JOB IDENTIFICATION Job Title: Community Mental Health Nurse, CMHT Band: Band 6 Salary: 27,635-37,010 plus DIA per annum pro rata Hours of work: 37.5 (1 WTE) Reporting to: Senior CMHT Clinicians Responsible to: Operational Manager, CMHT Department(s): Community Mental Health Team 2. JOB PURPOSE To have continuing responsibility for the delivery and evaluation of care of a designated caseload providing holistic, evidence based care to patients with mental health problems.

3 4. SCOPE AND RANGE The post holder will be responsible for the delivery and evaluation of care to a designated caseload. To undertake mental health assessments, and provide appropriate evidence based intervention and evaluation of recovery focused care. To supervise junior staff and students in the carrying out of their duties. To provide emergency psychiatric nursing assessment, advice and support as required as part of the duty / on-call rota 5. MAIN DUTIES/RESPONSIBILITIES Manage and organise an identified caseload, maximising use of available resources. Work in partnership with GPs, Consultant Psychiatrists, CMHN s, Social Workers, third sector providers and any other relevant community or hospital based personnel who may contribute to a specific individuals package of care. The post holder will be effective in allocating their own time to ensure that clients receive appropriate care from a coordinated multidisciplinary approach. Take into account at all times, measure and record the levels of risk involved in contacts with clients and act within NHS Orkney s risk management strategy. Assist in the formulation of nursing policies. Contribute to the innovation and implementation of new practices and procedures within own sphere of responsibility. Provide clinical support and supervision to junior staff and student nurses and act as a role model by demonstrating best practice. Contribute to training and education within the statutory and voluntary sector. Participate in the monitoring and development of clinical indicators. Contribute to the development and successful functioning of the Community Mental Health Team in Orkney. Contribute to working groups in promoting a positive image of mental health. Be proactive in challenging stigma associated with mental health issues Ensure their practice remains updated in line with nursing policy and that practice is research and evidence based. Represent the nursing service as required, working locally in partnership with colleagues in the statutory and voluntary sectors. Abide by the NMC Code of Professional Practice at all times. 6. SYSTEMS AND EQUIPMENT NHS Orkney systems for electronic communication; Blog, , NHS Mail, C-Cube. ELibrary and internet resources. Able to access or use IT systems which could include relevant databases, Microsoft office programmes i.e. Word, Excel, Outlook, Access, PowerPoint Desktop and Laptop computer equipment, printers, peripherals. Telephone systems and paging equipment, mobile phones. Clinical equipment relating to role; administration and safe disposal of syringes and

DECISIONS AND JUDGEMENTS To be responsible for the safe management of risk with regard to client, self and others, especially when working in isolation or situations where there is history or potential for actual aggression. To safeguard at all times the confidentiality of information pertaining to clients and staff. To avoid and take decisive action against any behaviour that discriminates against clients of colleagues on the grounds of gender, sexual orientation, marital status, race, religion, ethnic origin or ability. To abide by all relevant NHS Orkney policies and procedures. To adhere to professional and ethical guidelines. 8. COMMUNICATIONS AND RELATIONSHIPS Internal: Maintaining communication which is effective with CMHN colleagues and other members of the multidisciplinary mental health team, administrative staff, primary care staff, hospital, and other health board staff. External: Clients and their families or carers, consultant psychiatrists, community social services staff and voluntary or independent agencies. Act as a point of contact to external individuals/agencies and disseminate pertinent information to and from the CMHT 9. PHYSICAL DEMANDS OF THE JOB Daily driving between appointments and meetings. Requirement for long periods of concentration and managing unpredictable work patterns Providing support to air ambulance crews when transporting patients. Regular and prolonged keyboard use. Occasional challenges from clients who may be potentially or actually aggressive (verbal and/or physical), intoxicated or misusing substance, as well as those at risk of self harm and suicidal ideation. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB To often work at points of crisis, where situations may present risk.

5 Exposure to distressed patients and relatives To work as part of a team ensuring a high level of care is maintained. To work without the provision of inpatient services locally and with no direct consultant input. To provide the nursing care and management of patients awaiting and during transfer to an in-patient psychiatric facility To advise and be the source of information for a wide range of other health and social care professionals, as well as the public. To work in a small and close knit community whilst retaining professional boundaries and client confidentiality. 11. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB RN 1 st Level (Mental Health branch), active NMC registration Evidence of engagement with principles of continuing professional development Experience of working with clients and their families who experience acute mental health problems and with older people. Experience of working with clients who have a cognitive impairment. The ability to provide information and advice to other professionals on mental health issues. Experience of working within a multidisciplinary team Full UK Driving Licence
