JOB IDENTIFICATION Job Title: Responsible to: Department & Base: Clinical Pharmacist BGH Lead Clinical Pharmacist Pharmacy Department, Borders General Hospital Date this JD written/updated: Job Reference No.: PHAR JOB PURPOSE To deliver high quality pharmaceutical care to NHS Borders patients by contributing to the pharmaceutical care of individual patients within Borders General Hospital, other secondary care sites through the dispensary, ward based clinical pharmaceutical service and prescribing support. To assist in the delivery of high quality pharmaceutical services to patients across secondary care within NHS Borders. To assist in ensuring the safe, effective and cost efficient use of medicines throughout the NHS Borders. To assist in the implementation of medicine reconciliation on admission, transfer and discharge of patients throughout NHS Borders. 3. ORGANISATIONAL POSITION - see attached sheet

2 4. SCOPE AND RANGE Responsible for providing clinical pharmaceutical service to appropriate wards or areas including out-patients and other NHS Borders units. Responsible for monitoring medicines use & formulary management, including adherence to local prescribing policies & protocols Contribute to the supervision and teaching of pre-registration pharmacy graduates (hospital and community based), junior clinical staff and patients regarding medicine use Liaise with other clinical pharmacists within the pharmacy Overseeing dispensary as part of the working day, including professional checking & query resolution. Pre-checking & final checking aseptic & cytotoxic dispensing. Directs pharmacy staff including pharmacists, technicians (MTOs & ATOs) while involved in provision of pharmaceutical care 5. MAIN DUTIES/RESPONSIBILITIES Pharmacy Department Based services o Professional clinical check of prescriptions. o Dispensing of prescriptions as required o Final Checking of dispensary and aseptic/cytotoxic dispensing as required. o Clinical query handling and resolution. o Supervision of OTC medicines sales. Ward Based Services Provision of clinical pharmacy services to designated patient groups as appropriate: o Verification of in-patient and discharge medication orders o Medication history taking and medicines reconciliation verification o Documentation of care and risk management issues including: Patient allergy Patient self administration (including assessing patient suitability) Adverse drug reactions and interactions Individualising pharmaceutical care plans and documenting / resolving pharmaceutical care issues o Counselling patients on the use of their medicines

3 o Acting as primary contact point for ward staff in relation to pharmacy issues when providing ward pharmacy service o Liaising with ward staff and the pharmacy technicians to maintain and update ward stock lists o Provision of medicines information to clinical staff and patients Primary care based services rotation To gain experience, under supervision, of: o promoting evidence based cost-effective prescribing to GPs, care homes, and other healthcare colleagues and the public as part of the Prescribing Support Team o organising and actioning drug changes in practices for quality and cost reasons CPD/Training commitment Update current skills and knowledge in relevant practice areas as required. Undertake at least the minimum CPD commitment recommended by the General Pharmaceutical Council Participation in departmental continuing personal development programme including PDP as appropriate Participate in Scottish Hospital Pharmacists Vocational Training Programme where appropriate Participation in internal or external courses/meetings as appropriate. Development and maintenance of competency in aseptic manipulation Assisting in the training of other pharmacy staff in particular: o Student Pharmacy Technicians o Pre-registration Pharmacy Graduates from hospital and community sectors Comply with NHS Borders and pharmacy department strategy and policies Responsible for the provision of specialised advice to medical and other staff as well as to patients, their relatives and carers To participate in weekend/holiday/late night rotas as required, dispensing medicines and providing professional checks for prescriptions. Participate in the provision of BGH Pharmacists emergency duty service for NHS Borders hospital units. 6. SYSTEMS AND EQUIPMENT Use of ASCribe pharmacy stock control and dispensary systems Stock control of pharmaceuticals Dispensing of medicines for patients

