We welcome applications from both experienced registered midwives ready to join the team at Band 6 and newly qualified midwives to work at Band 5 and undertake a programme of preceptorship. Working as part of a friendly and supportive team based at the Gilbert Bain Hospital you will be involved in all aspects of antenatal, intrapartum and postnatal care with caseload management responsibility for a geographic area within Shetland. You must be able to work as part of a multidisciplinary team and have effective and adaptable interpersonal and organisational skills. This post involves travelling and a high degree of flexibility including on- call and air ambulance transfers to the mainland. The post-holder will work to uphold the philosophy of care within the unit to ensure that women and their families are the central focus of a high quality service and that care provided is individualised. You will have knowledge of child protection responsibilities and the GIRFEC process. There is a strong history of joint working with other agencies in Shetland to the overall benefit of the local community. JOB IDENTIFICATION Job Title: Integrated Midwife Band 5/6 (Annex T post) Responsible to: Senior Charge Midwife Department(s): Maternity No of Job Holders: 11 Last Update: 3/6/ JOB PURPOSE As an integrated midwife the post holder will have a defined clinical role and caseload providing antenatal, intra-partum and postnatal care to women in both hospital and community settings. As part of the multi-professional team the post holder will provide a range of health education to women and their families. Acting as an autonomous practitioner within the relevant scope of practice to facilitate care in accordance with the philosophy and policies of the NMC, NHS Shetland and departmental guidelines. The post holder will be required to rotate to night duty and participate in an on call commitment, there is a further requirement to undertake road and air ambulance transfers. 3. DIMENSIONS The post holder will: Ensure appropriate use of resources by ensuring high quality, cost effective care. Work to support the professional development of the maternity team, which encompasses both registered, and support staff Maintain and environment in which effective learning can take place Co-ordinates and advises on midwifery care and involvement of the multidisciplinary team within designated areas. ROLE OF DEPARTMENT The Maternity Department provides antenatal, intra-partum and postnatal care for the women of Shetland. Approximately 300 women per year receive antenatal and postnatal care locally, and approximately 1/2 to 2/3 of women remaining in Shetland for intra-partum care. For the remaining women who require transfer to an obstetric unit, this is co-ordinated by the Midwives and Obstetric GP. The Department consists of; one admission room, 2 labour and delivery rooms, one ultrasound scanning room, an outpatient room and 4 in-patient beds.
4 6. KEY RESULT AREAS Within the governance framework the post holder will focus on and support the following key result areas: 1. SAFE AND EFFECTIVE CLINICAL PRACTICE Evidence Based Clinically Effective Practice Support the development of clinical practice through the effective utilisation and integration of evidence into practice; setting, implementing and monitoring evidence based policies, procedures and protocols. Work and practice at all times in accordance with the NMC Code of Conduct, Midwives Rules and Code of Practice. Continuous Quality Improvement Develop a culture that improves quality by ensuring continuous review and development of practice through the use of audit, patient feedback and reflection on practice by self and other members of the team. Develop and maintain clinical skills while being conversant with changes occurring within the profession and the clinical arena. Be innovative and receptive to developments in midwifery practice. Be responsible for planning management and evaluation of programmes of care for individual groups of women. Encourage and participate in audit and research. Patient safety Promote a safe and clean environment for staff, patients and visitors by ensuring compliance with Safety legislation, policies and protocols e.g. health and safety, HAI, risk management, midwifery supervision and critical incident reporting. Assessing and managing actual and potential risks to health and well-being. Ensure high standards of record keeping in accordance with NMC, national legislation and local standards, facilitating effective communication with the multi-professional team regarding patient care. 2. PATIENT EXPERIENCE Coordination of the patient Journey Ensure the planning and coordination of the patient s episode of care, including the smooth transition to other settings, promoting effective discharge and communication with interdisciplinary and interagency teams as required. Maintain accurate records that reflect planning, communication and management of care. Clinical Expertise Coordinate midwifery interventions, influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and experience. Maintain a personal professional profile which provides evidence of completion of objectives and continuing professional development.
5 Promote a Culture of Person centred Care The post holder will be highly visible within the ward/department and communicate regularly with patients and their families within a multidisciplinary team environment. Facilitate care provision which is responsive to the needs of women and allow for informed choice acting as advocate for the patient. Be a credible practitioner promoting equality and diversity through person centred approaches to care. 3. EFFECTIVE CONTRIBUTION TO ORGANISATIONS OBJECTIVES Networking Through successful networking with peers, promote the exchange of knowledge, skills and resources. Service Development Participates with midwifery/obstetric colleagues and the senior midwife in the evaluation of the service for example by audit, development of policies. Involving/collaborating with service users in the development and delivery of maternity services for example talkbacks. Political and Strategic Awareness Develop and maintain a working knowledge of local, national and professional strategy and policy, demonstrating the ability to contribute to policy and strategy development at a departmental and organisational level and, where appropriate national level. The organisations goals will be reflected in the post holders personal objectives. 4. MANAGE AND DEVELOP THE PERFORMANCE OF THE TEAM Role Model As a role model, create a supportive ethos to empower staff to contribute to the delivery of high quality patient centred care. The post holder should be recognised as a clinical expert. Learning and Development Contributes to the development of a learning environment that ensures effective learning opportunities for all staff and students, facilitating a range of clinical support strategies e.g. mentoring, coaching, clinical supervision and action learning. Contribute/participate in ongoing education/updating relevant to clinical practice. Participate in teaching of peers, medical students, student nurses and midwives. Undertake all mandatory training and educational objectives set out in the PDP. Manage the Practice Setting Support the midwifery team by ensuring compliance by self and others with professional standards legislation, national and organisational policies. Maintain appropriate staffing levels taking account of clinical workload and competence of staff/skill mix.

