Specialist area; Aseptics Deputy Accountable Pharmacist To deliver specialist high quality pharmaceutical care input to BGH patients by contributing to the pharmaceutical care of individual patients within the hospital through the dispensary and ward based clinical pharmaceutical service To assist in the delivery of high quality pharmaceutical services to patients across other areas of secondary care within NHS Borders. To assist in ensuring the safe, effective and cost efficient use of medicines throughout the organisation. To provide & develop clinical pharmacy & pharmaceutical care services to a specialist service area. To undertake risk management & ensure compliance with medicines legislation within area of specialty. To provide expert advice on pharmaceutical matters to specialist clinical areas. To provide education & training to pharmacy & other professional staff within the organisation & within the specialist area. To assist in ensuring the safe, effective & cost efficient use of medicines throughout the organisation. To support other specialist pharmacists within specialist clinical areas and provide absence cover To deputise for the Aseptics Accountable Pharmacist in the following areas: provide professional leadership and to be accountable for services provided by the Aseptic Unit set and maintain standards for Aseptic service in line with current national standards and guidelines and legal requirements and ensuring all documentation is implemented and updated service planning and development

SCOPE AND RANGE Responsible for supporting the Aseptics Accountable Pharmacist in the management of the Aseptic Service, provision of Aseptic Dispensing Services for wards at the Borders General Hospital and identified outpatients. This includes the supply of e.g. chemotherapy, Total Parenteral Nutrition; Intravenous additives, bladder installations, intrathecals and opiate syringes. The Deputy Accountable Pharmacist role as defined by the ASSIG profile for Aseptic Services Responsible for leading the hospital pharmaceutical service for patients within the specialty area. Responsible for pharmacy contribution to service planning for this group of patients, including monitoring of introduction & subsequent usage of new treatments. Responsible for pharmaceutical input to the management of the drugs budget for specialist area, out of total hospital drug budget of 3.0m. Responsible for clinical pharmaceutical service to the specialist area & other wards and clinical areas within Borders General Hospital (BGH), as necessary for inpatients and outpatients. Responsible for monitoring medicines use & formulary management, including adherence to local prescribing policies & protocols. Liaises with other clinical pharmacists within the pharmacy & throughout national networks within specialist area. Oversees the dispensary as part of the working day, including professional checking & query resolution. Pre-checking & final checking of aseptic & cytotoxic dispensing. Directs pharmacy staff including pharmacists and technicians (MTOs & ATOs) while involved in provision of service to specialist area. 5. MAIN DUTIES/RESPONSIBILITIES Support the Aseptics Accountable Pharmacist in managing, developing and maintaining the BGH Aseptic Dispensing Service within the appropriate national standards and guidance in order to provide a safe aseptic dispensing service to patients Lead, co-ordinate and develop patient-focused pharmaceutical input to patients in the specialist area to maximise benefit, and minimise risk, in the use of their medicines. To provide specialist advice to clinical areas on pharmaceutical standards, legal requirements, medicines management and other issues involving medicines. To ensure that current, safe, policies and procedures for medicines are in place, are followed and are reviewed regularly. To maintain and develop a system of re-use of patients own drugs and one stop dispensing service. To contribute to accurate medication history taking through discussion with patients, clinical staff, GPs and Community Pharmacists, as appropriate, to ensure seamless pharmaceutical care provision. To provide discharge planning (including medicine reconciliation) for medication-related issues, such as the assessment and provision of compliance aids, through liaison with Community Pharmacists and GPs, as appropriate. To counsel, and provide advice to, patients and carers on medication-related issues.

4 To support the dispensary team by supervising and checking work in the main dispensary and Aseptic Dispensing Unit. To provide professional advice for over the counter sales to staff and members of the public. To provide education and training to other healthcare professionals as required. To participate in weekend/holiday rotas as required, dispensing medicines and providing professional checks for prescriptions. Participate in the provision of BGH Pharmacists emergency duty service for NHS Borders hospital units, as required. To comply with NHS Borders and Pharmacy Department strategies and policies. To provide specialised advice to medical and other staff as well as to patients, their relatives and carers Pharmacy Department Based services o Professional clinical check of prescriptions. o Dispensing of prescriptions as required o Final Checking of dispensary and aseptic/cytotoxic dispensing as required o Clinical query handling and resolution o Supervision of OTC medicine sales Ward Based Services Provision of clinical pharmacy services to designated patient groups: o Verification of in-patient and discharge medication orders o Medication history taking o Documentation of care and risk management issues including: Patient allergy Patient self administration (including assessing patient suitability) Adverse drug reactions and interactions Individualising pharmaceutical care plans and documenting / resolving pharmaceutical care issues Verification of paediatric and neonatal doses o Counselling patients on the use of their medicines o Acting as primary contact point for ward staff in relation to pharmacy issues when providing ward pharmacy service o Liasing with ward staff and the pharmacy technicians to maintain and update ward stock lists o Provision of medicines information to clinical staff and patients o Provision of clinical pharmacy to other clinical areas as required CPD/Training commitment Update current skills and knowledge in relevant practice areas as required. Undertake at least the minimum CDP commitment recommended by the Royal Pharmaceutical Society of Great Britain. Participation in departmental continuing personal development programme including PDP as appropriate Participation in internal or external courses/meeting as appropriate. Assisting in the training of other pharmacy staff in particular:

