3 EXECUTIVE SUMMARY Cvered Califrnia is the state s marketplace fr the federal Patient Prtectin and Affrdable Care Act. Individuals and small businesses can shp the marketplace fr affrdable and high quality health insurance plans. In additin, Cvered Califrnia helps individuals determine whether they are eligible fr premium assistance, cst sharing reductins r ther insurance affrdability prgrams such as lw-cst r n-cst Medi-Cal. Fr mre infrmatin n Cvered Califrnia, visit Cvered Califrnia is annuncing a Requests fr Applicatin (RFA) fr the Navigatr Prgram with a ttal available funding nt t exceed $6.675 millin fr the grant award perid f September 1, 2018 August 31, Grant cntracts awarded under this RFA will be renewed, upn apprval by the Bard, fr tw ne-year extensins. Navigatr Grantees will prvide utreach and educatin thrughut the grant award perid, and assist Califrnia cnsumers with the enrllment applicatin prcess during the annual Open Enrllment perid and during the Special Enrllment Perid. Cvered Califrnia is seeking rganizatins wh have demnstrated their ability t successfully enrll eligible cnsumers. In additin, have an existing presence and established trusted relatinships with cnsumers lcated in the cmmunities in which they supprt. Applicatins will be selected thrugh a cmpetitive grant applicatin prcess and evaluated based n the best verall value and mst effective enrllment strategies. 4 NAVIGATOR GRANT PROGRAM A. PURPOSE OF THE NAVIGATOR PROGRAM The Navigatr Prgram is a requirement f the Patient Prtectin and Affrdable Care Act f 2010 and is funded frm revenue generated by Cvered Califrnia. At a minimum, an entity that serves as a Navigatr must carry ut the Navigatr Prgram Activities described in the Enrllment Assistance Prgram regulatins (CCR Chapter 12 Article 8 Sectin 6664), including but nt limited t: 1. Maintain expertise in eligibility, enrllment, and prgram specificatins and cnduct utreach and educatin activities t raise awareness abut the Exchange; 2. Prvide infrmatin and services in a fair, accurate and impartial manner, which includes prviding infrmatin that assists cnsumers with submitting the eligibility applicatin; clarifying the distinctins amng health cverage ptins, including Qualified Health Prvider (QHP)s; and helping cnsumers make infrmed decisins during the health cverage selectin prcess. Such infrmatin must acknwledge ther health prgrams, such as Medi-Cal; 3. Facilitate selectin f a Cvered Califrnia Health Plan; 4. Prvide referrals t any applicable ffice f health insurance cnsumer assistance r health insurance mbudsman established under sectin 2793 f the PHS Act, r any ther apprpriate State agency r agencies, fr any enrllee with a grievance, cmplaint, r questin regarding their health plan, cverage, r a determinatin under such plan r cverage; 5. Prvide infrmatin in a manner that is culturally and linguistically apprpriate t the needs f the ppulatin being served by the Exchange, including individuals with limited English prficiency, and ensure accessibility and usability f Navigatr tls and functins fr individuals with disabilities in accrdance with the Americans with Disabilities Act and sectin 504 f the Rehabilitatin Act; 6. Ensure that vter registratin assistance is available in cmpliance with Califrnia Cde f Regulatins, Title 10, Chapter 12, Article 4 Sectin 6462; 7. Prvide pst enrllment supprt t ensure successful enrllment and retentin, including increasing health literacy, assisting with renewals, and educating cnsumers n hw t avid disenrllment fr nn-payment; and 8. Maintain a physical presence in the state f Califrnia s that face-t-face assistance can be prvided t applicants and enrllees. B. PURPOSE OF THIS REQUEST FOR APPLICATION The purpse f this RFA is t slicit applicatins frm interested rganizatins t participate in the Navigatr Prgram. Organizatins selected will develp a strategic wrkplan fr their target market that will leverage existing relatinships within their cmmunities t reach eligible cnsumers t enrll them in Cvered Califrnia Health Plans. This strategic wrkplan shuld take int accunt the enrllment gals and identified areas f enrllment pprtunityt establish an utreach, educatin, and retentin strategy that incrprates a staffing plan t accmplish the gals. The strategic wrkplan shuld include a plan t cnduct utreach and educatin thrughut the term f the cntract with enrllment activities cncentrated during Open Enrllment. Page 3

