1 Head Office: Thames Court, Richfield Avenue, Reading, RG1 8EQ Regional Offices: Centenary House, Durrington Lane, Worthing, BN13 2BQ Kilburn Healthy Heart Centre, 210 Kilburn High road, London NW6 4JH Kentish Town Health Centre, 173 Kentish Town Road, London NW1 8PD Lansdowne Bldg, Lansdowne Road, Croydon CR9 2ER West Wing, St Pancras Hospital, 4 St Pancras Way, London, NW1 0PE USA: 77 Water Street 8th Floor, New York City, New York JOB DESCRIPTION FOR: Outreach Health Check/Stop Smoking Advisor and Driver Reporting to: Salary: Location: Contract type: Travel: Outreach Operations Manager 17,000 to 19,000 pro rata Peterborough Part-time (30 hours) Travelling to various clinics within a 10-mile radius JOB SUMMARY To carry out NHS Health checks in accordance with the national programme led by the Department of Health including supporting and motivating clients to achieve their desired goal of stopping smoking MAIN DUTIES AND RESPONSIBILITIES Drive the Company mobile clinic Wellness on Wheels Assessment of Cardiovascular disease risk using an approved assessment tool Communication of risk to clients Advice based on current health messages Provide advice and behavioural support to enable the individual to make a quit attempt Follow protocols for medication requests and use of equipment such as infection control procedures and carbon monoxide monitoring Advise on appropriate medication required to maximise the objective to quit Brief intervention for lifestyle improvement Signposting motivated clients to appropriate local services. 2 Accurate and timely recording of medical notes/consultations: o Ensure accurate and legible notes of all consultations and treatments are recorded in the clients notes o Reporting of serious clinical findings appropriately and in a timely manner to clients GP o Ensure the clinical computer system is kept up to date with accurate details recorded. o Ensure accurate completion of all necessary documentation associated with client health care The post will also include administrative duties: o Ordering/Restocking/maintenance of equipment, and stock rotation and reporting about them regularly to the Project coordinator. o Ensure clinical waste collection runs smoothly, reporting any problems to the Project coordinator. o Participate in the administrative and professional responsibilities of the project team. o Ensure appropriate items of service claims are made accurately, reporting any problems to the Project Coordinator. o Assisting with the collection and collation of data on needs related to health and well-being. Attend and participate in project meetings as required Ordering and display of Health Promotion materials in conjunction with client participation group. To participate in any training programme implemented by the project as part of this employment, such training to include: o Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development o Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. Clinical governance responsibilities for own and colleagues quality and performance, e.g.: o Alerting other team members to issues of regarding quality of service, risk reduction and reporting of near misses. o Assess own performance and take accountability for own actions, either directly or under supervision o Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the

3 team s performance Networking and sharing of information/best practice: Work effectively with individuals in other agencies to meet expected client needs Communicate effectively with clients and other team members. Recognize the need for alternative methods of communication/language and respond accordingly. Effectively manage own time, workload and resources to meet the needs of the clients and service. Participate in and support health information or other campaigns as directed.
