3 BLOCK ROTATIOIN LEARNING DESCRIPTION EXAMPLE: ADULT MEDICINE General Descriptin: The adult medicine experience is a 3 week experience. The Adult Medicine Service prvides direct patient care fr patients wh are seen in cnjunctin with the medical resident service. The PGY-1 pharmacy resident will jin the general medicine team fr daily runds which generally begin at 8:00am. The team cnsists f a chief resident (usually a third year), fur r five ther residents, tw attending physicians, and a pharmacist. The medicine team may have anywhere frm tw t twenty patients n the service. This multi-disciplinary team discussin prvides mre cmplete direct patient care. Activities include identifying patients current acute prblems, gals, mnitring parameters and making recmmendatins. The pharmacist will als research and present drug infrmatin questins and actively participate during his/her assigned time M-F f the week. It is expected that the pharmacy resident be able t perate independently by the end f the experience. The rle f the pharmacist is: is t attend mrning runds with the medical attending staff t prvide pharmaclgical based infrmatin fr patients with a variety f disease states. This includes mnitring, analyzing, and applying knwledge via written and ral cmmunicatin in healthcare team envirnment. This rle mentrs all residents and students wh participate in the experience. Training/ Milestnes: Week 1: The resident will attend FMC runds with preceptr while the preceptr mdels patient electrnic chart assessment, prblem identificatin, ptential interventins and recmmendatins. On the secnd day the resident will wrk up patients prir t runds and review with preceptr after runds t allw fr cllabrative discussin. Patients will then be discussed daily. Week 2: The resident will attend runds daily and the preceptr will attend 2-3 times this week fr bservatin and supprt. Week 3: The resident will attend runds daily autnmusly and will review patients with the preceptr daily. The initial part f training will fcus n accurately identifying acuity f the patient prblems, cllecting disease and medicatin apprpriate infrmatin ( verbally and frm the electrnic chart), analyzing the data, creating a patient specific plan, making recmmendatins t ther health care prfessinals, implementing f plans and reassessment f the plan with apprpriate actins. At the end f 3 weeks the resident will need t cmplete: Achievement in cmpetency is what determines a trainee the ability t practice independently. T be cnsidered "cmpetent" in this rtatin, the trainee must verbally give 10 patients t the preceptr that includes the pertinent infrmatin utlined in the tasks. These patients will be signed by the preceptr and kept in the binder. 7 EXAMPLE LONGITUDINAL LEARNING DESCRIPTION: Lngitudinal Practice Management, Administratin, Prject, Academia/Teaching Experience General Descriptin The Practice Management, Administratin, Prject and Academia/Teaching experiences will happen ver a 54 week perid. These experiences have the ptential t take place in cnjunctin with pharmacists practicing in bth the inpatient and utpatient setting. All bjectives fr these experiences are dented belw. All dcumented resident prgress will take place face t face at the Mnthly Resident Prgress Meetings, Quarterly Midpint in PharmAcademic and End Quarter PharmAcademic Assessment. Practice Management Site(s) FMC, Hspital Pharmacists The pharmacists will help facilitate, mentr and participate when discussing varius methds f rle practice and utilizatin f management tls with the residents. The pharmacist will have a strategic time rganizatinal methd t maintain his/her practice. The scpe f practice and structure f pharmacy services is cmplex. There are many stakehlders and pharmacists must interact with a variety f health prfessinals n a daily basis. T be successful in this envirnment the pharmacist resident must gain expertise in managing his r her pharmacy practice. The pharmacy resident will be expected t explre generalities f practice management by reading bks, reviewing articles and discussing these issues with preceptrs. (Examples f tpics include persnal missin statement, time management, prject management etc.) The pharmacy resident will have pprtunities in all rtatins t hne their ability t manage their practice and bserve hw the preceptr manages his r her practice. Hwever, because sme tpics may nt be experienced during the curse f a specific rtatin, practice management will have lngitudinal cmpnents as weekly tpics and as required prjects as part f the lngitudinal experience. The PGY-2 