2 2016 Implementatin Plan fr the CHNA Adpted n August 25, 2016 Executive Summary Intrductin This reprt prvides a summary f the Cmmunity Health Needs Assessment and Implementatin Planning activities recently cmpleted. Seven lcal healthcare rganizatins (referred t in this dcument as Lcal Prviders) came tgether and participated in this cmmunity health needs assessment and planning prcess: Sakakawea Medical Center (Critical Access Hspital), Cal Cuntry Cmmunity Health Center (FQHC), Custer Health (lcal public health), Hilltp Hme f Cmfrt (lng term care), Knife River Care Center (lng term care), Mercer Cunty Ambulance (EMS), Suthwestern District Health Unit (lcal public health). Each f these rganizatins plays a critical and distinct rle in the delivery f healthcare services t the residents f Dunn, Mercer and Oliver cunties. The health f ur cmmunities and the prcess that was facilitated in this planning prcess is strengthened due t the fact that multiple prviders came tgether, bringing varius skills, diverse knwledge and resurces t bear. The cmmunity health needs assessment was finalized in June f 2016 and was facilitated in rder t help infrm lcal prviders abut the cmmunity s health needs. The purpse f the assessment was t describe the health f the lcal ppulatin, evaluate the use f lcal healthcare services, priritize the identified cmmunity needs, and assist health care and cmmunity leaders identify ptential actin t address the cmmunity s health needs. The infrmatin gathered was then summarized and reprted t Lcal Prviders and cnstituents, and used in planning fr the future delivery f healthcare services. With assistance frm Prairie Health Partners and the Center fr Rural Health at the University f Nrth Dakta Schl f Medicine and Health Sciences, Sakakawea Medical Center, in cllabratin with Cal Cuntry Cmmunity Health Center and ther area health rganizatins, cmpleted the cmmunity health assessment. A summary f the Implementatin Plan that describes hw the rganizatins plan t meet the priritized needs identified in the assessment has been develped, and will be used t measure and reprt prgress n the actins taken t address thse needs within the cmmunity. 2 P a g e

3 Participants The fllwing Lcal Prviders were the participants in the Cmmunity Health Needs Assessment and Implementatin Planning prcess: Sakakawea Medical Center (SMC) a 25-bed Critical Access Hspital lcated in Hazen, Nrth Dakta wns and perates a 34 bed basic care facility, as well as Hme Health and Hspice Services. The nn-prfit hspital is cmmunity wned and gverned by a vlunteer bard f directrs. Cal Cuntry Cmmunity Health Center (CCCHC) is a lcal nn-prfit health care prvider with clinics in Beulah, Center, Hazen and Killdeer. As a federally qualified health center (FQHC), Cal Cuntry imprves access t care by serving all residents, including lw incme and medically underserved peple. Cal Cuntry is gverned by bard members frm the cmmunities it serves. Custer Health a five-cunty multi-district health unit prviding health services t the peple f Mercer, Oliver, Grant, Mrtn, and Siux Cunties. Funded in 1950, Custer Health s services and prgrams include nursing services, envirnmental health, health maintenance hme care, immunizatins, schl health services and a variety f ther services. Hilltp Hme f Cmfrt - a nn-prfit public rganizatin, is a 55 bed skilled nursing care facility lcated in Killdeer. Hill Tp prvides apprpriate nursing care in a hme like atmsphere. Knife River Care Center (KRCC) is licensed fr 86 skilled nursing care beds. Frmally knwn as the Beulah Cmmunity Nursing Hme, Knife River Care Center was incrprated in After varius remdeling prjects, Knife River built a new facility in 2007 and prvides imprtant lng term care services t the area. Mercer Cunty Ambulance (MCA) serves an area f mre than 1,000 square miles with Ambulance Bays in Hazen and Beulah. With paid staff and an active grup f vlunteers, Mercer Cunty Ambulance has apprximately 800 ambulance runs each year. Suthwestern District Health Unit - has been caring fr their cmmunity s health since 1945 and is a multi-district health unit respnsible fr public health in Stark, Dunn, Adams, Billings, Bwman, Glden Valley, Hettinger and Slpe cunties. The health unit prvides a variety f services and prgrams that maintain r imprve the health status f the general ppulatin and their envirnment thrugh Cmmunity Health Nursing, Envirnmental Health/Sanitatin and Nutritinal Services. 3 P a g e

4 These entities will cntinue t wrk tgether int the future t prvide fr the health needs f area residents. While these entities will cntinue t wrk tgether in the service area wide assessment and planning activities, each individual entity will be encuraged t develp an rganizatinal specific strategy fr the unique services it prvides t lcal residents. The Lcal Prviders will meet n a regular basis t cmmunicate prgress twards individual rganizatinal gals and the cllective prgress in meeting cmmunity health needs. SMC and CCCHC as individual rganizatins and cllectively as cllabrating healthcare prviders will wrk tgether t develp strategic implementatin plan pririties and gals. The fllwing pages f this planning dcument are dedicated t the lcal health plan that Sakakawea Medical Center and Cal Cuntry Cmmunity Health Center will use in partnership with area health prviders. Sakakawea Medical Center and Cal Cuntry Cmmunity Health Center Our Cllabrative Missin Wrking tgether as partners t enhance the lives f area residents by prviding a neighbrhd f patient centered healthcare services that prmte wellness, preventin and care crdinatin. Our Visin T be the preeminent prviders f innvative and cllabrative healthcare services. Since March f 2011, SMC and CCCHC have develped a management and gvernance mdel that encurages cmmunicatin and transparency while simultaneusly meeting the prgrammatic and regulatry requirements that each rganizatin must perate under. The rganizatins have reduced, and in sme cases eliminated, duplicative health care services prvided in the Beulah and Hazen cmmunities. The rganizatins have realized the psitive effects f these cllabrative effrts and thrugh the crdinatin f services and resurces are able t prvide services in a mre cst effective and efficient manner while imprving quality and health utcmes. In s much as SMC and CCCHC have implemented cllabrative strategies that have prven successful in imprving and maintaining access t quality healthcare services in the areas they serve, the rganizatins wish t cntinue and expand upn these cllabrative prcesses. T d s, the rganizatins have develped a lcal Implementatin Plan, using infrmatin frm the Cmmunity 4 P a g e

5 Health Needs Assessment, and lessns learned thrugh their cllabratin. The Implementatin Plan will address the pririties identified in the Cmmunity Health Needs Assessment by establishing gals, bjectives and initiatives t imprve the healthcare prvided in the area and imprve the health f the cmmunities that are served. The leader and team assciated with the varius initiatives will develp actins steps and timeline that will be fllwed. The leader and teams will reprt the prgress t the CEO. The Bard f Directrs, Prviders and Management team f SMC and CCCHC will then mnitr prgress and prvide reprts t ur partners and cmmunity residents. Thrugh this prcess, gals have been established t address the identified health needs. Objectives and actin steps have been develped t address these gals.