5 C. OUTREACH AND SALES TEAM Grantees will be required t participate in Cvered Califrnia Sales effrts, initiatives, enrllment events, and utreach activities upn request. These reginal field effrts will invlve strategy develpment and cllabratin with ther Navigatr Grantees and lcal stakehlders. Meetings will be held t ensure a shared visin f gals and bjectives fr reaching Cvered Califrnia s subsidy eligible cnsumers and identified areas f enrllment pprtunity. D. GRANT AWARD PERIOD The grant award perid is frm September 1, 2018 thrugh August 31, The amunt f funding in the 2 nd and 3 rd year will be determined at the beginning f each cntract year, but will in nt exceed $500,000 per year. Cvered Califrnia may terminate cntracts with Grantees wh may nt have perfrmed as expected. During the entire term f the cntract, Navigatr Grantees will perfrm utreach, educatin, enrllment, renewal assistance, and pst-enrllment supprt activities. The majrity f enrllment activities will ccur during the Open Enrllment and Special Enrllment perids. GRANT AWARDS Cvered Califrnia has allcated up t $6,675,000 in grant funds fr the 2018/19 State Fiscal Year. Funding levels fr the secnd and third years will be determined in June 2019 and June Cvered Califrnia will award grants in the amunts f $50,000 t $500,000. Final grant awards will be at the discretin f Cvered Califrnia. FUNDING Applicants must submit prpsals that reach a target ppulatin, gegraphic regin and identified areas f enrllment pprtunities. Applicatins reaching target ppulatins will be based n gegraphic factrs t reach thse market segments and ppulatins that have significantly high rates f uninsured individuals and identified areas f enrllment pprtunity. In the areas surrunding areas f Glendale/Burbank/Van Nuys cntains a dense ppulatin f Armenian Americans, which are sme f the densest areas f ethnicity/language in Califrnia. Within the surrunding areas f Burbank, Glendale and West Hllywd there exists a dense ppulatin f Russian Americans. There exists a dense ppulatin f Japanese Americans in the surrunding areas f Trrance. There exists a dense ppulatin area in Playa Vista and Santa Mnica alng the cast. There is an additinal dense area lcated in East Ls Angeles arund Mntebell and Mnterey Park. The areas surrunding Irvine are the densest ppulatins f Chinese Americans in Orange cunty. There exists a dense ppulatin f Filipin Americans in the Chula Vista and surrunding areas. There is als a dense ppulatin in Mira Mesa and Natinal City surrunding areas. The areas surrunding San Jse and Cupertin cntain dense Chinese American ppulatins with Cupertin being the mstdense area. The mst dense ppulatin f Vietnamese Americans in Santa Clara cunty, starts in the Seven Trees district and cntinues nrth thrugh San Jse. There is a dense ppulatin f Filipin Americans in the Milpitas area and Santa Clara's ttal ppulatin is cmprised f a significant number f Filipin Americans. San Mate San Mate Santa Clara Santa Clara Santa Clara Examples f Target Markets include: Hard-t-mve ppulatins with high numbers f uninsured (e.g. yung adults) wh are unlikely t btain health care cverage because they d nt understand the value f having cverage; Ppulatins with Limited English Prficiency; Cllege students; LGBTQ individuals; Culturally diverse ppulatins and cmmunities, such as Native American Indians, Latins, Asians, Asian Pacific Islanders, and African Americans; Families with mixed immigratin status; and Emplyment sectrs in which there are high numbers f uninsured wrkers, including but nt limited t: Cnstructin; Restaurant and ther fd services; Crp prductin; Elementary and secndary schls; Grcery stres; Truck transprtatin; Real Estate; Autmtive repair and maintenance; Page 6

