Join this dynamic organization and drive your career forward Competitive Salary Package arrangements are available This role Reporting to the Senior Social Health Clinician will support IUIH to deliver Mental Health, Social, and Emotional Wellbeing Transitional Care Service for Aboriginal and Torres Strait Islander Men in the Woodford Correctional Centre as part of a new contract with the Metro North Health and Hospital Service (MNHHS) The Transition Care Coordinators support the direct implementation of the Transition Care Service, effecting the planning and coordination of transitional needs for men in their last 6 weeks of custody and supporting their transition from custody to the community. In doing this, you will: Coordinate activities contributing to the ongoing design, development, and implementation of the IUIH Prison Transitional Service Work with the clients and Indigenous Mental Health Intervention Program (IMHIP) to jointly develop a transition care plan that will identify and address the client s mental health, physical health and social support needs for the first two weeks upon release Provide assistance for the client to make contact within the first month of release with a primary care provider able to resume and / or take on regular, ongoing care for the client To be successful in this Senior Prison Transitional Care Coordinator role, you will have: Qualification in Social Work / Mental Health /Behavioural Science or related field At least 5 years experience working in the field of mental health, drug and alcohol services, or related social service fields Eligible to obtain entry into Queensland Correctional Services This will mean you need to have: Ability to work collaboratively and constructively with a wide range of service providers and community supports Capacity to work ethically and effectively within a culturally responsive framework Demonstrated leadership qualities, skills and experience In return, you will be rewarded with the opportunity to develop your career within this organisation that truly values its people. 3 POSITION OBJECTIVE IUIH has been contracted by the Metro North Health and Hospital Service (MNHHS) to deliver Mental Health, Social, and Emotional Wellbeing Transitional Care Service for Aboriginal and Torres Strait Islander Men in the Woodford Correctional Centre. The Transition Care Coordinators support the direct implementation of the Transition Care Service, effecting the planning and coordination of transitional needs for men in their last 6 weeks of custody and supporting their transition from custody to the community. The Target Population for this service is Aboriginal and Torres Strait Islander men in custody in the Woodford Correctional Centre and who: are aged 18 years and over. have received the IMHIP service whilst in custody and have been offered the transitional care service support and consented to participate GENERAL RESPONSIBILITIES The roles and responsibilities of this position include, but are not limited to: Program Leadership Coordinate activities contributing to the ongoing design, development, and implementation of the IUIH Prison Transitional Service Provide a point of communication on a day to day basis for the MNHHS IMHIP team regarding for the operation of the IUIH service, ensuring a timely response to any matters of Ensure the IUIH Prison Transitional Service is meeting its contractual obligations to Metro North HHS, working within allocated budget and providing timely reports on service activity and outcomes as required Communicate regularly with IUIH and Member Service Managers and Team Leaders, in particular in the IUIH Connect, Clinic, and Family Services areas, to build a sound understanding of the Service and to ensure that strong pathways for connection and referral are in place for men as they transition out of prison Communicate regularly and effectively with external health, social support and community stakeholders both inside and in relevant areas outside of SEQ to build an understanding of the IUIH Transitional Care Service and to forge pathways for connection and referral outside of IUIH and member service networks Provide direct support for the IUHI Transitional Care Support Worker(s), ensuring staff are working to maximum effect and within their scope of practice, and are accessing structured IUIH Clinical Supervision Provide leadership in continuous quality improvement of the IUH Transitional Care Service, ensuring the Service engages in active processes for reflection and monitoring, review and analysis of key data including client and community feedback, implementation of service refinements in response Transition Planning and Coordination With client consent, receive referrals from the Indigenous Mental Health Intervention program (IMHIP) Work with the client and IMHIP to jointly develop a transition care plan that will identify and address the client s mental health, physical health and social support needs for the first two weeks upon release Where consent is granted to make contact with the client s family assess and undertake planning with the family well in advance to prepare for the client s release Work with the client and IMHIP to update the Stay Strong Plan near release Ensure the client s Growth and Empowerment Measure (GEM) assessment has been completed and is up to date
4 Establish the IUIH multidisciplinary Transition Care Coordination Team tailored to the needs of the client identified in the client s transition care plan and identify the client s community-based Case Manager Communicate regarding the client s pending release with the various IUIH programs and services and/or external organisations that will be involved in the client s care in the first six months postrelease Client handover and transition into community Provide assistance for the client to make contact within the first month of release with a primary care provider able to resume and / or take on regular, ongoing care for the client Link all clients with appropriate health, mental health, social and other support services as identified and recorded in the client s transition plan, and in keeping with their Stay Strong Plan Maintain regular contact with client for six months post-release, either directly or through an assigned local case manager providing follow through / continuing care and support for the client Review and update the client s Stay Strong Plan and GEM at 3 and 6 month intervals, either directly or in conjunction with the assigned local case manager Record Keeping and Reporting Maintain timely and accurate records of client contacts and care Ensure all client data is obtained and maintained in line with relevant privacy legislation, and in particular in keeping with IUIH Policies and Procedures governing the collection, storage, maintenance and release of personal health information Provide reports as required by Management for submission to MNHHS, in line with contractual requirements Professional Development and Team Contribution Participate in targeted training provided by QH / IMHIP, IUIH or other providers to build job-specific skills and capability Actively participate in a program of clinical and cultural supervision Participate in the IUIH performance review process Commit to ongoing professional development and maintenance of professional competencies Contribute to effective team dynamic and performance Promote a safe and healthy work environment Develop and Maintain Linkages Work closely with the IUIH Connect team, tapping into an extensive existing network for connection and referral Actively foster productive working relationships with staff in the Woodford Correctional Service and specifically with the IMHIP team Foster productive working relationships with other IUIH staff and Business Units Develop relationships with key relevant providers and linkage agents outside of SEQ Please note that the duties outlined in this position description are not exhaustive, and only an indication of the work of the role. The organisation can direct you to carry out duties which it considers are within your level of skill, competence and training. SELECTION CRITERIA At least 5 years experience working in the field of mental health, drug and alcohol services, or related social service fields Experience working with Aboriginal and Torres Strait Islander clients and families Capacity to work ethically and effectively within a culturally responsive framework Demonstrated leadership qualities, skills and experience Highly effective communication skills