ambulatry care pharmacy resident assesses, revises, and maintains the lngitudinal curriculum. Administratin Site(s) FMC, Hspital Pharmacists rle The pharmacy directr/manager is respnsible fr versight f all pharmacy related peratins f the hspital. They are accuntable fr bth the clinical and financial perfrmance f the department. A wrking administrative skill set is as imprtant t a pharmacist as a patient care skill set. In this rtatin, the PGY-1 pharmacy resident will develp his/her wn department budget, great a business plan based in his/her perceived next practice and cnduct a SWOT analysis as the first step t priritizing a new initiative. Residency Prject Site(s) FMC, Hspital Pharmacists Review IRB submissins, guide residents in creating a timeline, and being available fr residents t rle check in fr prgress meetings f their prjects. Preceptr is als t help guide resident n the creatin f their manuscript.. The pharmacy resident will cmplete a pharmacy residency prject that is either research r perfrmance imprvement based. The prject will be relevant and useful t the respective site. The pharmacy resident will present the final prject at Great Lakes Residency Cnference in April (required) and at ASHP Midyear in December (if interim data is available). The

8 PGY-1 resident will take necessary steps during the curse f the residency t publish the article in an apprpriate jurnal. The PGY-2 resident is required t publish. Additinally, precept students frm the respected clleges f pharmacy, in additin t precepting medical residents n rtatin at the Family Medicine Center. At the cmpletin f this lngitudinal experience, the pharmacy resident will be able t effectively and efficiently precept pharmacy students independently. The pharmacy resident will design, rganize, and precept PharmD candidates fr at least ne cmplete APPE rtatin. Additinally, the resident will evaluate the PharmD candidate and assist them in the selfevaluatin prcess. By the end f this experience, the pharmacy resident will be able t effectively present didactic lectures t pharmacy students and family medicine resident physicians. The pharmacy resident will create, administer and grade examinatin questins fr nursing r pharmacy students. The pharmacy resident will have met the requirements fr the Indiana Pharmacy Resident Teaching Certificatin by cmpletin f the residency prgram. (Patient educatin will be addressed n specific direct patient care rtatins.) Disease States: A variety f disease states will be discussed in tpic discussins and clinical learning sessins during didactics in bth the practice management and academic experiences. Preceptr Interactin/Cmmunicatins: The resident will need t set meetings at the beginning f each quarter t review the learning descriptin with the respnsible preceptr/faculty. A calendar will need t be presented r discussed at the beginning f each quarter. 9 Administratin: Due t the nature f the rtatin, much f the tasks will be cmpleted upn active discussin with the preceptr and applicatin f material. The resident pharmacist will be expected t schedule all necessary discussins with the preceptr and be actively engaged in these discussins. The resident pharmacist will cmplete required prjects independently with review by the preceptr upn cmpletin, with the preceptr available as a resurce during their develpment. Thrughut the 4 quarters the resident will have discussin with the pharmacy directr, manager, supervisr and clinical crdinatr regarding peratinal prcess, clinical prcess, state and natinal regulatins including (but nt all inclusive) t Jint cmmissin and state bard f pharmacy/health visits, budgets, emplyee relatins with hiring, firing and due prcesses, and meeting prcesses. When the resident cmpletes his/her residency, he r she will be able t navigate many f the administratin prcess within the hspital pharmacy. Prject: Quarter 1: Resident will identify a prject, create a timeline identifying the implementatin, cllectin tl, evaluatin, presentatin and publicatin, submit an IRB applicatin. Quarter 2: Resident will actively cllect data, lead and/r participate in meetings develped arund identified prject. Quarter 3: Resident will cmplete data cllectin and begin t analyze data. Practively set up practice lectures fr GLPC. Quarter 4: Resident will cmplete data analysis, frmulate GLPC presentatin. Present data analysis at the lcal institutin and GLPC. Academia/ Teaching Experience: Quarter 1: The resident will receive direct instructin, bserve mdeling and receive caching activities frm his/her preceptrs regarding students.
