An exciting opportunity has arisen within NHS Orkney s Cancer and Palliative Care Service based in the Macmillan Unit in the Balfour Hospital in Kirkwall. We are looking for an experienced 1 st level registered nurse to join our specialist oncology and palliative care team. This small team of 3 clinical nurse specialists provides oncology and palliative care/support for patients and families predominantly affected by cancer but also other long term conditions throughout Orkney,. You will provide expert clinical care as part of the Macmillan Service team to patients presenting with a range of care needs both within the community and the unit as required. The post holder will be expected to deliver specialist oncology and palliative care interventions, including the delivery of cytotoxic therapy, as an autonomous and self-directed practitioner and ideally be an independent nurse prescriber. Applicants will be expected to have excellent communication and interpersonal skills and have evidence of postgraduate study in either cancer or palliative care. Orkney has an established and highly regarded service providing support and care for patients throughout the islands. Macmillan House is a small unit providing day case treatments alongside 4 in-patient beds for oncology and palliative care based within the Balfour Hospital. The team is based here providing outreach across the Orkney Islands. JOB PURPOSE The Macmillan Cancer and Palliative Care Nurse Specialist will ensure that a co-ordinated and timely specialist oncology nursing service is provided for referred patients receiving cancer care across both the hospital and community settings. The post holder will facilitate the development of oncology nursing interventions, including chemotherapy delivery. The specialist nurse will provide clinical expertise, including giving specialist oncology and palliative care advice to nursing staff, medical staff, and other associated health care professionals locally, outside agencies, and the patient and family. The postholder will provide professional leadership within own sphere of responsibility and will be expected to develop and implement educational programmes and participate clinical research as appropriate. The postholder will help promote and develop the Orkney Macmillan Service, whilst increasing public and professional understanding of the nature, scope and function of the Macmillan Service. 8. DIMENSIONS (Job Facts and Figures) NHS Orkney Board employs approximately 600 staff The post-holder is based within the 4 bedded cancer and palliative care unit at the Balfour Hospital that provides a cancer and palliative care service to the population of Orkney. The outpatient provision is coordinated from the unit in conjunction with the in patient service which is a 24/7 service. The unit is geographically remote from the mainland cancer centre and is supported by Consultant Oncologists, Haematologists and Consultants in Palliative Care by telephone and video conference links with Grampian services.

4 The post holder provides: - Specialist nursing to a caseload of patients requiring oncology interventions and support. This service is provided throughout Orkney, including: Macmillan House, the Balfour Hospital and Primary Care. - Specialist cancer and palliative care advice to other services throughout Orkney, when requested including GPs - Organisation of chemotherapy delivery in Orkney. Administers chemotherapy interventions to in-patients and out-patients as and when required and supports other staff in these activities. - Frequent communication with mainland cancer services to help plan individual patient treatments and support - Education and support to ensure that knowledge and skills are shared with clinical staff in the hospital and community. - An accurate and effective record-keeping system, collecting and collating statistics as required. - Support and leadership in the development of local, regional and national initiatives for cancer care frequently communicates with mainland cancer services to help plan individual patient treatments and support frequently participates in bereavement support, initiating further referrals when required - The Specialist Cancer and Palliative Care Nurse is required to demonstrate a range of abilities including expertise and knowledge in advance practice and clinical leadership to enable others healthcare professionals to develop expertise. - The core functions in this specialist-nursing role are clinical expertise, education, research and case management, contributing to multidisciplinary activities in either a hospital or a community setting. - The post holder is expected to function at an advanced nursing level, often autonomously, with the support of both the Scottish mainland cancer services and the local team in Orkney. It is essential that the post holder is at all times mindful of their requirements to facilitate the development of skills and knowledge of other health care staff in the area of cancer care. The post holder will also be required to work in other clinical areas in NHS Orkney, as service demands indicates and/or requested by the Senior Management Team in NHS Orkney. EQUIPMENT AND MACHINERY

