1 ABORIGINAL AND/OR TORRES STRAIT ISLANDER HEALTH WORKER Ongoing, full time Moreton ATSICHS, which is operated by IUIH, has a number of clinics across the Moreton region including Morayfield, Strathpine, Deception Bay and Caboolture which provide comprehensive primary health care services to an expanding Aboriginal and Torres Strait Islander population. The clinics provide general health screening and provides for a comprehensive range of chronic disease management activities supported by a multidisciplinary team. As part of the multidisciplinary team, is a range of Allied Health professionals providing support to clients in accordance with GP management plans and their needs and requirements. Located at MATSICHS Strathpine, the role is a member of a multidisciplinary team, and will be responsible for delivering a high standard of client services and activities in line with the requirements of health assessments (initial and follow-ups) in alignment to the IUIH Model of Care and MBS requirements. Key Duties Provide health screening for clients and families including but not limited to: routine clinic observations; comprehensive health screening as the first stage of an annual Preventive Health Check (item 715) and scheduled screening prompted by the MMEx to do list, within scope of practice Receive and act on 81300/10950 EPC Referrals to provide specified follow-up services for clients and families who have completed comprehensive preventive health assessments (Item 715) and / or GPM Ps / TCAs Work collaboratively and holistically within a multidisciplinary team environment to engage clients and families in the management of their wellbeing from a social, cultural, physical, and emotional perspective Provide education for clients and families on the roles of various providers involved in their care, reasons for referral and importance of follow up care Key Requirements Minimum Cert III in Aboriginal and Torres Strait Islander Primary Health Care, and/or Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice), or willingness to work towards completion of qualifications Demonstrated ability to communicate, engage effectively and work together, with Aboriginal and Torres Strait Islander peoples and interpersonal skills that demonstrate your focus on client outcomes Demonstrated clinical experience with the ability to provide follow-up care services for clients and families who are identified as being at risk and/or have a chronic condition Enquiries and Expressions of Interest (resume and brief covering letter addressing the key requirements above) can be directed to Layla Scott, Practice Manager by at layla.scott@iuih.org.au Your application (Resume and brief covering letter addressing the key requirements above) must be submitted via Seek.com.au APPLICATIONS CLOSE MONDAY 16 OCTOBER 2017

The IUIH is a lead agency working in partnership with key stakeholders to support the effective implementation of the Council of Australian Governments Closing the Gap initiatives. IUIH established MATSICHS in 2011 as a strategic response to the significant growth and geographic dispersion of the Aboriginal and Torres Strait Islander peoples in the Moreton Bay area. We respect and empower the individual, the family and the community, to foster linkages with our Clinics; Home Care Support and Family Wellbeing Services. Each of the four (4) MATSICHS clinics are located in areas which are close to, or easily accessible to, Aboriginal and Torres Strait Islander people. The role of each Clinic is to provide comprehensive, holistic health care and a one stop shop for primary health care, to our clients. We identify that Closing the Gap is a major aim of the work that we do, and ensure that referral pathways from primary health care to allied and specialist health practitioners provides the necessary health care to the health and wellbeing of the community. Collaboration Collaboration with and among Community Controlled Health Services and partners is fundamental to realising our vision in SEQ. In line with the regionally endorsed Smoking Cessation Protocol, provide comprehensive assessment, brief intervention advice, and intensive follow
4 up and support for clients wanting to quit Provide assessments of lung health on referral, including performance of PEFR and / or spirometry Undertake venesection and point-of-care testing as requested by the GP and in accordance with own competencies and scope of practice Actively maintain knowledge of the application of the MBS relevant to your job role; ensure all EPC follow-up visits are compliant with the referral and that recording and reporting of follow-up services meets the criteria for MBS claiming Where barriers exist for clients and families in attending the clinic, provide services in the home or community in keeping with scope of practice, and in accordance with the IUIH Home Visiting Policy guidelines Education and promotion of health literacy Provide education for clients and families on the roles of various providers involved in their care, reasons for referral and importance of follow up care Support clients and families with information, resources and advice in response to specific requests and / or identified need Working closely with the Community Liaison Officer, identify opportunities for individual, family and community education to build knowledge about wellbeing including healthy life choices, risk factors, chronic conditions, and the breadth and availability of health care and related services Collaborate Work collaboratively and holistically within a multidisciplinary team environment to engage clients and families in the management of their wellbeing from a social, cultural, physical, and emotional perspective Provide support and guidance to non-aboriginal/torres Strait Islander Colleagues on cultural ways/societal factors which might influence or require consideration when working with clients and families Service development and improvement Actively participate in regular clinic meetings, case conferencing continuous quality improvement meetings and activities in the clinic Identify opportunities for contributing to regional service development and quality improvement, through regional Aboriginal and Torres Strait Islander Health Worker forums, Lead Clinician Group meetings, and other forums Drawing on direct input and feedback from clients, families and community, along with information drawn from the electronic Patient Information Management System, contribute to the understanding of family and community needs from a social, cultural, physical and emotional perspective Professional development Actively participate in continuing education opportunities provided within the organisation Actively identify opportunities for contributing to regional service development and quality improvement, through regional Aboriginal and Torres Strait Islander Health Worker forums, Lead Clinician Group meetings,
5 and other forums Participate in scheduled performance appraisals; undertake further training and skills development identified for optimum ongoing performance in the job role Maintain currency of CPR / First Aid certification Self-Management & Team Contribution Maintain confidentiality in line with Code of Conduct & comply with all Corporate Policies and Procedures Participate in the performance review process, commit to own self development, maintain professional competencies Contribute to process improvement Contribute to effective team performance Assist with induction and other training for other team members, as required Promote and ensure a safe and healthy work environment Work within a legal and ethical framework Develop and Maintain Linkages Foster productive working relationships with other IUIH and MATSICHS staff in the delivery of seamless support services Work to foster productive working relationships with relevant government departments, Queensland Health and other specialist providers Develop and maintain effective relationships with the communities in the target areas Legal Obligation: All relevant health professionals who, in the course of their duties, formulate a reasonable suspicion that a child or young person has been abused or neglected in their home/community environment, have a legislative and a duty of care obligation to immediately report such concerns to the Department of Child Safety Please note that the duties outlined in this position description are not exhaustive, and only an indication of the work of the role. SELECTION CRITERIA Key Requirements A demonstrated ability to communicate, engage effectively and work together, with Aboriginal and Torres Strait Islander peoples and interpersonal skills that demonstrate your focus on client outcomes Commitment to the philosophy and practice of Aboriginal Community Control Sound knowledge health issues which influence the wellbeing of Aboriginal and Torres Strait Islander people and a strong commitment to improving health and wellbeing outcomes Sound knowledge of local Aboriginal and Torres Strait Islander community networks Demonstrated high level communication skills and confidence in interpersonal communication Demonstrated ability to provide follow-up care services for clients and families who are identified as being at risk and/or have a chronic condition Ability to recognise individual learning needs and to adapt messages and approaches accordingly Demonstrated skill working as a member of a multidisciplinary team Self-motivated with capacity to manage own workload with limited supervision Computer skills able to work with electronic client record system Demonstrated ability to meet targets and performance outcomes. QUALIFICATIONS / EXPERIENCE Minimum Cert III in Aboriginal and Torres Strait Islander Primary Health Care, and/or Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice), or willingness to work towards completion of qualifcations Previous experience working with an electronic Patient Management System Current First Aid Certificate
