Our primary goal is to provide opportuni es for individuals with a disability to develop recrea on skills and explore interests while providing a break for their caregivers. Although we also offer some group and family-oriented ou ngs, we encourage parents to take advantage of our respite service. Taking me away from caregiving responsibili es allows caregivers to take care of themselves such as run errands, go to doctor s appointments, maintain social rela onships and to re-energize, which ul mately allows for more effec ve caregiving. Programs that are in the sec on Programs for Individuals are intended to be respite in nature and count toward waiver par cipa on. Individuals must par cipate in at least one individual program each year to remain ac ve on CP Rochester s roster. Our programs are community-based and par cipant-driven. We encourage families and par cipants to make sugges ons and provide feedback on the programs that are offered. Please note that the only programs that we offer a staffing ra o of 1:1 is for the Individual Swim Lessons, Fun Fitness and Adap ve Bouldering. Other programs u lize staff and volunteers for support and oversight in an age appropriate ra o. To ensure a fun and safe environment, staff and volunteers consider individual s abili es to determine whether se ng and ac vi es are appropriate for each program. Most of our ac vi es take place locally, however, others explore venues outside of Monroe County. We are always open to working with new partners and venues. 2 Page 2 Programs for Individuals Individual Swim Lessons Instruc on is provided by our Adapted Physical Educa on teacher and swimming instructors and is designed for individuals who have special needs. Students will receive 30 minutes of instruc on and will work toward developing independent swimming skills and increased safety awareness in the water. For each swim program the cost is: $90 per person Ages: 3 and up Space is limited! Please indicate your preferred me on your registra on form and include a second choice. 3 Page 3 Programs for Individuals S P R I N G Bowling League Our bowling league is a great recrea onal and social opportunity. We bowl weekly and celebrate at the end of the league with a pizza party & award ceremony. No previous experience is necessary, just an interest in bowling. Individuals will be grouped into teams and score will be kept each week. Bumpers & bowling ramps are available so people of all abili es and experience levels can par cipate! Tuesdays Program #3: March 22, 29, and April 5, 12, 19, 26 May 3, 10 Time: 6:30-8:00pm Deadline: March 1st Cost: $54 Ages: 8 and up Group Size: Limit 12 people Loca on: AMF Fairview Lanes, 1407 Fairport Rd, 14450

4 Page 4 Programs for Individuals Gymnastics Jump, run, flip, and be crea ve in this gymnas cs class at The College at Brockport; program is supported by students in the Physical Educa on program. Par cipants are partnered with a volunteer throughout the class and as they move from sta on to sta on. Individuals can try out the balance beam, roll and flip on the mats, hop over the horse, bounce on the (small) trampoline, and (everyone s favorite) hop into the big foam pit to relax at the end of class! Program #4: Time: 9am-9:45am Ages: 6 18 Cost: No cost to par cipant Group Size: Limit 8 individuals Deadline: This session is currently underway and full. 5 Page 5 Programs for Individuals S P R I N G :1 Fun, Fitness, and Physical Activity This fun fitness program provides the opportunity for increased physical ac vity levels and skill development by working with a 1:1 instructor. Sessions are geared toward the individual's age and interests with a focus on aerobics, muscle strength, endurance, and flexibility components. Each session is 30 minutes long and is available for ages Descrip on for individuals ages 5-9: Sessions include skill development and opportuni- es for increased physical ac vity through par cipa on in ac vi es/games. Fitness levels will be assessed by observa on. Descrip on for individuals ages 10-17: Sessions will involve fitness tes ng (Brockport Physical Fitness Test) to assess each individual s specific needs. A er assessment, target areas of fitness will be selected by the individual; games and ac vi es will supplement the session. 6 Page 6 1:1 Adaptive Bouldering This unique form of rock climbing goes sideways rather than ver cal, therefore, no ropes or harnesses are required. This program is a great way to get youth exposed to the world of rock climbing in a safe quiet environment. Students will receive 30 minutes of instruc on and will work toward developing coordina on and balance. (Close toed shoes required) Ages: 5 10 Program #7: Wednesdays, March, 16, 23, April 6, 13, 20, 27 Lesson Times: 4:00pm, 4:30pm, and 5:00pm Loca on: CP Rochester Gym at 3399 S. Winton Rd. You can also enjoy the traveling exhibits of the RMSC during your visit. Please note, this is a family program and staff is not present at the museum. 9 Page 9 The Enrollment Process 1. Contact your Medicaid Service Coordinator 2. Complete an Intake Packet for Respite / Recrea on > Available at on.asp 3. Submit your completed Intake Packet to the Intake Coordinator [mail to CP Rochester at the address listed in the box below] 4. Obtain approval for Recrea on Respite services In order to par cipate in Respite Recrea on Programs, individuals must: have Medicaid be enrolled in the HCBS waiver have received approval for the service from CP Rochester and DDRO par cipate in a Respite Recrea on Program annually How to Register for Programs All required intake documenta on must be received and processed prior to registering for an event or program. A) Complete the registra on form on the last page of this brochure and mail it to: CP Rochester, 3399 Winton Road S Rochester, NY A n: Recrea on B) your registra on informa on to klyon@cprochester.org; please include all the informa on that is required on the registra on form so it can be processed correctly. A confirma on will then be sent to you confirming the date(s) and me of the ac vity. Please keep this as a reminder of your upcoming programs. Prior to program registra on, staff must ensure that all the required documenta on is in place for each person, and at mes we must follow up with your Medicaid Service Coordinator. Sending payment in with your registra on will not guarantee a spot in the program. You may call or to check on the status of your registra on: Phone: ext. 2271/ klyon@cprochester.org Program Fees We try our best to offer low or no-cost recrea on programs, however, some ac vi es do incur expenses beyond basic staffing resul ng in fees which must be paid a er successful registra on. This cost applies to both HCBS/Care at home waiver enrolled AND non-waiver enrolled individuals. If you are enrolled in the HCBS Waiver or the Care at Home Waiver, you will s ll need to pay the fee that is listed for that ac vity. If requested, we will provide a receipt for payments made to CP Rochester for Recrea on Programs. You may choose to submit your receipt for Family Reimbursement please contact your Medicaid Service Coordinator for further informa on. Payments or Cancellations Payments or cancella ons must be made by the program s registra on deadline. If we have not received payment one week prior to the event s date or start of the sessionand you have not contacted us, we will assume that you are not going to par cipate in this program anymore. Past due payments will affect registra on for future recrea on programs. In the event that a program is cancelled, we will issue a voucher that can be used toward future recrea on programs; we do not issue refunds. Please make checks payable to: CP Rochester Project Access Transportation Project Access is a grant supplemented service that provides, low cost transporta on to individuals with disabili es living with family in Monroe county. 12 Page 12 We would like to introduce you to Ray s Social Club; an inclusive social outlet for young adults ages Ray s Social Club brings together teens and young adults twice per month for discussions, social activities, friendship and support. Social activities throughout the community are planned in advance with assistance from CP Rochester staff. The social activity takes place on the 1 st Saturday of each month. Additionally, members of the Club take part in monthly facilitated group discussions. Guided by staff with a counseling background, members are able to talk about challenges, concerns and frustrations that they are experiencing and gain peer support. The group discussions take place on the 3 rd Thursday of each month. Regular attendance is a requirement of Ray s Social Club. We are excited to share this opportunity with you!