SYSTEMS AND EQUIPMENT Requires in-depth knowledge of all the physical aspects of the Aseptic unit and equipment including air handling plant, fabric of the unit, refrigerators, isolator cabinets, clean room clothing, sundries, safe management systems etc. Use of ASCribe pharmacy stock control and dispensary computer systems Stock control of pharmaceuticals Dispensing of medicines for patients Medicinal charts for patients Use and creation of management reports Create/updating of patient records Use of Microsoft Office suite (internal and external) Internet enquiries use of specialist medical/pharmaceutical databases etc Word processing, policies, notices, forms etc Spreadsheets; Management information, statistics etc PowerPoint Presentations - Presentation of management information, CPD Use of medicines information software & CD-ROM databases. - internet databases - e-library Responsibility for the security of pharmaceuticals, includes security of keys and use of alarm system Use of telephone Use of Pharmacy aseptic equipment: Isolators, clean room clothing, syringes, needles, QC monitoring equipment etc 7. DECISIONS AND JUDGEMENTS The postholder is a member of the professional staff of the pharmacy department and will work within strategic and policy guidelines established by the NHS Borders. The postholder is a senior independent practitioner who contributes to pharmacy strategies relating to their specialist area. The postholder has responsibility for managing their allocated workload within specialist area without supervision but will have close links with the BGH Aseptics Accountable Pharmacist and the Lead Clinical Pharmacist. Uses knowledge and experience to assimilate, and evaluate, often conflicting information in order to advise on pharmaceutical matters

6 Annual objectives will be agreed jointly with the BGH Lead Clinical Pharmacist/Aseptics Accountable Pharmacist and review of performance will be carried out by BGH Lead Clinical Pharmacist/Aseptics Accountable Pharmacist. 8. COMMUNICATIONS AND RELATIONSHIPS The postholder develops and maintains communications within & without the organisation Communicate directly with: Colleagues from all disciplines & professionals within NHS Borders (medical, nursing, AHPs, GPs, community & senior pharmacists ) Middle and senior managers on pharmaceutical service and policy issues. Pharmacy staff pharmacists, technicians (MTOs & ATO s) and A&C staff Members of clinical professions (medical, nursing, AHPs etc) on individual clinical matters Patients at in- or out-patient situations clinical issues, service issues, queries and complaints Public service issues and queries Pharmaceutical industry at local or national level for information and negotiation. Nationally with pharmacists & other healthcare professionals in the Scottish/UK national networks/special interest groups etc Represent NHS Borders and Pharmacy at meetings of various groups both locally & nationally Communication will include face to face, telephone, written, , meetings, presentations. 9. PHYSICAL DEMANDS OF THE JOB Combination of walking, standing, sitting, driving with occasional light physical effort General keyboard skills High degree of precision & accuracy for dispensing & checking, often with strict time constraints. Clinical work requires a high level of concentration characterised by assessing different pieces of sometimes conflicting information and forming a clinical opinion. This work is often conducted under time pressures and subject to frequent interruptions. Frequent levels of high concentration, often with interruptions when checking dispensing Near patient contact involving exposure to environmental risks at ward level. Potential exposure to toxic pharmaceutical materials Validated aseptic techniques including working in isolators with restricted movement (complex manipulations with arms in glove parts of isolator fixed working position) Emotional effort - - occasional exposure to distressing or emotional situations, both in case of patients & staff

MOST CHALLENGING/DIFFICULT PARTS OF THE JOB To provide expert pharmaceutical advice to specialist area To provide high quality pharmaceutical care tailored to meet the needs of individual patients. This requires continual updating of clinical pharmacy knowledge base. To ensure the safe, efficient & economical use of medicines including the delivery of individualised patient care. To effectively prioritise the workload to meet the needs of users (patients, colleagues, managers) within the resources available To maintain & improve working relationships within the specialist area To provide high quality, timely, responses to challenging Medicines Information queries, within currently available resources

8 Essential Desirable 1 Qualifications Masters Degree in Pharmacy Registered with General Pharmaceutical Council (GPhC) Member of the Royal Pharmaceutical Society of Great Britain 2 Experience Evidence of commitment to Continuing Professional Development Post qualification experience working as a clinical pharmacist, or equivalent experience. Experience in a hospital pharmacy department at Band 6 pharmacist level. Experience of data interpretation and clinical audit of prescribing. Experience of hospital aseptic and cytotoxic dispensing and management Able to effectively interpret medical case notes when performing audits in relation to patients condition and the medication prescribed 3 Skills Therapeutic knowledge and practical skills for dealing with a complex range of pharmaceutical interventions. Expertise in pharmaceutical standards & guidelines pertaining to hospital pharmacy Clinical pharmacy and care planning skills. Ability to prioritise own workload Working knowledge of standard computer programmes and of clinical and pharmacy systems. Aseptic dispensing skills and knowledge of principles of aseptic dispensing and management of an Aseptic Dispensing Unit. Knowledge of NHS Borders policies and procedures and a detailed knowledge of pharmacy policies and procedures. Experience of the Scottish Patient Safety Program (SPSP) model for improvement and associated improvement tools and methods. Knowledge of the SPSP Medicines Management workstream.
