The Regional Services Unit provides direct clinical services, contracted Allied Health professional services, programs and care coordination support to IUIH and Member Services to improve access for local Aboriginal and Torres Strait Islander peoples to a comprehensive and effective range of extended Health services and clinical programs. The positions main responsibilities are: to assist IUIH program staff and clients involved in the Work it Out Chronic disease rehabilitation program as require. to support health initiatives within the Aboriginal and Torres Strait Islander communities of South East Queensland. Role Responsibilities Provision of Care In accordance with Care Plans and in conjunction with other key staff members in the clinic and across IUIH sites: Assist and support allied health programs and activities at IUIH premises. This

3 may include hydrotherapy (at pools and swimming facilities) and/or exercise programs (at gyms, parks etc.). Assist and support the delivery of information sessions with individuals, families and other groups, such as at schools and community events on: o exercise and exercise programs o maintaining healthier lifestyle choices o chronic diseases and chronic disease management Encourage and assist with participation of clients and/or assist and facilitate participation of clients at community events Provide basic information to clients for referral onto Aboriginal Health Checks Refer people to existing lifestyle modification programs and other health programs as appropriate Collect data to assist in the evaluation of the COAG National Partnership Agreement on Closing the Gap in Indigenous Health Outcomes Assist in evaluations and reports on capacity and risks as required Perform other duties as agreed Care Planning and Administration The position will assist others in the: planning, implementing, reviewing and evaluating programs reviewing client care plans and update client information when required Checking equipment regularly and report any faulty equipment to Administration/Maintenance Manager Complete daily/weekly documentation. Recording, electronically and where applicable, manual records in client s record books, all instances of planned and unplanned service delivery Assist in conducting client, carer and program surveys on a periodic basis or as stated in the policy and procedure manual Attend program and participate in meetings as requested Collect data and feedback to assist in the evaluation of activities Prepare ad hoc reports as required. Relationship Management Foster productive working relationships with other IUIH staff in the delivery of seamless support services Encourage cross-functional collaboration to achieve the best outcome for the organisation. Wear/use and maintain/care for personal protective clothing and/or equipment provided and report any defects. Use lifting equipment and assistive devices as required. Participate in rehabilitation process if injured or ill due to work. Demonstrate leadership in IUIH s Work Health Safety goals and objectives remaining complaint with WHS Legislation and IUIH s policies and procedures. Carry out your duties safely in accordance with your ability and competence. Legal Obligation All relevant health professionals (including registered nurses and medical officers) who, in the course of their duties, formulate a reasonable suspicion that a child or young person has been abused or neglected in their home/community environment, have a legislative and a duty of care obligation to immediately report such concerns to the Department of Child Safety. Other duties consistent with the position where required and/or requested by your Manager. IUIH can direct you to carry out duties which it considers are within your level of skill, competence and training at any time. Role Requirements Essential Incumbents are required to obtain and maintain a Current Blue Card (working with children and young people) and a Satisfactory Criminal History Check. Any changes to criminal history should be advised to the supervisor as soon as practicable. Current First Aid Certificate Some work out of normal hours of duty may be required Travel across South East Queensland is required to undertake activities at rostered/scheduled activities Current C Class Drivers Licence (Qld) essential Physical fitness to carry out duties allocated Qualifications and Experience Certificate III in Allied Health and/or other relevant qualification (or willingness to complete) Desirable Aboriginal and Torres Strait Islander persons. An understanding of the health

5 and social issues impacting upon indigenous people in the area Knowledge of WPH&S regulations in relation to cleaning and chemicals Demonstrate good time management skills - staff must have the ability to work effectively to meet deadlines and work within timeframes set Be able to complete tasks unsupervised Demonstrate respect and understanding of client issues including cultural appropriateness and anti-discrimination Demonstrate good oral and written communication skills Ability to complete required paperwork Ability to exercise at all times a Duty of Care when working with clients