EQUIPMENT AND MACHINERY The post holder is expected to have a knowledge of all equipment used in the clinical area, including: Moving and Handling equipment to assist patient mobility and promote comfort Medical and technical equipment to record vital signs and administer treatments IT equipment to read, analyse, record and transmit information 7b. SYSTEMS The post holder will work with or contribute to: Maintenance of patient nursing records in accordance to NMC guidelines and NHS Shetland Standards Annual appraisal and Personal development plan Formatting and populating data bases as required 8. ASSIGNMENT AND REVIEW OF WORK The post holder will be responsible to the senior charge midwife for clinical guidance and professional management, work review and formal appraisal of performance. The post holder is required to: Ensure adequate staffing levels based on clinical need, to provide safe and effective care. Participate in the day to day management of the clinical area and deputise for the senior charge midwife in her absence Implement agreed policies and procedures and participate in the evaluation of these Participate in the formulation of clinical care guidelines Be accountable for the optimum use of resources 9. DECISIONS AND JUDGEMENTS The post holder will be constantly required to make judgements involving a range of facts and evolving situations, which require the analysis of data from a variety of sources and comparison of a range of options, and to review the judgement of others. The post holder will
7 be clinically and professionally expected to make autonomous decisions on a daily basis, advising on a range of issues to patients and maternity staff and supporting the multidisciplinary team in decision making. Responsible for counselling women with abnormal results. Function as an autonomous practitioner, working within clinical and professional guidelines. Make autonomous decisions in planning and prioritising patient care and delegating to team members. Recognise own limitations in the provision of clinical care referring to other professionals. Ensure adequate staffing levels based on clinical need. Administer prescribed medications in line with NMC regulations. Take responsibility for the supply and administration of medicines covered by Midwives Exemptions i.e. diamorphine and maintain appropriate records. Manage a safe clinical environment, ensuring that all aspect of health and safety for patients, staff and visitors are being met. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Managing the demand of direct patient care within existing resources. Negotiating packages of care with other disciplines/agencies who work within different cultures. Managing a wide range of patients from low to high risk women and neonates. Coordination of care with a wide range of specialist staff utilising their skills effectively. Coping with highly charged emotional situations when breaking bad news to parents. Informing women of abnormal screening results. Individualised counselling of women with high risk screening test results. Ability to make immediate decisions. Management obstetric and neonatal emergencies. Frequent exposure to body fluids. Undertaking intimate examinations. 11. COMMUNICATIONS AND RELATIONSHIPS Establishes and maintains robust communication networks. Communication will be both informal and formal and consist of verbal reports and presentations to a range of audiences. Internal

Regularly communicates information to parents relating to miscarriage and occasionally fetal anomaly and still birth. Regularly inform, counsel and support women/parents who have abnormal screening results. Maternity colleagues- patient care, allocation of work, professional issues. Work with team members in implementing change, developing the service and providing the optimum level of care. Communicate effectively with all members of the multidisciplinary team in a range of situations to promote teamwork and enhance good working relationships. Be able to effectively communicate with learners during teaching/training sessions. Support leadership and teambuilding. Other departments- Human resources, Infection control, Estates. Other agencies i.e. drug and alcohol services, mental health team, housing department. 12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB Physical skill Manual handling skills. Keyboard / word processing skills. IV Cannulation, venepuncture and the administration of intravenous and intramuscular injections. Undertake perineal repair high degree of manual dexterity required. Assist women in the birthing process. Manage obstetric and neonatal emergencies. Assist with breastfeeding. Physical effort needed for post: Moving and handling patients and equipment. Working in cramped and/or restricted position. Risk of repetitive strain injury.
9 Ability to work in a sustained and focused manner. Standing/kneeling/bending and twisting for long periods. Ability to cope with stressful situations on a frequent basis. Resilience. 13. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB Registered Midwife with evidence of continuing professional development. Good interpersonal skills. Excellent communication skills. Excellent decision making skills. Able to work well within a multidisciplinary team. Competent in basic neo-natal resuscitation and contribute to managing obstetric emergencies. Experience of mentoring and preceptorship and a mentoring qualification or willing to undertake the training.
