and provide career opportunities Needs Statement Venture Community Services seeks community partnerships with individuals and businesses that believe in their mission. The agency requests donations during its annual appeal each fall, but the need for financial support is year-round. Despite the agency's overwhelming success working with state funded contracts and Mass Health, there are still many items and experiences that state funding cannot possibly provide. State contracts only cover staffing and basic necessities for the individuals we serve, not the experiences that create a full and meaningful life. That is where our organization can step in - with valuable fundraising dollars, we are able to provide our participants with adaptive equipment and experiences that can enhance their quality of life, allow them to live in their community of choice, and create a spirit of inclusion among their peers. Venture also seeks sponsorships for its three annual events and invites members of the community to support the organization by attending the agency's fundraising events. Venture also gladly accepts donations in-kind, such as adaptive equipment and assistive technology, like ipads and adaptive sports equipment. 3

4 CEO/Executive Director Statement The past year at Venture has been very busy, yet successful. Programming expanded as the agency finished building its newest home in Rutland and welcomed four young men into their beautiful house. Venture recently also opened another home in Franklin through a partnership with the Franklin Housing Authority and the Department of Developmental Services. We also continue to see growth of our Shared Living Program, and our community day programs received a three-year accreditation from the Commission on Accreditation of Rehabilitation Facilities. We expect to continue our development in the coming year with new programs and expanded services, particularly with regard to autism and day programs. In early November, Venture was the recipient of a distinguished award when the company was named an Employer of Choice by the Employers Association of the Northeast. This award is given to companies that demonstrate a commitment to empowering and valuing employees through a compassionate and supportive workplace. The award, coupled with another award bestowed by the Worcester Business Journal last year, is validation that Venture's ongoing dedication to the many professionals who provide vital supports to the people in Venture programs. We are truly fortunate to have such a talented group of professionals who make a positive difference every day in the lives of the individuals we support. These awards reaffirm our responsibility to recognize the important work being done by so many people. Even with all of our recent successes, the agency must be ever mindful of the future. There is a growing need for vital programming and we have an obligation to meet this challenge. Working with various funding sources, we will continue to explore different ways to assist people of all ages living with autism. We are committed to developing innovative and creative support services that will help the Commonwealth to efficiently assist a growing population of people in need. In early 2017, the agency will be amending its strategic plan to reflect the the development of programs that utilize enhanced therapeutic techniques and maximize the use of assistive technology. In addition, Venture will continue working with trade associations to constantly advocate on behalf of the men and women who work incredibly hard every day to provide the people we support with opportunities and dignity. Board Chair Statement I understood at an early age how much of a need for services there was for individuals with disabilities and their families. When I began volunteering with children with developmental disabilities, I realized that they would need someone to advocate for them in the future to help them become productive members of their communities and that is exactly what Venture Community Services does. When I was approached about joining the Board, I felt that it would be a great opportunity to give back. 6 Management CEO/Executive Director Executive Director Term Start Jan Mr. Michael Hyland Experience Hyland brings 20 years of extensive experience to Venture and a sound history of nonprofit leadership, advocating for people of all abilities and the direct care staff that support them. He most recently served as the Vice President of Adult Programs at Kennedy Donovan Center where he oversaw the comprehensive clinical models supporting adults with developmental disabilities and youth with histories of abuse, abandonment and disabilities. Prior to that, he was the Director of Clinical Residential Services at South Middlesex Opportunity Council and a Program Director for Wayside Youth and Family Support Network. 