2 The Embassy Suites Norman is hosting the Oklahoma Association of Health Care Providers and the Oklahoma Assisted Living Association Annual Convention and Trade Show. Our theme is focused on Jimmy Buffet Caribbean style Changes in Latitudes, Changes in Attitude in Senior Care Convention Objectives: The objectives of the Convention are to provide quality, affordable education programs that are designed to inform and educate providers on the trends, regulations, and newest products and developments in Senior Living. Convention Overview: This convention is comprised of 15 hours of CEU hours for Long-Term Care, Assisted Living, Residential Care and Home Health Care Administrators. A registered attendee may attend any session. This year brings together national, state and local speakers with topics relevant to Senior Care. The trade show opens with an evening reception Monday, May 5th and a silent auction. This is the largest health care trade show in the State of Oklahoma. Early Bird Registration: Early Bird registration makes the convention even more affordable. All on -line registrations received by April 4th at 5:00 p.m. will receive over 22% off the regular registration fee. TO RECEIVE EARLY BIRD DISCOUNT, REGISTRATION MUST BE PAID IN FULL BY APRIL 4th. Registrations/payments not received by close of business on April 4th will pay regular registration fees and will be invoiced as such. Members of OAHCP and OKALA receive significant discounts in registration; however, EARLY BIRD registrations must be paid by April 4th by close of business 5:00 p.m. Pay on-line when registering via credit card or PayPal. Name Badge: Upon registration check-in, each attendee will receive a name badge and must wear during all convention functions. Continuing Education: A registered attendee may attend any session for CEU s. These education offerings have been submitted but not yet approved to the National Association of Board of Examiners of Long Term Care Administrators (NAB) for Nursing Home, Assisted Living and Residential Care Administrators; to the Home Care Administrator Directory of the Oklahoma State Department of Health for Home Health Care Administrators; and to the National Assisted Living Administrator certification program administered by OKALA for 15 participant hours. This program has also been submitted but not yet approved to the Certifying Board of Dietary Managers and the Oklahoma Dietetic Association for 6 participant hours on Wednesday, May 7th. CEU punch forms must be completed and turned in at the end of the day on May 7th to receive continuing education hours. A certificate of attendance will be mailed to all CDM attendee s that have signed/completed attendance forms. For accurate CEU and attendance accounting, registrants MUST check in EVERY DAY at registration desk to receive a CEU punch form. There is a total of 15 hours of continuing education available for this 3-day convention. Monday = 4 hours; Tuesday = 6 hours and Wednesday = 5 hours. It is the responsibility of the person seeking continuing education to abide by the rules in receiving actual hours attended. 2

3 CEU punch forms are duplicate 2-ply pages. Please be sure to turn in the white copy of the filled out and punched card DAILY for accurate accounting for your CEUs. Please keep the yellow copy for your records. If you need to verify your CEU attendance in the future, this is the only mechanism for OAHCP or OKALA to use in order to assist you. Trade Show Admission: The Trade Show is open to all convention attendees wearing a name badge. We are so appreciative of our vendors who exhibit in our trade show and thereby make this convention such a success. Therefore, only exhibitors and sponsors of this convention are allowed to showcase their products to our attendees. Lodging: Embassy Suites Norman Hotel & Conference Center, 2501 Conference Drive, Norman, OK is our host hotel. We have a block of rooms available at a special room rate of $133 (single/ double). Links are available on our websites or Smoking rooms are available but limited so make your reservations early. The cut-off date is Sunday, April 20th. Cancellation Policy: Early Bird registration payment must be received by 5:00 p.m. on April 4th. Regular registration fee will be invoiced if payment not received by April 4th. Cancellations received up to 4 weeks of event date (April 7th) will receive full refund minus a $40 per person administrative fee. Cancellations received up to 2 weeks of event date (April 21st) will receive a 50% refund. NO REFUND/CREDIT received on cancellations after April 21st; however, a substitution may be sent at no additional cost. Contact OAHCP at (405) from April 21st to May 1st of any substitutions. After May 1st substitution will be handled on-site at the walk-in desk at convention. Cancellations must be faxed or ed to ccook@oahcp.org before first day of convention. Registrants who fail to cancel and do not attend are responsible for the full registration fee. A full refund will be granted for a registrant in the case of a medical emergency or death in the immediate family. A written request for a refund explaining the reason is required before a refund will be granted. A written request along with supporting documentation showing the date of the survey is required before a refund will be granted. 4 Attire: The appropriate dress for the convention is business casual or appropriate beach themed clothing. However, participants are encouraged to bring a light jacket or to dress in layers to accommodate varying room temperatures. Please remember that comfortable shoes will be helpful since you will be walking throughout the convention center and tradeshow.