8 Child day care services; Traveler accmmdatin; Hspitals; Investigatin and security services; and Independent artists, perfrming arts, spectatr sprts and related industries. Applicatins targeting a gegraphic regin may include target markets within a specific cunty r gegraphic regin. Cvered Califrnia anticipates funding allcatins fr each targeted ppulatin will be based n the estimated distributin f the uninsured individuals in the targeted ppulatin and the number f cnsumers that the applicant is prpsing t reach. E. ELIGIBLE ENTITIES All Navigatr Prgram Applicants must meet the fllwing eligibility criteria as described belw and in cmpliance with the Enrllment Assistance Prgram regulatins: Demnstrate that the entity has existing relatinships, r culd readily establish relatinships, with Cvered Califrnia eligible cnsumers and self-emplyed individuals likely t be eligible fr enrllment in a Cvered Califrnia Health Plan; Meet any licensing, certificatin r ther standards prescribed by Cvered Califrnia; Nt have a cnflict f interest during the term as a Navigatr Grantee; and Cmply with the privacy and security standards adpted by Cvered Califrnia. Cvered Califrnia welcmes Applicatins frm rganizatins and entities that meet these criteria and als demnstrate prven experience in cnsumer utreach, educatin and enrllment activities. COLLABORATIVE APPLICATIONS AND USE OF SUBCONTRACTORS While Cvered Califrnia will cnsider cllabrative applicatins, these applicatins shuld identify existing partnerships that can demnstrate peratinal readiness and the ability t meet enrllment gals. Cllabrative applicatins shuld identify a lead rganizatin, and list all ther cllabrative partners as subcntractrs. It is the sle respnsibility f the Grantee (lead rganizatin) t ensure subcntractrs meet the eligibility criteria and fllw all ther aspects f the Navigatr Prgram. Cvered Califrnia may require lead rganizatins t submit a cpy f their subcntractr agreements prir t executing an agreement with the lead rganizatin. If a prspective applicant plans t subcntract any enrllment services, the prpsal must include the infrmatin detailed in the Subcntractr Infrmatin (Sectin B.2 in Attachment I). In additin, the applicant and each Subcntractr must submit a Letter f Intent t Participate. There is n prvisin fr re-granting. The use f any subcntractr(s) must be fully explained in the Grant Applicatin/Prpsal). GRANT APPLICATION PROCESS, INSTRUCTIONS AND SCHEDULE The Grant Applicatin Prcess is a cmpetitive prcess where Cvered Califrnia will evaluate strengths and weaknesses f each applicant and make final selectins based n the criteria cntained in this dcument. The gal f the cmpetitive Grant Applicatin Prcess is t identify rganizatins that will prvide the best verall value, quality strategies t address the identified areas f enrllment pprtunity and the mst effective activities t meet the gals, bjectives and guiding principles f the Navigatr Prgram. Applicants wh demnstrate their experience and ability t effectively prvide the services sught, at a cmpetitive return n investment, will be favrably cnsidered fr grant funding. Clarificatins t the Request fr Applicatin will be psted at C. PROTEST PROCESS A prtest may be submitted accrding t the prcedures set frth belw. If an rganizatin has submitted an Applicatin which it believes t be respnsive t the requirements f the slicitatin prcess and shuld have been selected, accrding t the Evaluatin Prcess and Selectin Criteria Sectin, and the applicant believes Cvered Califrnia s evaluatin prcess and selectin criteria was arbitrary and capricius, the applicant may submit a prtest f the selectin as described belw. Final decisins regarding the selectin f Grantees and prtests will be at the sle discretin f Cvered Califrnia s Executive Directr r Designee. All prtests must be made in writing, signed by an individual wh is authrized t cntractually bind the applicant, and cntain a statement f the reasn(s) fr prtest, citing the law, rule, regulatin r prcedures n which the prtest is based.
