4 AREA PROFILE CLINICAL AREA LOCATION Planned Care, Wrking Aged adults West Park Centre Newcastle General Hspital Westgate Rad Newcastle upn Tyne NE4 6BE The Cmmunity Mental Health Centre is based n the Newcastle General Hspital site adjacent t Hadrian Clinic TELEPHONE NUMBER STAFF LIST WORKING HOURS Open 9am t 5pm, Mnday t Friday (except Bank Hlidays). PHILOSOPHY OF CARE Westpark CMHT aims t prvide a cmmunity based mental health service fr adults living in the uter west f the city. The CMHT serves individuals wh require medium t lng term interventin which can be psychlgical, scial and pharmaclgical. The team aims t wrk in partnership with service users, carers and ther prfessinals, by giving advice n management f mental health prblems t ther prfessinals and by prviding evidenced based treatments fr thse individuals with time limited disrders and thse with cmplex enduring mental health needs. 4

9 CLIENT PATHWAY CLIENT GOES TO GP (OR OTHER PROFESSIONAL WHO MAY LIAISE WITH GP) WHO FEELS REFERRAL IS APPROPRIATE. ALL REFERRALS ROUTED VIA GP USUALLY BY LETTER UNLESS EMERGENCY All rutine referrals are triaged by Clinical lead except Urgent Referrals that are immediately triaged by Duty Wrker.
