1 FAMILY/MATERNAL & CHILD HEALTH ROTATION OBJECTIVES FORM Purpse f Rtatin: The family is an imprtant influence n health nt nly because it is an integral part f many peple s lives, but als because it is central t tw f the scial determinants f health: early child develpment and the scial supprt netwrk. Much f what impacts family health will be encuntered during clinical training, particularly in the care f wmen f reprductive age and f children. Where gaps exist, particularly fr thse residents pting t cmplete nly ne year f clinical training, this rtatin will bridge thse gaps. It is expected that bjectives will be custmized t acknwledge the specific experiences in which the resident partk. Instructins: The frm is t be cmpleted, reviewed and signed ff with yur rtatin supervisr befre the start f each rtatin. A signed cpy is t be submitted t the Prgram Directr n later than the first day f the rtatin. Keep a cpy fr review at mid-unit evaluatin and at final evaluatin.
