This survey cntinues ASTHO's effrts t prvide a cmplete and accurate picture f gvernmental public health in the U.S. territries and freely-assciated states. Since this study is the nly surce fr much f this infrmatin, yur participatin is essential. Instructins: Please cmplete the entire questinnaire by Tuesday, May 31, 2016 In the Table f Cntents belw, we have made suggestins as t the mst apprpriate respndents fr each sectin f the survey. There are tw ways yu can accmplish this: 1. The Primary Senir Deputy, r the designated Primary Cntact fr the survey (if different frm the Primary Senir Deputy), can frward the survey link received via t the apprpriate staff members and ask them t enter the infrmatin directly int the web-based survey. As mentined abve, it will be imprtant that individuals crdinate when they are entering infrmatin s that tw individuals d nt try and cmplete the survey simultaneusly. 2. The Primary Senir Deputy r the designated Primary Cntact can print ut a blank questinnaire (see instructins belw), distribute hard cpies t the apprpriate individuals, and then g nline t enter the infrmatin they prvide. 1
