These are prvided nly as authrized by case management staff in the participant s Service Plan. Client must meet RCL eligibility as defined in RCL Transitin Guidelines. When t Authrize WA Rads Service prvides WA Rads fr three chrts f individuals: Individuals mving frm qualified institutinal settings t qualified cmmunity settings fr intensive ne n ne relcatin supprt Individuals living in the cmmunity wh are functinally and financially eligible fr waiver/state plan HCBS, and have any f these characteristics: Unstable residential r in-hme settings Frequent institutinal cntacts (ER visits, SNF stays, hspital admits, etc...) Frequent turnver f caregivers Multiple systems invlvement (DOC, psychiatric institutins, etc...) Individuals living in subsidized husing that has been crdinated thrugh ALTSA (including NED, Bridge, 811, etc...), regardless f whether they are currently eligible fr r receiving waiver/state plan HCBS RCL/WA Rads Services Cmmunity Chice Guides - which may include but are nt limited t: Cnducting a pre-transitin cnsultatin with ptential participants t determine viability fr clients interested in transitining frm an institutinal setting and transitin/supprt service needs. Acting as a liaisn with and amng the client, nursing r institutinal facility staff, case managers, husing prviders, medical persnnel, legal representative, frmal caregivers, family members, infrmal supprts and any ther invlved party Prvisin f assistance and supprt t ensure the participant s successful transitin frm the institutinal setting as authrized by HCS and/r AAA staff.

Wrking in partnership with lcal husing authrities and ther cmmunity resurces regarding husing availability, subsidies and vucher capacity. Setting up the participant s new hme including such as arranging utility hk-ups and the acquisitin f furnishings and husehld gds. Arrangement f the participant s physical mve frm the institutinal setting. Liaisn between the participant and husing prvider Training r educatin abut ability t access cmmunity settings, r health. Persnal skill develpment fr client and/r caregivers related t the individual s care plan. Cmmunity integratin activities, such as exercise classes, that might be fund thrugh lcal YMCAtype rganizatins, Senir r Cmmunity Centers, etc. Prfessinal Supprt Services Cvers a range f fr participants whse needs include therapeutic nt therwise cvered by Medicaid. It cvers supprts and that are typically perfrmed r prvided by peple with specialized skill, certificatin r licenses. Are prvided nly as authrized in the participant s Service Plan May include Physical, Occupatinal, Speech r ther prfessinal therapy RCL Prfessinal Supprt Services include: Behaviral Cnsultatin and Technical Assistance- which may include but are nt limited t: Develpment and implementatin f designed t help facilitate inclusin in the cmmunity. Training, behavir supprt plans and/r specialized cgnitive cunseling. Cnsultatin may be in hme r in cmmn cmmunity settings that the persn needs t navigate (i.e., stres, ffices, parks, etc.). Mental health fr participants transitining frm institutinal t cmmunity settings, such as smene experiencing mild depressin r anxiety related t their transitin, in thse instances where the authrized Medicaid benefit amunt, duratin r scpe f service des nt meet the individual s needs. Example: A participant wh may be experiencing depressin r anxiety related t their transitin.
