TAIHS's main operating income comes from grants from Commonwealth and State funding bodies. Our main funding bodies are the Commonwealth Department of Health, the Commonwealth Department of Prime Minister & Cabinet, the Queensland Department of Communities, Child Safety and Disability Services, and Queensland Health. Funds are also generated through Medicare billing of patient services. TAIHS provides a holistic primary health care service incorporating a bulk-billing general practice medical clinic, a dental clinic, an individual support and advice service, mental health counseling, community health screening and health promotion activities. TAIHS also provides a range of community services, including crisis accommodation, child safety and early family intervention programs, and a specialist service for young people. In addition to the operation of comprehensive primary health services and the provision of a range of community services, TAIHS seeks to improve health and social outcomes for the community through; strategic partnerships, advocacy for needs, promotion of awareness and education in improved health and social practices, promotion of knowledge and understanding of underlying issues and unique needs and development of the evidence base. Our Vision TAIHS More than a Health Service is to strive to provide a truly comprehensive model of care that responds to the physical, social, emotional, cultural and spiritual needs of our people The Supported Bail Accommodation Service (SBAS) will provide a home-like environment for young service users, which will include structured and unstructured programs and activities. All young people bailed to the SBAS will be on the Conditional Bail Program, meaning that Youth Justice will be the lead case worker for all young people at the SBAS. Page 1 of 4

2 The target group for the Service is young males aged years, who are at risk of remand due to unstable accommodation, who have been granted bail with the condition of residing at the SBAS. At times there may also be young women aged years bailed to the SBAS. The practice framework focuses on the importance of building relationships as being foundational in supporting the social and emotional wellbeing needs of the client; with the Staying Strong framework aiming to provide guidance and practical steps toward social and emotional wellbeing, in a practical and culturally responsive manner. Purpose of Position The role of a Shift Supervisor (Case Worker) is to take on the role supervision and incident management outside of business hours, under the advice, supervision and support of the Program Manager. They will also be responsible for the provision of appropriate care; client programs and activities, transport, mentoring and support. Including the performance of all duties required to ensure the operations of the service are in line with the programs duty of care, practice framework established protocols and the service agreement. This position will be work predominately as shiftwork outside of normal business hours and including some weekends. Key Responsibilities and Performance Indicators AREA OF RESPONSIBILITY Operations: In accordance with agreed Procedures and Protocols: RESPONSIBILITIES Provide supervision, support and guidance to the Case Workers while on shift Report issues in relation to staff engagement interaction and/or performance to the program Manager Manage incidents and issues as per TAIHS policies/procedures and protocols with the advice and direction of the Program Manager Complete all required reporting and requirements including incident reports, registers and logs Participate in case planning and case management Liaise with the Senior Case Worker and Youth Justice Case Workers in relation to all client related matters; including client needs, engagement, issues, observations KPIs Supported and clear direction given All issues/concerns documented and reported Incidents managed as per direction and protocols All documentation completed as required Documented participation Documented communication in relation to all clients of the service Page 2 of 4

3 Administration Self-Management & Team Contribution Quality and Compliance Foster Linkages or concerns Develop and maintain effective relationships with clients, nurturing and supporting clients while assisting in the development of their skills needed to engage in non-criminal activities. Provision of information, support and advocacy for clients accessing the service. Provide feedback or participate in strength and needs assessments Ensure that dangerous objects, alcohol, illicit drugs or potential weapons are not brought into the place of safety, and that prescribed medications are stored securely Participate in the delivery of programs both structured and unstructured aimed at meeting the needs of the individual or group Maintain necessary client and operational information, including log books and case notes, on a daily basis Ensure secure management of all program files and compliance with applicable privacy legislation and policies Participate in the performance review process (PP&R) Follow all safety procedures and contribute to a safe work environment Comply with all legal, ethical, confidentiality and regulatory requirements Attend relevant training such as workshops on self-care and case management Perform work in accordance with organisational policy, process control plans and work instructions Contribute to continuous quality improvement, eg identify improvement to policy, work instruction, or the work environment Work to ensure effective case liaison with other program workers and other involved providers Demonstrated and documented positive engagement with service clients. Case notes and required documentation completed accurately and as Participation or input documented Conducts safety and security checks as directed or required completed as per TAIHS protocols and procedures. Accurate records/registers are maintained Programs delivered and participation recorded All documentation completed as required and within the required timeline All information is managed as per TAIHS policies and procedures PP&R completed annually Demonstrated compliance Risk feedback documented on LOGIQC system Demonstrated compliant ethical work practice Attendance and participation recorded Demonstrated understanding and active practice within TAIHS framework Feedback documented on LOGIQC system Documented case liaison Page 3 of 4

4 Work to foster productive relationships with all TAIHS Community Services staff Develop productive working relationships with relevant agency staff, Queensland Police Service and Youth Justice Effective internal collaborative working relationships are in place Collaborative relationships in place and interactions/activities are documented Please note that the duties outlined in this position description are not exhaustive, and only an indication of the work of the role. TAIHS can direct you to carry out duties which it considers are within your level of skill, competence and training. Required Qualifications and Experience Qualification/s relevant to the field of case management and/or youth work or a related field of discipline, with an appropriate Diploma (or a Cert IV with a minimum of 2 years experience) relevant to the work required to be performed; eg Diploma or Cert IV level in Community Services or equivalent Previous experience in a relevant industry, service or an equivalent level of expertise and experience to undertake the range of activities required with the willingness to undertake studies to gain the required minimum qualification Have an understanding of the historical, social, cultural and economic issues effecting the Aboriginal and Torres Strait Islander communities and young people today Demonstrated experience/positive engagement when working with persons of Aboriginal and/or Torres Strait Islander backgrounds and an understanding cultural competency Ability to operate within a case management model Ability to work with a team in ways that are respectful, transparent and acknowledging of individual s skills, abilities and capacities Ability to display initiative, self-motivation, time management and solution focused skills Ability to demonstrate strong capabilities around building relationships with and motivating vulnerable people, assessing needs and advocating for the client Practical Requirements Work out of normal hours of duty may be required Travel across Region may be required Current C Class Drivers Licence (Qld) Current Blue Card (working with children and young people) or willingness to apply for a Blue Card Current Queensland Notice of Suitability or willingness to apply Satisfactory Police Check no Serious/Criminal/Court Record (if required) Selection Criteria Duties & Responsibilities Satisfactory capability and/or potential to execute the key responsibilities of the position Page 4 of 4