7 The post holder is required to directly use and/or maintain resuscitation equipment, manual handling aids and all clinical equipment as required within the service. The Post Holder will be familiar with and use and the following equipment: Digital thermometers Oxygen and suction systems Syringe drivers IV infusion pumps Oxygen saturation monitor Blood pressure monitor Medisense Glucose testing machine Naso-gastic/PEG feeding system Basic Life Support Equipment Defibrillator machine Nebuliser machines Doppler ECG Sluice master 11b. SYSTEMS Local and national systems to record patient and staff information within the boundaries of local and national policies and legislation e.g. SSTS, Outlook,TRAKCARE and RIM. Maintains and develops clinical standards and documentation in accordance with NMC Code of conduct and assists in the development of new initiatives The post holder is required to be computer literate, be able to access current research and development and be competent in the skills required within the service area E.g. intra/internet skills. The post holder is required to report and investigate any incidents according to the Datix Reporting System 12. ASSIGNMENT AND REVIEW OF WORK This Job Description will be used as a basis for individual performance review between the post holder and the manager. Regular 1:1 meetings will ensure a regular review of work. The Job Description covers only the key result areas and, as such, does not intend to provide a comprehensive list of objectives. Specific objectives will be reviewed each year and may develop to meet the changing needs of the service. The post holder will need to take due account, in the way they achieve the key result areas, of Board policies and procedures. Professional Leadership will be provided by the Lead Nurse. The Board aims to maintain the goodwill and confidence of its own staff, service users and the general public. To assist in achieving this objective it is essential: The post holder is required to be a registered Nurse with current Nursing and Midwifery Council registration with at least 5 years of clinical experience with proof of CPD Previous experience working within Cancer and Palliative Care field as a registered nurse (Band 6) is essential for at least 2 years. The post holder should be able to insert venflons, nasogostric tube, urethral catheter, administer chemotherapy, look after patient with hickman/picc lines post chemotherapy/radiotherapy - If the post holder does not have above skills it would be essential to complete the core competence within 6 months 13. DECISIONS AND JUDGEMENTS The postholder must always work within the NMC Code of Professional Conduct The post holder will be required to use their own initiative and be able to make sound and rational decisions and remain focused and directed throughout the process.

8 The post holder requires the ability to remain objective and supportive of others at all times. The post holder will be required to use own judgment whilst observing patient s condition and should report any change in prescriptive care to relevant disciplines The post holder will be required to use rapid judgment in the assessment and treatment of patients efficiently and appropriately in all clinical situations. The post holder will be required to use safe and efficient bed utilisation due to the unpredictability of emergency admissions 14. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB The post holder will need to use a variety of communication skills effectively and professionally at all times. Utilising effective leadership skills in order to manage workload and develop a clinical environment which is positive and effective for both staff and patients. The post holder should have the ability to adapt to a variety of situations that they will be presented with as the patients conditions can change rapidly. The ability to constantly change priorities as the demands of the workload dictates. Constantly dealing with complex emotional and psychological difficulties associated with palliative and terminal care including bereavement 15. COMMUNICATIONS AND RELATIONSHIPS The main focus of communication for the post holder is to be the central point of contact and knowledge for all members of the multidisciplinary team to ensure that the optimum care is delivered to the patient. The post holder must therefore possess effective interpersonal skills and participate in cross boundary working. Internal - Senior Charge Nurse, patients, carers, members of the multi-disciplinary team: in particular the Intermediate Care Team, Community Nursing team, all Allied Health Professionals. - Act as a specialist resource to hospital and primary care teams, including GPs, community nursing teams and other agencies involved in caring for cancer and palliative care patients at whatever stage of the disease trajectory External - Skills and ability to deal with with distressed, upset, angry or anxious patients, carers, relatives and staff. - Use communication skills to de-escalate situations whilst ensuring the safety of the post holder, other patients and staff.

9 16. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB The post holder will require manual handling skills, which should be kept up to date yearly or more frequently if required. Can work within confined spaces as other medical equipment maybe required for monitoring and supporting the patient s condition. The post holder will also frequently come into contact with and is exposed to bodily fluids and other clinical waste. Will frequently deal with and manage cytotoxic substances. This post holder will also have to drive a car regularly as part of their duties which will include driving in difficult weather and road conditions at times. There is also a requirement to sit for periods of time administering bolus chemotherapy. There is a need for accuracy and appropriate maintenance in the use of all clinical and other equipment used particularly regarding the administration and delivery of medicines and treatments. Maintenance of approiate emergency care, ensuring that ILS training is updated at least annually. Emotional demands ; By nature of the work involved, and close working proximity of small teams, sensitivity, confidentiality and understanding of the balance of professional and personal roles can be challenging within small rural communities. The post holder will be required to deal effectively with limited resources, whilst also being required to prioritise own workload and adapt flexibly to competing demands from various people.