4 Medicinal charts for patients Use of management reports Create/updating of patient records Use of Microsoft Office suite (internal and external) Internet enquiries use of specialist medical/pharmaceutical databases etc Word processing, policies, notices, forms etc Spreadsheets; Management information, statistics etc PowerPoint Presentations Use of medicines information software & CD-ROM databases. - internet databases - e-library Use of datix incident reporting system Responsibility for the security of pharmaceuticals, includes security of keys and use of alarm system Use of telephone, radiopager 7. DECISIONS AND JUDGEMENTS The postholder is a member of the professional staff of the pharmacy department and will work within strategic and policy guidelines established by the NHS Borders. The postholder has responsibility for managing their allocated workload within specialist area with minimum supervision but will have close links with the Lead Clinical Pharmacist or with the Lead Prescribing Support Pharmacist under rotation to Prescribing Support. Uses knowledge and experience to assimilate and evaluate, often conflicting, information in order to advise on pharmaceutical matters Annual objectives will be agreed jointly with Lead Clinical Pharmacist and review of performance will be carried out by Lead Clinical Pharmacist. 8. COMMUNICATIONS AND RELATIONSHIPS The postholder develops and maintains communications within & without the organisation Communicate directly with: Colleagues from all disciplines & professionals within NHS Borders (medical, nursing, AHPs, GPs, community & P&CS Board Pharmacist) Pharmacy staff pharmacists, technicians (MTOs), assistants (ATO s) and A&C staff

5 Clinical staff at ward level Patients at in- or out-patient situations clinical issues, service issues, queries and complaints Public service issues and queries Pharmaceutical industry at local or national level Nationally with pharmacists & other healthcare professionals in the Scottish/UK national networks/special interest groups etc Represent NHS Borders and Pharmacy at meetings of various groups both locally & nationally Communication will include face to face, telephone, written, , meetings, presentations 9. PHYSICAL DEMANDS OF THE JOB Combination of walking, standing, sitting, driving with occasional light physical effort General keyboard skills High degree of precision & accuracy for dispensing & checking, often with strict time constraints. Clinical work requires a high level of concentration characterised by assessing different pieces of sometimes conflicting information and forming a clinical opinion. This work is often conducted under time pressures and subject to frequent interruptions. The allocation of clinical work will vary with service need and clinical rotation. Frequent levels of high concentration, often with interruptions when checking dispensing or reviewing patient records. Near patient contact involving exposure to environmental risks at ward level. Potential exposure to toxic pharmaceutical materials Validated aseptic techniques including working in isolators with restricted movement (complex manipulations with arms in glove parts of isolator fixed working position) Emotional effort :- -occasional exposure to distressing or emotional situations, both in case of patients & staff

MOST CHALLENGING/DIFFICULT PARTS OF THE JOB To provide high quality pharmaceutical care tailored to meet the needs of individual patients This requires continual update of clinical pharmacy knowledge base. To ensure the safe, efficient & economical use of medicines including the delivery of individualised patient care. To assist in the implementation of medicine reconciliation on admission, transfer and discharge of patients throughout NHS Borders, working with pharmacy and other clinical teams within the timeframe of this post and clinical excellence project. To effectively prioritise the workload to meet the needs of users (patients, colleagues, managers) To maintain & improve working relationships within NHS Borders.

7 Clinical Pharmacist Band 6 (updated 11.11) Essential Desirable 1 Qualifications 2 Experience Masters Degree in Pharmacy Registered with General Pharmaceutical Council (GPhC) Evidence of commitment to Continuing Professional Development 1 year post qualification training and experience as a preregistration pharmacy graduate Member of the Royal Pharmaceutical Society of Great Britain Experience in a hospital pharmacy department at pharmacist or preregistration pharmacy graduate level 3 Skills 4 Personal qualities Knowledge (therapeutic) and practical skills for dealing with a range of pharmaceutical interventions. Ability to prioritise own workload Excellent interpersonal communication skills both written and verbal Ability to form working relationships with wide variety of people Working knowledge of standard computer programmes and of clinical and pharmacy systems. Knowledge of NHS Borders policies and procedures and a detailed knowledge of pharmacy policies and procedures. Experience of the Scottish Patient Safety Program ( SPSP) model for improvement and associated improvement tools and methods. Knowledge of the SPSP Medicines management workstream. Experience of implementing and delivering medicine reconciliation in the medicines management process, at admission, transfer and discharge of patients. Ability to prioritise own workload Ability to work as part of a team

8 5 Research and training Participation in local, regional and national educational opportunities and other sources of continuing professional development to develop and maintain currency of knowledge. Completed (or willing to undertake) the Scottish Hospital Pharmacy Vocational Training Scheme Stage 2, (or demonstrable evidence of clinical knowledge equivalent to that level) 6 Other Willing to participate in weekend/holiday/late night rotas as required, and in the provision of BGH Pharmacists emergency duty service for NHS Borders hospital units.