7 Experience/Biography Dot has been with the agency for more than 30 years, and most recently served as interim President and CEO while the agency's Board of Directors undertook a nationwide search for a new Chief Executive Officer. She was promoted to Executive Vice President in January Dot began working at Venture in 1982, and has overseen its growth from a small, regional nonprofit to a $32 million agency with a statewide presence. Kerrie Mason Title Experience/Biography Vice President of Marketing Kerrie has been working as the marketing director for Venture since In October 2013, she was promoted to Vice President of Marketing. Kerrie is responsible for community relations, producing printed marketing materials, advertising, communications, and managing the organization's social media presence. Kerrie also spearheaded a major rebranding process to produce a new name, logo, and mission statement for the organization. Prior to her position at Venture, she worked as a Marketing & Events Manager for the Public House Historic Inn in Sturbridge, Massachusetts and as a Channel Marketing Manager for Digital Equipment Corporation in Acton, Massachusetts. Kerrie holds a Bachelor of Arts in Marketing from Westfield State University. Kevin Hughes Title Vice President of Day Services Experience/Biography Kevin is responsible for the creation and implementation of innovative programming to ensure individuals receive access to opportunity at their community day program. Previously, Kevin worked as a Behavior Analyst for the agency. He holds a Bachelor of Science in Psychology from Bridgewater State University and a Master of Arts in Clinical Psychology from the University of Massachusetts at Dartmouth. He completed further post graduate work at Western New England University in Springfield, Massachusetts in order to fulfill the requirements necessary to receive his Board Certified Behavior Analyst Certificate. Tiffany Breau Title Experience/Biography Vice President of Facilities & Information Technology Tiffany, who has been with the agency since 2002, oversees property management for the organization's Sturbridge headquarters and day program, its day program in Leominster, and the 44 residences it operates from western Massachusetts to suburban Boston. She also coordinates communications and technology among all of Venture's properties. She previously worked as the Executive Assistant to the agency's CEO. 7

8 Kyle Murray Title Experience/Biography Vice President of Quality Assurance & Clinical Services Kyle is responsible for implementing an effective, continuous improvement plan that will ensure the health and safety of individuals supported by Venture. Murray brings 20 years of extensive experience to the organization and a sound history of overseeing clinical services for various human services populations. He most recently served as the Director of Neuro- Behavioral Services at the Neuro Rehab Center in Worcester, where he oversaw a department that supported individuals with traumatic brain injuries and mental health concerns. Prior to that, he was the Director of Residential Services as Aditus, Inc., a Program Director at Brightside for Children and Families, and the Director of Behavioral Services at Options Unlimited. Kyle earned a Bachelor of Arts in Psychology from Quinnipiac University in Hamden, Connecticut and a Master of Science in Counseling Psychology from Capella University. Through our broad reach, services, and expertly trained professionals, we are committed to expanding possibilities in the care and offerings we provide. We believe it is our responsibility to take an innovative and visionary approach that empowers and assists individuals and their families. Venture defines success through the bench marking of strategic goal accomplishment, data analysis, staff orientation testing scores, staff evaluations, and satisfaction surveys completed by individuals served as well as their families and guardians. Venture also analyzes fiscal reports and overall implementation of the agency's strategic plan. The Quality Assurance department develops statistics from data collected and responds to challenges with the addition of new systems. Key staff members from the department meet regularly to review progress, identify concerns, and resolve issues that require action. Venture also greatly values the input of the individuals supported. The agency also has an active Human Rights Committee that conducts site visits and reviews behavior plans. Venture defines success through the progress of the individuals supported - physical, mental, and emotional health, positive experiences, effective integration into the community, increased independence, and the accomplishment of personal goals. 14 Capabilities Venture has a long history of success in opening and operating impressive group homes and continues to increase the number of programs offered to accommodate significant growth. Over the years, the organization has continued to develop new programs that promote independence and a higher quality of life for individuals with disabilities. Venture has forged successful and productive relationships with many offices of the Department of Developmental Services and has a history of collaboratively with service coordinators to provide the best support possible. Venture currently operates 43 community residences, two community day programs, and services such as supported employment and shared living across Massachusetts, providing services to more than 270 individuals. Additionally, all direct support professionals are trained extensively - Venture provides 60 hours of training to its newly hired employees, which focuses on providing behavioral, medical, and therapeutic supports. Venture employs nearly 700 people, many of whom have ten or more years of experience working with this population. 14
